Diffuse mesenteric lipodystrophy (massive fat necrosis) in a cat.

Clinical, radiographic, and pathological findings of diffuse mesenteric lipodystrophy in a 3-year-old, female domestic short-haired cat are described. The animal had fallen from a height of approximately 20 metres 6 weeks previously, after which it developed respiratory distress and anorexia. The cat was thin, exhausted and had rapid, shallow, open-mouthed breathing. Serial upper gastrointestinal contrast radiographs showed a prolonged transit time of the contrast medium due to restricted gastric emptying, and filling defects of the duodenojejunal portions of the small intestines. The cranial

[Postoperative fistulas. How to close them].

Postoperative enterocutaneous fistulas are usually the result of an anastomotic leak. The likelihood of a postoperative fistula developing depends on concomitant diseases. An established gastrointestinal fistula is associated with considerable morbidity and mortality. Appreciable losses of fluid, protein and electrolytes often result in malnutrition, which has a negative impact on wound infection and the mental performance of the patient. Conservative treatment must aim to compensate these deficits as early as possible. Adequate amounts of energy (carbohydrates and fats) vitamins and trace ele

Colonic transit studies and colonic manometry in pediatric constipation.

OBJECTIVES: In a subgroup of children with refractory constipation, colonic function investigations, such as colonic transit scintigraphy (CS) and colonic manometry (CM), are used to define the underlying pathophysiologic mechanisms. There are limited studies comparing colonic transit and contractile function. We aimed to correlate CS and CM and assess whether specific manometric patterns might predict the luminal transit findings. METHODS: Children with refractory constipation undergoing both CS and CM between 2012 and 2022 at two Tertiary Pediatric Gastroenterology Services were retrospecti

The particulate passage rate, nutrient composition and fermentation characteristics across gastrointestinal tracts in lactating dairy cows fed three different forage source diets.

This study was conducted to investigate the particulate passage rate, nutrient characteristics and fermentation parameters across the gastrointestinal tract (GIT) in lactating dairy cows fed cereal straws in comparison with alfalfa hay. Eighteen multiparous Holstein cows were randomly assigned to one of three experimental diets consisting of 55% concentrate, 15% corn silage and 30% different forage sources as follows (% of dry matter [DM]): (i) 23% alfalfa hay and 7% Chinese wild rye hay (AH); (ii) 30% corn stover (CS); and (iii) 30% rice straw (RS). The Cr-mordanted corn silage-neutral deterg

Anatomical registration and three-dimensional visualization of low and high-resolution pan-colonic manometry recordings.

BACKGROUND: Colonic propagating sequences (PS) are important for the movement of colonic content and defecation, and aberrant PS patterning has been associated with slow transit constipation. However, because these motor patterns are typically recorded over long periods (24 h +), the visualization of PS spatiotemporal patterning is difficult. Here, we develop a novel method for displaying pan-colonic motility patterns. METHODS: A 3D mesh representing the geometry of the human colon was created as follows: (i) Human colon images from the Visible Human Dataset were digitized to create a 3D data

[Duodenogastric reflux in patients with recurrent postoperative complicated duodenal ulcers].

Duodenogastric reflux (DGR) was revealed in 52.6% patients, suffering recurrent postoperative complicated duodenal ulcers (RPOCDU). Pylorodestructive operations performance, pyloric involvement into ulcerative infiltrate and absence of chronic duodenal impassability (CHDI) correction during the first operation done had constituted the DGR occurrence causes. While establishing the indications for elective operation performance as well as choosing the surgical method of the RPOCDU treatment it is necessary to take into account the presence and severity degree of DGR. Surgical treatment of DGR mu

Evaluation of continuous infusion of lidocaine on gastrointestinal tract function in normal horses.

OBJECTIVE: To determine the effect of continuous infusion of lidocaine on fecal transit time in normal horses. STUDY DESIGN: Experimental randomized cross-over study. ANIMALS: Healthy horses (n=6). METHODS: Barium-filled microspheres were administered to horses by nasogastric intubation and feces were collected every 2 hours for 4 days. A bolus of 2% lidocaine (1.3 mg/kg) was administered randomly, followed by a continuous infusion of lidocaine (0.05 mg/kg/min) for 3 days or an equivalent volume of saline. The washout period was 10 days. Variables assessed included defecation frequency, wei

Effect of cholecystokinin octapeptide and atropine on human colonic motility, tone, and transit.

The role of cholecystokinin (CCK) in postprandial control of colonic motility is controversial. To test the hypothesis that CCK stimulates colonic tone, motility, and transit we measured these colonic functions in 16 healthy subjects using intraluminal manometry, barostatic balloon measurements, and radioscintigraphy. This was a randomized-order, double-blind, sequential study design in each subject of saline and either atropine (0.01 mg/kg stat and 0.01 mg/kg/hr by infusion) or CCK-octapeptide (OP, 30 ng/kg stat and 60 ng/kg/hr by infusion). Atropine was used as control to demonstrate respons

[Evaluation of small intestinal motility].

Disturbances of small bowel motor function are increasingly recognized in clinical practice, either in the setting of an underlying disease that may affect the neuro-hormonal control of gut motility, such as diabetes or scleroderma, or as part of unexplained intestinal dysfunctions such as the irritable bowel syndrome or chronic idiopathic intestinal pseudo-obstruction. In the absence of endoscopic or radiological mucosal disease, it is often clinically helpful to define the motor function of the small bowel to understand the origin of the patient's symptoms. Fluid loading of the human colon: effects on segmental transit and stool composition.

BACKGROUND: Because segmental storage by the colon is relevant to diarrheal states, we quantified colonic transit in 13 healthy volunteers. METHODS: We infused radiolabelled liquids and solids into the cecum. Gamma camera images were obtained for 48 hours and counts measured in ascending (AC), transverse (TC), descending (DC), and rectosigmoid (RS) colons. Bowel movements were scored for consistency and stool outputs of isotopes were quantified. RESULTS: No volunteers developed diarrhea. Times for isotopes to transverse AC and TC were shorter with rapid infusions (P < 0.001). Counts remainin

Surgical aspects of severe chronic non-Hirschsprung constipation.

This paper reviews the methods available for selecting patients with disabling non-Hirschsprung constipation eligible for surgery, and the surgical methods reported. It is concluded that careful evaluation of the patient may help in selecting the most suitable treatment. The demonstration, however, of a specific morphological abnormality would, of itself, not seem to justify surgical correction. The surgical criteria remain to be defined by additional physiological research, and the outcome of the various surgical procedures is always uncertain. Surgery should only be undertaken in patients wi

Exploration of Exercise-Induced Gastrointestinal Syndrome in Endurance-Trained Adolescents: Is There Cause for Concern in Regard to Ultra-endurance Event Participation.

BACKGROUND: Exercise-induced gastrointestinal syndrome (EIGS) in response to prolonged exercise is well-established in adult athletes but no study has investigated how EIGS presents in adolescent athletes despite an increase in youth participation in ultramarathon events. This study aimed to determine the impact of prolonged exercise on gastrointestinal integrity, functional responses, systemic immune profile, and gastrointestinal symptoms (GIS) in youth (YOUTH) vs adult (ADULTS) athletes. METHODS: Twelve youth (≤ 18y) and twelve adults (≥ 30-50y) completed 3 h treadmill running at 60% V̇O2ma

Hirschsprung Disease: The Rise of Structured Transition and Long-term Care.

Hirschsprung disease, like most surgically correctable congenital anomalies, is now survivable well into adulthood. Patients are, therefore, presenting later in life with problems that were previously infrequently encountered by colorectal surgeons or gastroenterologists. The goals of our review are to describe the current state of care transition for this patient population, identify the specific needs for these patients, and propose guidelines which could inform a future model of transition from the pediatric to adult setting. This is a review of the current state of care transition and long

Diagnosis and treatment of chronic constipation--a European perspective.

BACKGROUND: Although constipation can be a chronic and severe problem, it is largely treated empirically. Evidence for the efficacy of some of the older laxatives from well-designed trials is limited. Patients often report high levels of dissatisfaction with their treatment, which is attributed to a lack of efficacy or unpleasant side-effects. Management guidelines and recommendations are limited and are not sufficiently current to include treatments that became available more recently, such as prokinetic agents in Europe. PURPOSE: We present an overview of the pathophysiology, diagnosis, cur

Postgastrectomy pancreatic malabsorption: is there a case for intervention?

More patients are surviving surgery for gastric carcinoma than ever before. The possible causes of malabsorption following total gastrectomy are multifactorial, and pancreatic insufficiency has been proposed as one mechanism. Other contributing factors include loss of gastric reservoir, rapid small bowel transit, small bowel bacterial overgrowth and the type of operation performed. Although pancreatic exocrine insufficiency has been demonstrated after gastrectomy, the key question remains whether pancreatic enzyme supplements offer any substantial clinical benefit to the patient. The evidence

[Irritable bowel syndrome in children].

In the adult, the irritable bowel syndrome is characterized by intestinal transit disorders associated or not with chronic abdominal pain. Two different forms can be seen: in one, pain and constipation are predominant, while in the other, pain and diarrhea alternate. The second form is encountered with predilection in the child. Various terms can be used to name the syndrome including colitis, non specific or benign colitis, irritable bowel syndrome in the child, infantile diarrhea, and others, all of which attests to our ignorance of the pathophysiology of this disorder. This syndrome is by f

Concordance Between Pan-Intestinal Capsule Endoscopy and Colonoscopy for the Detection and Characterization of Colonic Lesions: Post Hoc Analysis on the Impact of Colonic Transit Time.

INTRODUCTION: Pan-intestinal capsule endoscopy (PCE) is increasingly used as a minimally invasive alternative to colonoscopy in the evaluation of mid- and lower gastrointestinal bleeding (MLGIB). However, the impact of colonic transit time (CTT) on diagnostic performance remains insufficiently characterized. This study aims to assess the concordance between PCE and colonoscopy for colorectal lesions and to determine whether CTT influences diagnostic accuracy. METHODS: This post hoc analysis of a prospective single-center trial included patients with suspected MLGIB who underwent PCE followed

Sex- and Gender-Related Differences in Common Functional Gastroenterologic Disorders.

Functional gastrointestinal (GI) disorders (FGIDs) result from central and peripheral mechanisms, cause chronic remitting-relapsing symptoms, and are associated with comorbid conditions and impaired quality of life. This article reviews sex- and gender-based differences in the prevalence, pathophysiologic factors, clinical characteristics, and management of functional dyspepsia (FD) and irritable bowel syndrome (IBS) that together affect approximately 1 in 4 people in the United States. These conditions are more common in women. Among patients with IBS, women are more likely to have severe sym
