Tibial lengthening over an intramedullary nail.

BACKGROUND: Long-term application of an external fixator to treat leg-length discrepancy and short stature often causes complications, such as pin-tract infection or loss of range of motion at the knee or ankle (or both). Prolonged fixator use also interferes with the activities of daily living. To minimize such problems, we have combined intramedullary nailing with external fixation. Using this technique, the external fixator can be removed more quickly after completing the lengthening. METHODS: We combined intramedullary nailing with lengthening in 13 tibias (8 patients) and then compared t

Treatment of severely displaced proximal humeral fractures in children with elastic stable intramedullary nailing.

We report on our 8-year experience of using elastic stable intramedullary nailing for severely displaced proximal humeral fractures in children. Fourteen patients (mean age 13.4 years) with seven epiphyseal and seven metaphyseal fractures underwent intramedullary nailing, using single nail fixation in 12 cases. Clinical and radiological healing was achieved at 2.4 and 3.2 months, respectively. Complications included temporary shoulder and elbow stiffness in one and four cases, respectively, one nail breakage at removal, two cases with minor humeral shortening, and two cases with minor varus de

Does postoperative immobilization affect final alignment of pediatric femur fractures treated with flexible intramedullary nailing?

Flexible intramedullary nail (FIN) fixation of pediatric femur fractures is a popular method of fixation in children. Typical immobilization options include spica casting, long leg cast, knee immobilizers, or no immobilization and the decision to use each is usually left to surgeon preference. Our primary aim is to evaluate whether different postoperative immobilization status influences outcomes, namely radiographic alignment at the time of healing. A retrospective chart review was conducted of all patients with femur fractures treated with FIN fixation at a pediatric hospital from April 2018

Prophylactic Intramedullary Rodding Following Femoral Lengthening in Congenital Deficiency of the Femur.

INTRODUCTION: Femoral fracture after lengthening of congenital femoral deficiency (CFD) is a common complication with a high incidence, up to 50%. The purpose of this study is to determine whether prophylactic intramedullary Rush rodding after lengthening for CFD is an effective method to prevent femoral fracture and to assess any complications that may result. METHODS: Forty-five femoral lengthenings (43 CFD patients) were performed using external fixation. At the time of frame removal (except for 3 cases, 8 d later), prophylactic intramedullary Rush rods were inserted. Special reaming techn

Ender nail fixation in long bone fractures: experience in a level I trauma center.

Early fixation of extremity fractures in polytrauma patients is a well accepted and desirable treatment. In our patient population, the indications for immediate Ender nail fixation was for Grade I and Grade II open long bone fractures and closed long bone fractures in patients with small intramedullary canals. Ender nails were also used in patients in whom reaming was undesirable. This included patients with open physes for immediate fracture fixation and in those with Grade III open wounds in a delayed fashion. These flexible nails were also implemented for use to minimize anesthesia time as

Entrapment of the Median Nerve after Elastic Intramedullary Nailing of a Pediatric Forearm Fracture.

Closed reduction and fixation with elastic intramedullary nails is a popular technique to treat displaced pediatric forearm fractures and has yielded good functional results. Postoperative neurological symptoms can be due to neuropraxia and expectative treatment has been advised for 3 months until further investigation is necessary. We present a case of an 11-year-old boy that presented with median nerve palsy 2 months after a displaced forearm fracture treated with elastic intramedullary nails. Ultrasound and nerve conduction study confirmed the presence of a median nerve entrapped in the cal

Management of Open Tibial Shaft Fractures: Does the Timing of Surgery Affect Outcomes?

BACKGROUND: Open tibial shaft fractures require emergent care. Treatment with intravenous antibiotics and fracture débridement within 6 to 24 hours is recommended. Few studies have examined outcomes when surgical treatment is performed >24 hours after occurrence of the fracture. METHODS: This retrospective study included 227 patients aged ≥18 years with isolated open tibial shaft fractures in whom the time to initial débridement was >24 hours. The statistical analysis was based on time from injury to surgical débridement, Gustilo-Anderson classification, method of fixation, union status, and

Pathological fracture of the humeral shaft: Antegrade intramedullary nailing versus plate osteosynthesis with bone defect cementation.

PurposeClinically, metastatic pathological fractures of the humeral shaft remain a challenging issue. Plate osteosynthesis and intramedullary nailing are common fixation methods; however, the optimal strategy remains debated, especially in patients with limited life expectancy where surgical goals include rapid pain relief, functional recovery, and fewer reoperations.MethodsWe retrospectively reviewed 27 patients with metastatic humeral shaft fractures treated surgically during 2009-2019. Patients were grouped by plate osteosynthesis (n = 17) and antegrade intramedullary nailing (n = 10), and

No Difference Between Intramedullary Kirschner-Wires and Titanium Elastic Nails in Treating Pediatric Femur Fractures: Results From a Randomized Clinical Trial in Dar Es Salaam, Tanzania.

BACKGROUND: Femur fractures are a common cause of disability of children in low-and middle-income countries (LMICs). The introduction of flexible intramedullary nails improved the care of children with diaphyseal femoral fractures. Application of intramedullary nails is challenging in resource-limited settings due to implant cost and availability. Intramedullary Kirschner-wire (K-wire) fixation has been proposed as a low-cost alternative option for pediatric femur fractures. This study examines the non-inferiority of intramedullary K-wire to titanium elastic nail (TEN) for diaphyseal pediatric

Outcome of Locking Plate Fixation Adjunctive to Intramedullary Rodding in Osteogenesis Imperfecta Patients.

BACKGROUND: An intramedullary rodding is the preferred fixation method in limb stabilization of OI patients. However, the intramedullary rod may not provide adequate fixation and rotational stability, especially in adolescents and adults. The incorporation of adjunctive plate fixation alongside intramedullary rodding has been introduced to enhance this stability, although its complications remain insufficiently understood. The goal of this study was to explore the outcomes of adjunctive plating in conjunction with intramedullary rodding for limb stabilization in OI patients, emphasizing the co

Minimally invasive plate fixation of paediatric lower limb long bone fractures: a review on when and how.

Fractures of the lower limbs represent a common cause of hospital admission. Surgical intervention is increasingly indicated in paediatric patients. The gold standard for treating long bone fractures in the immature skeleton is intramedullary fixation with titanium elastic nails. Minimally invasive plating is considered a suitable option when elastic nailing is not feasible. The use of a plate provides adequate mechanical stability for weight-bearing and limb mobility while preserving biological bone integrity and resulting in limited scarring, generally well accepted by patients and families,

Comparison of the intramedullary nail and low-profile plate for unstable metacarpal neck fractures.

BACKGROUND: There is no robust evidence for the best treatment practice for metacarpal neck fractures. The purpose of this comparative study was to investigate whether the intramedullary nail or low-profile plate allows for good clinical and radiological results for displaced metacarpal neck fractures. METHODS: We prospectively reviewed 30 patients with a displaced metacarpal neck fracture who underwent surgery: 15 with intramedullary nails and 15 with low-profile plates. Radiographic and clinical outcomes of both groups were compared. Objective findings of range of finger motion and grip str

Retrograde intramedullary nailing of comminuted intra-articular distal femur fractures results in high union rate.

PURPOSE: Retrograde intramedullary nailing of intra-articular distal femur fractures with metaphyseal and/or epiphyseal comminution is controversial and considered a contraindication to nailing. The purpose of this study was to report union rate, complications, and secondary procedures after open reduction and retrograde intramedullary nailing of comminuted, intra-articular, distal femur fractures. MATERIALS AND METHODS: A retrospective review performed at an urban level one trauma center identified 16 patients AO/Orthopedic Trauma Association (OTA) 33-C2 and 33-C3 femur fractures treated wit

Operative treatment of humeral shaft fractures. The Leuven experience.

For many years, plating has proved to be a reliable method for the fixation of fractures of the humeral shaft. In the early nineties however, intramedullary devices became increasingly popular for fractures of the humeral shaft. This was based on a global tendency towards minimally invasive surgery, and the attractiveness of the relatively simple procedure and potentially lower complication rate of intramedullary nailing, which had proved to be successful in osteosynthesis of the lower limb, However, until now there is no consensus in the literature as to which device is preferable for differe

[Shaft fractures of the lower limb in adolescents].

Shaft fractures of the lower extremities are rare during adolescence. Conservative treatment is no longer used for the upper leg and the significance is declining for the lower leg, as only minor axis deviations and malrotations of the shaft are acceptable and conservative therapy is associated with severe impairments in the daily routine; however, conservative treatment is still an option for femoral fractures in children < 3 years of age and for stable fractures of the lower leg. Various osteosynthesis procedures can be used depending on the body weight and epiphyseal maturation. Elastic sta

[Distal femoral fracture--technical possibilities].

The fractures of the distal femur are classified as extra-articular fractures (type A), partial articular fractures (type B), and complete articular fractures (type C). The aims of operative treatment are anatomical reconstruction of the articular surfaces, restoration of rotational and axial alignment, stable fixation of the condyles to the shaft of the femur and early functional aftercare. Techniques and implants for treating intra-articular fractures lies in the reduction of the joint fragments and fixation with lag screws. For the treatment of the extra-articular fractures different approa

Simultaneous ipsilateral diaphyseal fractures of the femur and tibia: treatment and complications.

We present our experience of intramedullary nailing (IM) and external fixation in the treatment of 54 patients with ipsilateral diaphyseal fractures of the femur and tibia. Eight femoral and 24 tibial fractures were open. They were classified into three groups: IM nailing of both fractures (group A, 19 patients); IM nailing of the femoral and external fixation of the tibial fracture (group B, eight patients); and external fixation of both fractures (group C, 27 patients). In group C (which included all but one grade III open fracture), two patients died and four underwent amputation. Femoral f
