Double Sigmoid colon perforation due to migration of a biliary stent.

Migration of pancreatico-biliary stents is a rare event, usually benign, but which can lead to severe complications such as digestive tube perforation. We report the case of a patient with double sigmoid perforation due to distal migration of a biliary stent placed to decompress a pancreatic head carcinoma.

Successful endoscopic ultrasound-guided overstenting biliary drainage through a pre-existing proximal migrated metal biliary stent.

Biliary endoscopic drainage using metallic self-expanded stents has become a well-established method for palliative treatment of malignant biliary obstruction. However, its occlusion, mainly by tumor overgrowth, is still the main complication without a standard treatment. We here describe a new method of treatment for biliary metallic stent occlusion, through the echo guided biliary drainage. We present a 68-year-old patient with metastatic pancreatic cancer previously treated for jaundice with ERCP and self-expandable metallic stent insertion. Four weeks later, the patient developed jaundice

Erosive bladder perforation as a complication of ventriculoperitoneal shunt with extrusion from the urethral meatus: case report and literature review.

Ventriculoperitoneal (VP) shunt surgery is the most common technique used in the treatment of hydrocephalus. Erosive bladder perforation is an extremely rare complication of VP shunt surgery. Only 2 cases of erosive bladder perforation by a peritoneal catheter have been reported in the English literature. The authors present the case of a 14-year-old male with a history of cerebral palsy, severe developmental delay and hydrocephalus who presented with VP shunt tubing protruding from his urethral meatus. The patient had no evidence of neurological change. The VP shunt had been last revised over

Gunshot bullet embolus with pellet migration from the left brachiocephalic vein to the right ventricle: a case report.

We report the case of a 16 year old male who was the victim of a drive by shooting sustaining the rare but recognised complication of cardiovascular bullet embolism. He was seen as a trauma call in the emergency department and CT scanning revealed 70 shotgun pellets scattered throughout left sided sub-cutaneous tissues of the head and neck, and more significantly a single pellet within the right atrium. It is believed to have got there via injury to the left brachiocephalic vein which was demonstrated by extravasation of contrast on the CT scan. He remained stable throughout admission and the

Intraluminal migration of mesh following incisional hernia repair.

Incisional hernia is the most common long term complication of colon surgery. These hernias are often repaired with synthetic mesh. Mesh migration is an infrequent occurrence. There are reports of mesh migrating into the peritoneum and into viscous organs causing a variety of symptoms including recurrent hernia, infection, and obstruction. We present the case of transmural mesh migration from the abdominal wall into the small bowel presenting as small bowel obstruction.

CooperSurgical Colpotomizer Koh Cups are being left in patients after uterine surgery.

The Koh cup--a component of the CooperSurgical Koh Colpotomizer system, which is used in some surgeries of the uterus-may detach during removal of the system from the patient, presenting the risk that it will be retained in the patient following surgery. Users should confirm that the system is intact when it is removed from the patient.

A ventriculoperitoneal shunt catheter wrapped around a right mammary prosthesis forming a pseudocyst.

Breast cerebrospinal fluid pseudocysts are a rare complication of ventriculoperitoneal (VP) shunting. It is very unusual for the peritoneal catheter of a shunt to become wrapped around a prosthesis a previously augmented breast. Three previously reported patients developed an enlarging breast lump and were diagnosed with shunt migration around the prosthesis. Our patient underwent bilateral breast augmentation and subsequently a VP shunt. We observed on frequent follow-up chest X-rays performed for pneumonia and mechanical ventilation that the peritoneal catheter had gradually wrapped around h

Migrating shrapnel: a rare cause of knee synovitis.

UNLABELLED: Shrapnel injuries in soft tissues often do not require surgical excision. Metals that remain embedded in the surrounding tissue are not thought to cause significant damage and the patients are generally asymptomatic. This case presentation describes a patient who sustained a penetrating shrapnel injury to his thigh, where the metal fragment was not removed. However, more than 20 years later, the patient developed knee synovitis. On X-ray the shrapnel was seen in the suprapatellar pouch. An arthroscopy was preformed and the shrapnel was removed with full healing of the patient. CON

Cholecystitis caused by a fish bone.

The accidental ingestion of a foreign body is not uncommon. However, the presence of a foreign body in the gallbladder is extremely rare. Here, we present a case of cholecystitis due to a fish bone that may have penetrated through the stomach wall and into the gallbladder without causing peritonitis. A laparoscopic cholecystectomy was performed; a fish bone, measuring 4.0 cm in length, was found in the gallbladder. To the best of our knowledge, this is the first such case to be reported.

An unusual complication 4 years after laparoscopic adjustable banding: jejunal obstruction due to the connecting tube.

Laparoscopic adjustable gastric banding (LAGB) is an often performed bariatric procedure for morbid obesity. Several complications are known, including surgery and port-side-associated complications. As rare but serious late complication of LAGB, we describe a 50-year-old patient with periodic incomplete intestinal constriction due to the silicone connecting tube, 4 years after laparoscopic adjustable gastric banding was performed. Abdominal CT scan showed an obstructive ileus due to a loop in the connecting tube. After surgical removal of the connecting tube, the patient recovered without seq

Intracranial ricocheting of bullet from anterior clinoid process.

Gunshot wounds to the head are usually mortal injuries. We present a unique case of intracranial ricocheting of bullet without neurological deficits. Patient was treated conservatively with antibiotics for one week and prophylactic anticonvulsants for six weeks. Patient is doing well at six months follow up. Repeat X-ray skull showed that bullet was lying in the occipital region. It is recommended that deep seated bullets should be left behind as any attempt to remove that bullet may increase the morbidity and mortality. However close follow up of these patients is very important as these pati

Perforation of an adjustable gastric banding connecting tube into distal transverse colon with intra-luminal migration.

Laparoscopic adjustable gastric banding (LAGB) is one of the most common bariatric procedures performed in Europe and Australia. Major post-operative complications are limited but once they occur, prompt diagnosis and treatment are mandatory. LAGB complications are related either to the port and the connecting tube, such as infection or tubing disconnection and migration, or to the band, such as slippage, pouch dilatation, or intra-gastric migration. We report a case of intra-colonic migration of the connecting tube occurring 4 years after gastric banding placement in a patient otherwise asymp

A wandering stent in the ascending aorta.

A 15-year-old girl underwent patch aortoplasty for repair of a long-segment coarctation of the aorta 7 years ago. Balloon angioplasty had been performed twice, 3 and 5 years after the aortoplasty, because of recurrent coarctation of the aorta. She was scheduled for balloon angioplasty and stent implantation. In catheterization, there was 55 mmHg gradient between ascending and descending aorta and the angiography showed long-segment coarctation of the aorta, from the transverse aorta, after the innominate artery, to the descending aorta, after the left subclavian artery. During the final pressu

Intestinal perforation after the incidental ingestion of a fishhook.

The vast majority (75%) of ingested foreign bodies pass through the gastrointestinal tract spontaneously and require no surgical intervention. Indeed, the emergency department (ED) management of ingested foreign bodies is largely based on the age of the patient and the time elapsed since ingestion, as well as the physical characteristics of the foreign body. We present this case to emphasize the importance of radiologic imaging studies in the ED management of adults with an acute abdomen of unknown etiology. A 44-year-old woman presented to the ED with right upper quadrant pain of 2 days durat

The perforated intrauterine device: endoscopic retrieval.

BACKGROUND: Uterine perforation is the most serious complication associated with an intrauterine contraceptive device (IUD). Minimally invasive techniques, such as hysteroscopy and advanced laparoscopy, are ideally suited to the diagnosis and surgical management of the perforated IUD. CASE REPORTS: Three cases of uterine perforation caused by an IUD and treated with endoscopic surgery are presented. In all 3 cases, the IUD was located by using x-rays, ultrasonography, or pelvic magnetic resonance imaging. Diagnostic laparoscopy was performed to identify the specific location of the IUD and to

Surgical removal of an intrauterine device perforating the sigmoid colon: a case report.

BACKGROUND: Intrauterine devices (IUDs) are among the most common contraceptive methods in the world, but have been associated with extrauterine dislocation and bowel perforation. We report a case of an IUD perforation of the sigmoid colon. CASE REPORT: A 35-year-old woman with complaints of abdominal pain, fever, and diarrhea underwent exploratory laparotomy. During the procedure, a left tubo-ovarian abscess was found as well as an IUD extruding through the uterine cavity with the two wings of the device entirely in the lumen of the sigmoid colon. The patient had total abdominal hysterectomy
