The data of literature and results of treatment of 17 patients with necrotizing fasciitis are analyzed. Original conception of pathogenesis based on thrombosis of microcirculatory vessels of superficial fascia is described. Endotoxemia manifested with clinical picture of sepsis and septic shock. Successful treatment is possible in cases of early diagnosis, urgent surgical treatment and intensive antibiotic therapy. Urgent cryobiopsy of tissues is an important diagnostic point. Lethality was 50% (it reaches 75% according to the literature data), this can be explained by late hospitalization and…

BACKGROUND AND OBJECTIVE: Eosinophilic fasciitis is an uncommon scleroderma-like syndrome with unknown etiology and pathogenesis. We report the clinical manifestations and response to therapy in a series of patients diagnosed with eosinophilic fasciitis.
PATIENTS AND METHOD: The records of 10 eosinophilic fasciitis patients seen at our hospital between 1996-2004 were reviewed.
RESULTS: Five patients were males, ages ranged from 20 years to 67 years, with a mean of 59 years. Patients were followed for 33.4 (27.1) months (SD). Nailfold capillaroscopy was near normal in all patients. Only one p…

OBJECTIVE: The objective of this study was to describe MRI findings in patients with eosinophilic fasciitis (EF) and to correlate clinical and laboratory findings with the MRI findings.
MATERIALS AND METHODS: Six patients with histologically proven EF underwent MRI at the time of diagnosis and after therapy (15 MRI examinations). Unenhanced T1-weighted, T2-weighted, and STIR sequences were performed using a 1.5-T MRI system. In addition, all patients were imaged with contrast-enhanced T1-weighted sequences. MRI findings, clinical findings, and laboratory parameters were retrospectively review…

Necrotizing fasciitis is a rare, rapidly progressing soft tissue infection, characterised by extensive necrosis of subcutaneous fat and fascia with relative sparing of skin and underlying muscle. It is usually caused by toxin producing virulent bacteria. Although it can occur in otherwise healthy patients, the disease is usually seen in conjunction with immune deficiency disorders, intravenous drug abuse, peripheral vascular disease and diabetes. Necrotizing fasciitis is more frequent in abdominal wall, perineum and extremities. Involvement of the head and neck structures and especially the sc…

Although the number of patients presenting with necrotizing fasciitis remains small, the cases seen by practitioners become legendary. The purpose of this article is to demystify NF through a review of the pathophysiology and the clinical presentation of NE Recommended diagnostic and treatment modalities are also discussed. Future research is recommended in the area of advanced therapies, such as hyperbaric oxygenation and the utilization of burn centers. Without further advances, the practitioner must continue to be vigilant in completing rapid diagnosis and treatment so as to decrease morbid…

Necrotizing fasciitis is a life-threatening soft-tissue infection primarily involving the superficial fascia. This study investigated the microbiologic characteristics and determinants of mortality of this disease. The medical records of 87 consecutive patients with a diagnosis of necrotizing fasciitis from 1999 to 2004 were retrospectively reviewed. A single pathogen was identified as the infectious agent in 59 patients (67.8%), multiple pathogens were identified in 17 patients (19.6%), and no organism was identified in 11 patients (12.6%). Klebsiella pneumoniae, identified in 17 patients, wa…

Necrotizing fasciitis (NF) is a potentially life-threatening bacterial infection of the skin, deep subcutaneous tissue, and fascia. Early symptoms may be misdiagnosed as cellulitis. A hallmark symptom that distinguishes NF from cellulitis is severe local pain that is out of proportion to the size and type of the wound present. Early diagnosis and treatment of NF is imperative for a patient's survival. This article describes the pathophysiologic mechanisms, clinical manifestations, and treatment of NF, as well as implications for perioperative nursing.

The use of radiotherapy for plantar fasciitis has never been reported in Australasia and is scarcely found in the English language medical literature, but it is commonly used in Europe, especially in Germany. In Europe, treatment courses consisting of multiple small fractions have been associated with high levels of pain relief. In the present report, the use of single fractions or radiotherapy was evaluated by reviewing seven consecutive patients referred for treatment and by applying objective and subjective criteria for pain relief. One patient died of unrelated causes soon after treatment …

STATEMENT OF THE PROBLEM: Nodular fasciitis is a fibroblastic proliferation in which nodules develop rapidly. It most commonly affects the extremities followed by the trunk and occasionally the head and neck.
MATERIALS & METHODS: Case report.
RESULTS: A 32-year-old male was seen with a 4-month history of a painless infraorbital mass. On examination, a swelling was palpable anterior to the left infraorbital rim. There was some depression of the left corner of his mouth. Computerized tomography showed soft tissue swelling anterior to the left maxillary antrum which, also, appeared to be diseas…

Necrotizing fasciitis is a soft tissue infection that causes necrosis of subcutaneous tissue and fascia but usually spares skin and muscle. Management of this condition consists of early diagnosis, broad-spectrum antibiotic coverage, aggressive surgical debridement, wound closure, and intensive supportive care. Mortality estimates reported in the literature have ranged from 20 to 75%. We report the cases of 12 patients treated at the Joseph M. Still Burn Center in Augusta, GA. Because aggressive surgical debridement combined with medical support is required for successful treatment, we recomme…

OBJECTIVE: Eosinophilic fasciitis (EF) is histologically characterized by a fibrous and inflammatory thickening of subcutaneous septal-fascial-perimysial collagenous scaffold. This study aims to define the immunophenotype of inflammatory cells of fascia and muscle underlying the in situ immune response in EF.
METHODS: In 11 cases of EF, we determined the phenotype of inflammatory cells, expression of MHC class I and class II antigens, and C5b9 membranolytic attack complex (MAC) deposits by immunohistochemistry analysis of fascia tissue. Muscle biopsies from 9 patients with active dermatomyosi…

Necrotizing fasciitis is a potentially life threatening, rapidly spreading polymicrobial infection rarely seen in the head and neck region. We present our experience of managing five patients with necrotizing fasciitis of head and neck at the Aga Khan University Hospital, Karachi, Pakistan. One patient had necrotizing fasciitis of the facial region only; three had necrotizing fasciitis of the neck region; and one patient had necrotizing fasciitis of the neck with extension to the chest. None of these patients was immunocompromised. Dental foci of infection was the root cause in three of the pa…

Necrotizing fasciitis is a rare soft tissue infection and a life-threatening emergency, often fatal. Its incidence and management are described plentifully in the medical literature regarding the most common anatomical sites involved like the abdomen, lower and upper limbs, and perineum. However, available data and case reports of chest wall necrotizing fasciitis after thoracic procedures are scarce, mainly after major cardiac operations. We report and discuss a case of necrotizing fasciitis of the chest wall occurring in the immediate postoperative period of a cardiac procedure, and include a…

Bacteria are everywhere. Most are not pathogenic unless the right environment or quantity is present. A superficial infection in the medically compromised patient can provide a microenvironment conducive to precipitating wound sepsis, local, and systemic immune responses. Necrotizing fasciitis (NF) is a subtle yet bold infection that presents like cellulitis; however, beneath the surface of the integument, the ravaging products of cellular degradation are fulminant. Large dermal and subcutaneous involvement, creating significant tissue defects, require aggressive assessment and management by a…
