Fracture-dislocation of the dorsal-lumbar spine. Acute operative stabilization by Harrington instrumentation.

Twenty-four patients (of whom 17 were paraplegic) with acute fracture dislocations of the dorsal-lumbar spine underwent stabilization with Harrington instrumentation and were followed for an average of 16 months. Immediate postoperative ambulation was achieved in 16 patients. There was little difference between compression and distraction with respect to reduction and maintenance of correction. Compression rods resulted in solid union in 15 of 16 patients, while distraction rods were successful in 6 of 8 instances. In contrast to reported series in which operative intervention has been avoided…

Popliteal artery injury following fracture or dislocation at the knee. Diagnosis and management.

Fracture dislocations or dislocations of the knee can produce an arterial injury that may be difficult to evaluate clinically. Aggressive diagnosis and treatment can produce satisfactory results in both vascular and orthopedic situations if performed early after the injury. Five cases of popliteal arterial injury were seen from April 1974 to August 1976. Three cases were following posterior knee dislocation, one was a femoral fracture displacement, and one was a rotary tibiofibular displacement. Four of the patients had complete popliteal artery transection and one had intramural hematoma and …

Acute dislocation of the patella with osteochondral fracture: a review of eighteen cases.

Eighteen patients with acute dislocation of the patella had associated osteochondral fractures. This fracture complicates approximately 5% of all acute dislocations of the patella occurring in children. Three types of fracture patterns were noted. All patients who were treated by immediate arthrotomy and excision or replacement of the osteochondral fragment and repair of the acute dislocation of the patella made an uneventful recovery with no recurrence of the dislocation. In those patients in whom the osteochondral fragment was removed but with no repair the dislocation recurred.

Surgical Management of a Lateral Compression (LC)III Pelvic Ring Fracture With Type I Crescent Fracture-Dislocation: A Case Report.

Lateral Compression (LC) pelvic ring fractures represent the most common pattern of pelvic ring injury in the Young-Burgess classification. The Crescent fracture is an uncommon subtype characterized by an iliac wing fracture that disrupts the posterior sacroiliac (SI) complex, creating a crescent-shaped fragment that is inherently rotationally unstable and associated with long-term complications including chronic pain and sexual dysfunction. LC fractures result from a laterally directed force and are subclassified (I-III) based on the degree of posterior ring disruption. We present the case of

[Subtotal corpectomy and intervertebral bone grafting through posterior approach alone in treatment of thoracolumbar burst fracture or thoracolumbar fracture-dislocation].

OBJECTIVE: To evaluate the early clinical outcomes of subtotal corpectomy and intervertebral bone grafting through posterior approach alone in the treatment of thoracolumbar burst fracture or thoracolumbar fracture-dislocation. METHODS: Between January 2009 and December 2010, 20 patients with thoracolumbar burst fracture or thoracolumbar fracture dislocation were treated with subtotal corpectomy and intervertebral bone grafting through posterior approach alone. There were 14 males and 6 females, with an average age of 36.1 years (range, 19-47 years). Fractures were caused by falling from heig…

Two-stage surgical approach for type III odontoid fracture with C1-C2 fracture dislocation: a case report and management overview.

INTRODUCTION AND IMPORTANCE: Odontoid fractures of the second cervical vertebra (C2) are categorized into three types, with type III extending into the body of the axis. These fractures, often resulting from high-energy trauma, can cause significant instability and neurological issues. This case report discusses a 43-year-old male with a type III odontoid fracture and C1-C2 fracture dislocation, demonstrating the effectiveness of traditional neurosurgical techniques in managing such complex injuries. CASE PRESENTATION: A 43-year-old male presented with upper cervical pain and right upper limb…

Posterior Hip Dislocation in Association With Anterior Column Acetabular Fracture, Femoral Head Fracture, and Sciatic Nerve Injury: A Case Report.

CASE: Posterior hip dislocations are classically associated with posterior acetabular wall fractures. We report the case of a 29-year-old man presented after a motorcycle accident with an unusual combination of injuries that included posterior hip dislocation, anterior column acetabulum fracture, femoral head fracture, and sciatic nerve injury. At the final follow-up, excellent outcomes were obtained with complete recovery of the sciatic nerve injury. CONCLUSION: A favorable outcome may be achieved in young patients who sustain this unusual compilation of ipsilateral anterior acetabulum fract

Simultaneous ipsilateral Monteggia fracture-dislocation and distal radius fracture: A report of a pediatric case and review of the literature.

We present the case of a 12-year- old boy who suffered from a combination of Monteggia fracture-dislocation along with ipsilateral distal radius fracture. The patient underwent closed reduction of the Monteggia lesion and the distal radius followed by percutaneous pinning. The postoperative course was satisfactory. Associated Monteggia fracture-dislocation and ipsilateral distal forearm fracture are rare. Our case and the literature review highlight the presence of common clinical features in pediatric patients with these particular injuries, including male dominance, age around 9 years, fall

Fracture of the Humeral Shaft Associated to Elbow Dislocation and Fracture of the Distal-third of the Forearm: Case Report.

Humeral shaft fractures combined with elbow dislocation and fracture of the distal third of the bones of the forearm are uncommon. No description of this simultaneous association has been found in the same patient. Some studies report the association of these two lesions; however, no reports on the three ipsilateral lesions have been found at the PubMed, Lilacs and Bireme databases. The present report describes a case that occurred in a 13-year-old boy who suffered a fall from a height of approximately three meters and was admitted to a trauma hospital. Radiographs showed an ipsilateral humera

Reverse shoulder arthroplasty with glenoid bone grafting for anterior glenoid rim fracture associated with glenohumeral dislocation and proximal humerus fracture.

BACKGROUND: Large fractures of the anterior glenoid rim can result in persisting instability and osteoarthritis of the glenohumeral joint When this fracture is associated with a glenohumeral dislocation and proximal humerus fracture could be a concern. The goal of this paper was to evaluate the clinical and radiological outcomes and complications of reverse shoulder arthroplasty (RSA) and glenoid bone graft in cases with a significant anterior glenoid fracture associated with a proximal humerus fracture. HYPOTHESIS: RSA and step bone graft harvested from proximal humeral head could be a viabl

Use of a bivalve finger fracture orthosis for a new treatment protocol of a PIP comminuted fracture and dorsal dislocation.

As therapists and physicians, we often need to work with our patients to cater treatment accordingly. These authors describe how they modified a treatment protocol for a patient that sustained a proximal interphalangeal joint comminuted fracture and dorsal dislocation, but refused surgery. Their modification allowed the patient to return to full activities. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

Reconstructive and salvage techniques for proximal interphalangeal joint fracture dislocations.

Proximal interphalangeal joint (PIPJ) fracture dislocations are frequently injured during sports, and often present in a delayed fashion. Untimely management may lead to recurrent subluxation and chronic stiffness, arthrosis, and pain.1 Unstable injuries frequently require surgical management with either internal or external fixation to maintain articular reduction and allow early mobilization. Delay in presentation or extensive fracture comminution presents technical challenges and may necessitate a reconstructive or salvage procedure. We have defined reconstructive procedures as those that r…

Knee dislocations and associated fractures: risk factors for surgical reduction.

INTRODUCTION: Dissociation of the knee joint, or knee dislocations (KD), can lead to severe complications, often resulting in multiligament injuries. A subset of these injuries are irreducible by closed reduction and require open reduction. Identifying KDs that necessitate surgical intervention is crucial for optimal outcomes. While previous studies have explored various risk factors, the influence of associated fractures is less understood. MATERIALS AND METHODS: We queried the Trauma Quality Improvement Program (TQIP) database from 2017 to 2021, for non-congenital closed knee dislocations r

Radiological analysis and outcomes of isolated greater tuberosity fracture-dislocations.

BACKGROUND: The purpose of this study was to investigate different radiological characteristics for isolated greater tuberosity (GT) fracture-dislocations and their effects on complication and reoperation rates. METHODS: A two-centre, retrospective study was performed on patients with a minimum 1-year follow-up (median 4.5 years). Patients were split into two groups, Group A (<65 years old) and Group B (≥65 years old). Outcomes included initial injury characteristics (dislocation and fracture type, AC/BC ratio and distances), the reduction environment and postreduction outcomes including comp

Bilateral asymmetrical hip dislocations with acetabulum fractures; a case report.

Bilateral hip dislocation is a rare injury and asymmetrical hip dislocations are even very rare entities. We are reporting two cases of bilateral asymmetrical hip dislocations with associated acetabulum fractures. Mechanism of injury in first case was fall from height and in other case road traffic accident. Associated injuries were midshaft femur fracture and sciatic nerve (Peroneal part). Anterior hip dislocation in both cases is reduced closely but posterior hip dislocation in both cases relocated openly due to ipsilateral femur fracture in first case and unstable hip in second case. Acetab
