Vascularized iliac bone graft for displaced femoral neck fractures in young adults.

From November 1989 to September 1994, a total of 26 patients (18 men and 8 women) with acute displaced femoral neck fractures were treated with closed reduction and Knowles pinning combined with vascularized iliac bone grafting. Mean patient age was 37.5 years (range: 24-48 years). The mean time interval from injury to surgery was 3.6 days (range: 1-10 days). One patient experienced loss of the reduction at 6 weeks postoperatively and subsequently went on to hip replacement. Mean follow-up for the remaining 25 patients was 56.2 months (range: 20-78 months). All fractures united within 4 months

Can tuning forks replace bone scans for identification of tibial stress fractures?

PURPOSE: To compare the performance of the tuning fork test (TFT) with nuclear scintigraphy for the identification of tibial stress fractures. METHODS: Fifty-two patients with a history and physical examination suggestive of tibial stress fracture underwent a TFT followed by a bone scan. The TFT was performed by applying a 128-Hz tuning fork to the anterior surface of the bared tibia. If the patient reported a marked exacerbation or reproduction of shin pain in a localized area of the tibia, the TFT was considered positive. All patients also underwent a bilateral lower-extremity bone scan. R

New form of bone dysplasia with multiple fractures associated with monosomy X.

We report on the clinical, radiologic, and pathologic findings in a 20-week-old fetus with monosomy X and severe hydrops associated with fetal dwarfism. The fetus presented with osteoporosis, bent bones, multiple fractures, and distinctive symmetric submetaphyseal transverse bone interruptions or pseudofractures. We excluded by radiologic and histopathologic examination the diagnoses of osteogenesis imperfecta, hypophosphatasia, campomelic dysplasia, achondrogenesis, hypochondrogenesis, and other types of bone dysplasia. To our knowledge, this is a previously undescribed bone dysplasia associa

The management of long bone fractures in the head-injured polytrauma patient.

The polytrauma patient who sustains a significant head injury (head and neck Abbreviated Injury Scale of 3 or greater) will require prolonged and technically demanding operative intervention for musculoskeletal and associated soft tissue trauma. The presence of a head injury may delay the immediate surgical intervention for long bone injuries, which has proven to have major advantages for patient care and well-being. This retrospective review of the Sunnybrook Health Science Centre experience between January 1, 1986, and June 1, 1988, identified 153 polytrauma patients with a significant head

Stress fractures of the tarsal navicular bone: CT findings in 55 cases.

OBJECTIVE: The purpose of this article is to present the CT findings in 55 cases of tarsal navicular stress fracture before and after treatment and to describe the CT protocol used. MATERIALS AND METHODS: Fifty-five navicular stress fractures in 54 patients whose initial and follow-up CT scans were available for study were retrospectively reviewed. In most cases, contiguous 1.5-mm axial and 3-mm coronal scans of the navicular bone had been obtained. Both feet were scanned for comparison. Follow-up scans were obtained between 4 weeks and 6 years. RESULTS: On review, fractures were evident in

Open tibial fractures. Treatment by uniplanar external fixation and early bone grafting.

Seventy-nine open tibial fractures were treated with unilateral uniplanar tubular external fixators. Excellent stability allowed early weight-bearing. All comminuted fractures, with or without bone loss, and some transverse or short oblique fractures with intermediate fragments were treated by early bone grafting through a posterolateral approach. The external fixator was dynamised as soon as periosteal callus was seen on the radiograph. Bone healing times ranged from 11 to 40 weeks (mean 20). Significant ankle stiffness occurred in 10.9% and leg shortening in 2.8%. Pin track infection was see

Late treatment of scaphoid fractures by bone grafting and compression staple osteosynthesis.

Scaphoid nonunions and delayed unions still offer a difficult challenge. In 15 of 16 cases it was possible to achieve a secure and lasting union by using a compression staple and a bone graft. The subjective and objective clinical evaluation also showed similar favorable results. This modification of the Russe operation is recommended as a relatively simple procedure with good results and minimal need for immobilization.

Metabolic bone disease presenting as multiple recurrent metatarsal fractures: a case report.

A case of adult hypophosphatasia involving a patient with recurrent bilateral metatarsal fractures is presented. This disorder, although relatively uncommon, is often associated with multiple pseudofractures and complete pathologic fractures of the lower extremity. It can be detected with routine laboratory analysis. Metabolic bone disease should be considered as a possible causative factor in patients with multiple "stress" fractures of the foot.

Fractures of the scaphoid bone. How to handle this common wrist injury.

A combination of forces makes the scaphoid bone susceptible to fracture. The common mechanism of trauma is a force applied to the palmar aspect of the wrist while it is in extreme dorsiflexion. Because early fracture diagnosis by x-ray study is difficult, all patients with a suspicious wrist injury and tenderness in the scaphoid region should be treated as if they have a fracture until radiographs at two and four weeks are normal. Treatment usually consists of immobilization using a long or short plaster arm cast. Various treatment methods, including electrical stimulation, are used when nonun

Subtalar distraction bone block fusion for late complications of os calcis fractures.

The authors are presenting a new subtalar fusion technique for late complications of calcaneus fractures. These complications include pain, shoe wear difficulties, and foot deformity. The complex pathology includes incongruous subtalar joint, loss of calcaneal body height, and decreased lateral talocalcaneal angle. The latter two factors can result in tibiotalar neck impingement, a deformity that has received little attention in the literature. The subtalar fusion technique involves distraction of the subtalar joint, insertion of a bone block, and rigid screw fixation. The distraction allows c

Bone scintigraphy in the management of X-ray-negative potential scaphoid fractures.

One hundred and eleven patients with signs and symptoms of carpal scaphoid injury, but with no fracture visible on X-ray, underwent bone scintigraphy of the wrists. The first 42 patients were re-X-rayed 10 days after injury: bone scanning had identified all fractures confirmed on this X-ray; there were no false negative bone scans. Sixty-seven patients (60%) did not have increased focal uptake over the scaphoid or distal radius, and were mobilised immediately. None of these had a fracture at follow up. Twenty-nine patients with increased uptake over the scaphoid area (26%) remained in plaster

Facial paralysis in longitudinal temporal bone fractures: a review of 26 cases.

The management of facial nerve injury resulting from temporal bone fracture continues to be a topic of discussion. The indications for surgery as well as the proper timing and extent of surgical exploration are the principal questions debated. This study reviews a large series of patients (26), treated for facial paralysis following a longitudinal temporal bone fracture. The most frequent site of injury was the area of the geniculate ganglion. The injury to the facial nerve was often severe. We recommend early exploration in these cases when electrical studies indicate a severe injury. In most

Primary operative fixation of long bone fractures in patients with multiple injuries.

Early mobilization is essential for patients with multiple long bone fractures associated with other injuries in order to avoid complications. Therefore, in 1969 we adopted a policy of rigid internal fixation of long bone fractures in such cases. To assess the results of this active treatment a number of consecutive patients were followed. The following conditions were necessary for the case to be included in this series: multiple injuries; treatment at the intensive care unit; and at least two long-bone fractures treated with internal fixation. During the period from 1969 to 1974 47 patients

The Phenotype of Bone Turnover in Patients with Fragility Hip Fracture: Experience in a Fracture Liaison Service Population.

Background: Hip fragility fractures are becoming one of the main health care problems in countries with an aging population. This study aimed to evaluate the clinical characteristics and the usefulness of bone turnover markers in patients with a hip fracture. Methods: In a cohort of 363 patients (84.1 ± 9.2 years) with hip fractures we measured 25-hydroxyvitamin D (25OHD), bone alkaline phosphatase, type I collagen β carboxy telopeptide (βCTX), and parathyroid hormone (PTH). We recorded patients’ Charlson Comorbidity Index (CCI) and previous history of fragility fractures. Results: Vitamin D a

Risk factors for nonunion of bone fracture in pediatric patients: An inception cohort study of 237,033 fractures.

UNLABELLED: Adult fracture nonunion risk is related to injury severity and surgical technique, yet nonunion is not fully explained by these risk factors alone; biological risk factors are also important. We test a hypothesis that risk factors associated with pediatric fracture nonunion are similar to adult nonunion risk factors.Inception cohort study in a large payer database of pediatric fracture patients (0-17 years) in the United States in calendar year 2011. Continuous enrollment in the database was required for 12 months, to allow time to capture a nonunion diagnosis. The final database c

Concomitant open reduction of a nasal bone fracture combined with a zygomatic fracture through a subciliary incision.

Nasal bone fracture is the most common of the fractures of the facial skeleton. For centuries, these injuries have been managed with closed reduction, but because of low surgeon satisfaction rates and high revision rates, open reduction is advocated in cases involving severe deviation of the nasal dorsum associated with septal fractures. There are many surgical approaches that can be used to expose the nasal bones, but we present a case where the subciliary incision was used to reduce and fixate the fracture in a patient with combined zygomatic fractures. Owing to the abundance of other concom

Sister's fracture history may be associated with perimenopausal bone fragility and modifies the predictability of fracture risk.

SUMMARY: The present study investigated the effects of first degree relatives' fractures on fracture incidence after the menopause. Sister's, but not other relatives', wrist or hip fracture history was associated with increased risk of fragility fractures after the menopause. This suggests genetic predisposition to bone fragility among postmenopausal women. OBJECTIVE: The aim of the present study was to investigate the association between first degree relatives' fractures and perimenopausal bone fragility. MATERIALS AND METHODS: The study sample of 971 perimenopausal women was extracted from

Temporal variation of applied inter fragmentary displacement at a bone fracture in harmony with maturation of the fracture callus.

The amplitude of inter fragmentary displacement in long bone fractures greatly influences the pattern and speed of healing. Unfortunately, the amplitude of natural cyclical displacement arising from patient activity is random because of the inherent flexibility of fixation devices under natural loading. Although fixators may be designed to control the amplitude of this displacement, the amplitudes most beneficial to healing have not been determined. Furthermore, the appropriate amplitude must vary during healing as the reparative tissue (callus) progresses histologically and stiffens during ma

Hip bone density is associated with concomitant distal ulna fracture in the setting of distal radius fracture.

UNLABELLED: Lower hip bone density showed the most consistent association with distal ulna involvement in surgically treated distal radius fractures, whereas local fracture severity showed minimal discriminatory value. Hip bone density was particularly associated with the ulnar metaphyseal subtype, characterizing conditional fracture-pattern variation within this selected population. PURPOSE: To characterize clinical and skeletal features across fracture subtypes within surgically treated low-energy distal radius fractures and to distinguish ulnar metaphyseal fracture from ulnar styloid fract
