Cheek drooping in 2 patients with maxillary fractures after rigid fixation with bioabsorbable mesh.

Bioabsorbable plate-screw systems are commonly used for the internal fixation of facial bone fractures. The anterior maxilla has a unique curved shape, and fractured bony fragments tend to be small and fragile; therefore, more effective rigid fixation can be achieved using a molded bioabsorbable mesh rather than a bioabsorbable plate. Herein, we describe 2 patients with cheek drooping after a rigid fixation of comminuted maxillary fracture using bioabsorbable meshes and screws. The postoperative courses were uneventful, but both showed soft tissue bulging in the cheek area of the operation site

Successful short-segment instrumentation and fusion for thoracolumbar spine fractures: a consecutive 41/2-year series.

STUDY DESIGN: A retrospective review of all the surgically managed spinal fractures at the University of Missouri Medical Center during the 41/2-year period from January 1989 to July 1993 was performed. Of the 51 surgically managed patients, 46 were instrumented by short-segment technique (attachment of one level above the fracture to one level below the fracture). The other 5 patients in this consecutive series had multiple trauma. These patients were included in the review because this was a consecutive series. However, they were grouped separately because they were instrumented by long-segm

Extra-articular proximal tibial fractures: nail or plate?

The surgical goals for treating proximal tibial fractures are to restore articular congruity, the mechanical axis, and knee motion while avoiding soft-tissue complications. The fracture pattern should be correctly identified and understood. For fractures with minimal intra-articular extension, fracture fixation with an intramedullary nail can decrease the risk of infection because it uses a small incision that is not placed directly over the injured soft tissue, and it provides better axial load sharing than a plate. Using the semi-extended technique, choosing the correct starting portal, inco

Tibial shaft fractures treated by rigid internal fixation: the early results in a 4-year series.

Ninety-nine fractures of the shaft of the tibia in 98 patients were treated by rigid internal fixation over 4 years. One patient died after operation from myocardial infarction; and one patient went abroad. The healing course of the remaining 97 fractures is described, classified according to the type of fracture and the accuracy of operative reduction. Seventy-five fractures had a normal time to union, defined as the lasting achievement of full weight-bearing within 4 months. In 7 fractures the healing period was moderately delayed (full weight-bearing being achieved in 4-6 months) and in 5 i

Treatment of open fractures of the tibial shaft: Ender nailing versus external fixation. A randomized, prospective comparison.

A randomized, prospective study comparing Ender nailing with external fixation for open fractures of sixty-three tibiae (sixty patients) was undertaken. Ender nailing proved to be at least as effective as external fixation with respect to seven parameters: time to union, tibial alignment, total number of operations, ranges of motion of the knee and ankle, pain, presence of infection, and complications. Ender nailing is a safe alternative to external fixation for grade-I and grade-II open fractures. It should not be used for comminuted fractures, which might shorten about the nails.

Tibial Spine Fractures in Children: Evaluation, Management, and Future Directions.

Fractures of the tibial spine are estimated to occur in 3 per 100,000 children annually, but account for 2 to 5% of pediatric knee injuries with effusion. Although these fractures were historically associated with bicycle accidents, the surge of organized youth sports in recent decades has brought renewed attention to this injury. While minimally displaced fractures can be treated nonoperatively, several techniques have been described for fixation of displaced or comminuted fractures. Sequelae of this injury can include arthrofibrosis, knee instability, and nonunion. Future collaborative endea

Predictors of unstable distal radius fractures: a systematic review and meta-analysis.

UNLABELLED: The aim of this study was to perform a systematic review in order to identify predictors of secondary displacement in distal radius fractures. We performed a systematic review and identified all studies that reported secondary displacement following distal radius fractures. Where possible, we pooled the odds ratios of predictors. The initial search yielded 3178 studies of which 27 were included. Multiple studies found that age, shortening, volar comminution, loss of radial inclination, the presence of a volar hook, AO type 3 fractures (A3, B3, C3) and the Older classification were

Wrist arthroscopy and intra-articular fractures of the distal radius in young adults.

UNLABELLED: The difficulty on treatment of articular fractures of the distal radius comes from their comminutive character and frequency of ligamentous lesions associated, turning diagnosis and assessment always subject to discussion. Fracture reduction and fixation under arthroscopic control, along with ligamentous injury treatment, provides the Surgeon with a valuable tool in order to achieve best results on follow-up. Eighteen fractures of the distal radius on 18 young adults (average age of 28.6 years), all intra-articular and 9 with ligamentous lesions associated, were operated on under a

Surgical treatment of traumatic injuries of the cranial base.

Skull-base fractures involve one or more of the cribriform plate of the ethmoid bone, orbital plate of the frontal bone, sphenoid bone, occipital bone, and petrous or temporal bone. Although the fractures themselves only require reduction and reconstruction when the skull base is severely comminuted and altered, even small fractures and the traumatic shear forces can create tears in the meninges and thus predispose to cerebrospinal fluid leaks. This article explores the preoperative, intraoperative, and postoperative management of skull-base fractures, which most commonly involves resolution o

Arthroscopic reduction of adult comminuted tibial eminence avulsion with single tunnel pull-through suture fixation: A case report of technical note.

INTRODUCTION AND IMPORTANCE: Tibial eminence avulsion is an exceptional knee injury in the adult population. The treatment of this injury is generally based on the pattern of the fragment fracture displacement and operative treatment is recommended for comminuted eminence fracture to prevent nonunion and knee instability. CASE PRESENTATION: We reported a 30-years-old female with severe left knee pain after falling from a motorcycle. Physical examination showed moderate left knee effusion with restricted knee joint range of motion from 0 to 90°. Anterior drawer test was positive without openin

Complications of clavicle fracture and acromioclavicular joint rupture. What the general surgeon should know.

Fractures of the clavicle and acute dislocations of the acromioclavicular joint are common seen in the emergency room. Displaced or comminuted clavicle fractures and acromioclavicular joint ruptures (types III-VI) are associated with complications, such as subclavian vessels injury, hemopneumothorax, scapula-thoracic dissociation or brachial plexus paresis. Herein we discuss the usually complications seen after these injuries and underline whatever a trauma surgeon should know in order to avoid fatal situations.

Radiologic and facial morphologic long-term results in treatment of orbital floor fracture with flexible absorbable alloplastic material.

PURPOSE: Although orbital floor fractures are frequently treated by the Ethisorb patch or polydioxanone foil, the utility of these treatments in extensive fractures remains controversial. The purpose of this study was to examine objectively the extent to which such flexible absorbable materials can restore orbital geometry in comminuted and defect fractures. MATERIALS AND METHODS: Twenty-one patients with isolated comminuted or defect fractures of the orbital floor (mean, 4.32 cm(2)) were recruited for this retrospective study. Using an infraorbital approach, 15 patients received an Ethisorb

Combined Orbital Fractures: Surgical Strategy of Sequential Repair.

BACKGROUND: Reconstruction of combined orbital floor and medial wall fractures with a comminuted inferomedial strut (IMS) is challenging and requires careful practice. We present our surgical strategy and postoperative outcomes. METHODS: We divided 74 patients who underwent the reconstruction of the orbital floor and medial wall concomitantly into a comminuted IMS group (41 patients) and non-comminuted IMS group (33 patients). In the comminuted IMS group, we first reconstructed the floor stably and then the medial wall by using separate implant pieces. In the non-comminuted IMS group, we reco

Arthroscopic Management of Tibial Spine Avulsion Fractures: Principles and Techniques.

Tibial spine fractures are uncommon injuries affecting the insertion of the anterior cruciate ligament on the tibia. They typically occur in skeletally immature patients aged 8 to 14 years and result from hyperextension of the knee with a valgus or rotational force. Diagnosis is based on history, physical examination, and standard radiographs. The use of MRI can identify entrapped soft tissue that may prevent reduction. Open or arthroscopic repair is indicated in patients with partially displaced fractures (>5 mm) with one third to one half of the avulsed fragment elevated, in patients who hav

[Intramedullary nailing of old femoral fracture in children].

Intramedullary nailing was performed for 40 old femoral fractures in children. Open intramedullary nailing was used in all fractures. The results showed that intramedullary nailing was suitable for upper and middle third femoral fractures, including transverse or short oblique fractures, multiple segmental fractures, malunited fractures and fractures failed to unite after internal fixation with plate. The authors consider that fresh long oblique or spiral fractures, severe comminuted fractures and lower third femoral fractures are not suitable for intramedullary nailing. In this series, 34 pat
