Pneumopericardium is an uncommon condition characterized by the presence of air within the pericardial sac and may rarely progress to cardiac tamponade. A patient with a history of Roux-en-Y gastric bypass presented with chest pain, dyspnea, and hemodynamic instability. Imaging demonstrated a large pneumopericardium with tamponade physiology. Further evaluation revealed a rare jejunopericardial fistula as the underlying etiology, resulting in communication between the gastrointestinal tract and pericardial space. Emergent pericardial decompression was performed with clinical improvement. This …

Spinal dural arteriovenous fistulas (SDAVFs) are the most common type of spinal vascular malformations. Multiple SDAVFs are unusual and can occur either synchronously or metachronously, as reported in the literature. We report on the unusual case of a woman with three separate SDAVFs, which were surgically treated within the same setting; the postoperative course was characterised by unexplained repeated haematoma formation within the surgical sites. A review of the literature was performed to assess the outcomes of patients with multiple distinct SDAVFs: the data of 19 patients were collected…

Grade C pancreatic fistulas are associated with severe morbidity and a significant risk of mortality. High-risk pancreatic anastomoses can be predicted to allow best practice fistula mitigation techniques. In these high-risk glands, any deviation from a stable postoperative clinical course should prompt early computed tomography and aggressive, percutaneous drainage of the operative bed. If salvage surgery is necessary, drainage of the operative bed and/or external diversion of pancreatic juice via stenting while completion pancreatectomy should be avoided. Senior mentorship in the perioperati…

Gastrostomy tubes are often placed in patients with poor voluntary intake, oropharyngeal dysphagia, or chronic illness to provide definitive nutritional access. Despite the widespread use of gastrostomy tubes, some patients can experience complications associated with this procedure including gastrocolic-cutaneous fistula and dislodgement of gastrostomy tube. This case discusses an instance of gastrojejunal fistula formation over one year after gastrostomy tube placement likely due to tube dislodgement. Imaging showed gastrostomy tube traversing the posterior wall of the stomach and creating a…

Pelvic floor disorders (PFDs) and obstetric fistula (OF) are common across the globe. PFDs include stress and urge urinary incontinence, overactive bladder, pelvic organ prolapse, fecal incontinence, sexual dysfunction, and pelvic pain. Although PFD and OF are common in low- and middle-income countries (LMIC) there is a lack of awareness and constraints in health care resources. This article focuses on epidemiology, risk factors, assessment, and treatment of PFD and OF in resource-poor settings. Adherence to basic medical ethics principles has to be maintained at all times, coupled with knowle…

INTRODUCTION AND HYPOTHESIS: This video demonstrates surgical repair of a vesicouterine fistula via a robotic, uterine-sparing approach.
METHODS: In this video, we present a vesicouterine fistula, which occurred after cesarean delivery. The patient presented with cyclical hematuria 4 years following delivery. She underwent uterine-conserving robotic repair via excision of the fistula tract through an intentional cystotomy. The uterus and bladder were closed in multiple layers.
RESULTS: The patient tolerated the procedure well, and CT cystogram 6 weeks following surgery demonstrated no concer…
