Multiple facial angiofibromas treated with high-frequency equipment.

Tuberous sclerosis is a rare genetic disease with autosomal dominant inheritance, associated with multiple hamartomas in several organs, such as the brain, skin, lung, kidney, heart and eyes. The authors of this study report a case of a 30 years old female patient with tuberous sclerosis, presenting multiple angiofibromas on face treated with high frequency equipment (radiofrequency), and discuss the therapeutic options for treatment of individuals with extensive cutaneous involvement in tuberous sclerosis.

Anaesthetic management of patients with Xeroderma pigmentosum. A series of three cases.

Xeroderma pigmentosum is a rare autosomal recessive disorder with clinical and cellular hypersensitivity to ultraviolet radiation and defective DNA repair. Skin cancer, mainly on the face, head or neck is very common. Inhalational anaesthetic agents and muscle relaxants are best avoided due to the possibility of inducing DNA damage and prolonged effects of muscle relaxants. These patients may have a difficult airway. These patients may have immature brain development which may render them sensitive to synergistic effect of benzodiazepines and opioids as seen in the first case. Total intravenou

The nonrandom distribution of facial hemangiomas.

OBJECTIVE: To map sites of occurrence of facial infantile hemangiomas and correlate these with pattern of tumor growth, clinical complications, and proximity to structural and developmental landmarks. DESIGN: A retrospective medical record review of 205 patients diagnosed with facial infantile hemangioma. SETTING: Arkansas Children's Hospital, Little Rock, a 250-bed teaching hospital affiliated with the University of Arkansas for Medical Sciences. PATIENTS: Based on their clinical photographs, 232 of the hemangiomas were mapped on a facial schematic. Each lesion was encoded with a number re

Evaluation of the Clinical Effects of Intraoperative Facial Nerve Repair and Reconstruction for Parotid Malignant Tumors.

OBJECTIVE: To evaluate facial nerve reconstruction's impact on quality of life (QoL) and clinical outcomes in radical parotid malignancy surgery. METHODS: Patients were stratified into the Preservation Group, Reconstruction Group, and Disconnection Group, retrospectively analyzed objective measures (from medical records and operative reports) and subjective measures (using the Sunnybrook Facial Grading System [SFGS] and Facial Clinimetric Evaluation [FaCE] scale) across the three patient groups. RESULTS: No significant differences in recurrence or mortality were observed between Reconstructi

Immediate Hypoglossal-Facial Anastomosis in Patients With Facial Interruption.

Hypoglossal-facial anastomosis provides excellent motor supply to the mimetic muscles of the face when there is no chance of recovery of the damaged facial nerve. However, to achieve optimal results, the timing of facial nerve surgery based on electrophysiological testing and clinical evaluation requires close follow-up of the patient. Functional results after delayed surgery are not predictable and depend on the number of surviving fibers, type of injury, severity of damage, degree of infiltration of inflammatory cells, and local fibrosis. Facial hypertonia, synkinesis, and involuntary mass m

Masseteric nerve transfer for short-term facial paralysis following skull base surgery.

BACKGROUND: Nerve transfers have been widely used to reanimate paralyzed facial muscles after irreversible proximal injuries to the facial nerve. The author has developed a technique involving masseteric nerve transfer combined with cross-facial nerve grafting for treating skull base surgery-induced facial paralysis. This paper aims to demonstrate that this procedure is effective and causes negligible donor site morbidity. METHODS: Seven patients who developed facial paralysis after the removal of skull base tumors were treated with masseteric nerve transfer combined with cross-facial nerve g

Facial paralysis reconstruction in children and adolescents with central nervous system tumors.

Facial paralysis remains a vexing problem in the treatment of posterior cranial fossa tumors in children. Fortunately, current techniques are available to reconstruct the paralyzed face in restoring balance, symmetry, and amelioration of functional sequelae. The restoration of structure and function of the paralyzed face is tantamount to proper social integration and psychosocial rehabilitation. In addition, the facial nerve is important in preventing drying of the eyes, drooling, and speech abnormalities, among other functions. The most visible evidence of facial paralysis is stark asymmetry,

Quantitative assessment of the variation within grades of facial paralysis.

A completely objective, unambiguous outcome measure of facial function is now available. A new automated computer-assisted clinimetric system combines the crucial detection capabilities of the human observer and the unique capacity of the computer to quantify the image light reflectance difference observed during facial expression. The new system was applied to 27 patients with a variety of diseases affecting the facial nerve. All subjects could be individually and objectively ranked, and disease-specific profiles could be constructed. These tasks are not possible with the House-Brackmann scal

Operative management of acoustic neuromas: the priority of neurologic function over complete resection.

The objective of surgical management of acoustic tumors is to remove them entirely and preserve facial nerve function and hearing when possible. A dilemma arises when it is not possible to remove the entire tumor without incurring additional neurologic deficits. Twenty patients who underwent intentional incomplete surgical removal of an acoustic neuroma to avoid further neurologic deficit were retrospectively reviewed. They were divided into a subtotal group (resection of less than 95% of tumor) and a near-total group (resection of 95% or more of tumor) and were followed yearly with either com

Facial nerve monitoring in acoustic tumor surgery.

Anatomic and functional preservation of the facial nerve during acoustic tumor surgery remains a primary goal. Intraoperative electromyographic facial nerve monitoring with auditory feedback has enabled the surgeon to more readily achieve this goal. We compared a group of monitored translabyrinthine acoustic tumor removals (N = 89) to a similar unmonitored group (N = 155) in regard to facial nerve function. Function was assessed immediately postoperatively, at time of discharge, and at 1 year postoperatively using the House six-point scale. Results were grouped as satisfactory, intermediate, o

Advances in Immunotherapy and Periocular Malignancy.

Immunotherapy has significantly advanced the field of oncology in recent decades. Understanding normal immunosurveillance, as well as the ways in which tumor cells have evolved to evade it, has provided the knowledge for development of drugs that allow one's own immune system to target and destroy malignant cells (immunotherapy). Cutaneous malignancies are particularly sensitive to this class of drugs. In a very sensitive anatomic region such as the periocular tissue, where surgical excision may come with significant morbidity, this technology has had a strong impact in the successful treatmen

[Lupus vulgaris manifestation as a destructive nose and facial tumor].

Lupus vulgaris is the most frequent manifestation of cutaneous tuberculosis, but in Europe it is limited to isolated cases. Mainly immunocompetent individuals are affected by this result of an endogenous reinfection on a lymphogenous-less frequently hematogenous-pathway. Lupus vulgaris has been observed to develop in more than 50% of all patients who already suffer from other manifestations of tuberculosis. The development of a squamous cell carcinoma in the lupus vulgaris is a rare complication; therefore, lupus vulgaris is deemed a facultative precancerosis.A 68-year-old female Serbo-Croatia

[Metastasizing basalioma of the temporal area or continuous extension? Surgical approach and results].

By means of histological sections the rare diagnosis of a basalioma of the temporal area with regional metastases in the parotid gland and in the neck could be verified. Subsequently the surgical treatment and the postoperative results are described. The operation consisted of an en bloc excision of the basalioma, exstirpation of the parotid gland and radical neck dissection. Since the tumor had also infiltrated the facial nerve, the nerve had to be sacrificed and reconstructed using the superficial peroneal nerve. In order to obtain immediately a satisfactory esthetic result in the face regio

[A case of intra- and extracranial development of a craniofacial teratoma. Excision during the neonatal period through a mixed cranio-facial approach (author's transl)].

A newborn infant was found to have a laterocervical swelling invading the floor of the mouth. Exploration of the mass revealed that there was invasion of the left side of the soft palate and, more particularly, a prominence in the temporal fossa causing an obvious cranio-facial asymmetry. Radiological and neuroradiological investigations demonstrated the presence of a tumor destroying the greater wing of the sphenoid, invading the left side of the temporal fossa, extending into the pterygomaxillary fossa up to the soft palate, and finally appearing exteriorly in the sub-maxillary region. F-wave recordings from nasal muscle for intraoperative monitoring of facial nerve function.

The generation of the F-wave in muscle after peripheral motor nerve stimulation depends upon the functional integrity of the entire motoaxon between its axon hillock and the motor endplates. Cerebellopontine angle (CPA) surgery bears the risk of damaging the facial nerve in its root exit zone; functional degradation should therefore be reflected by changes of the nasal muscle F-wave. Constant current pulses supraliminal for direct (M) muscular responses were applied subcutaneously to the zygomatic branch of the facial nerve ipsilateral to the operated side, EEG needle electrodes were placed in

Management of facial nerve during parotid recurrent pleomorphic adenoma revision surgery.

Revision surgery remains the standard treatment for parotid recurrent pleomorphic adenoma (RPA). Despite recent advancements in surgical techniques, the management of the facial nerve (FN) during parotid RPA surgery still poses a challenge. This retrospective study collected demographic data, clinical characteristics, and details of the surgical procedure, with a particular focus on FN management of parotid RPA patients between 2020 and 2024. Sixty-six patients who underwent parotid RPA surgeries were included; the FN was successfully preserved in 39 (59%). A significant difference in recurren
