Emerging Infectious Diseases in Pregnancy.

It has been recognized for centuries that pregnant women have unique susceptibilities to many infectious diseases that predispose them to untoward outcomes compared with the general adult population. It is thought a combination of adaptive alterations in immunity to allow for the fetal allograft combined with changes in anatomy and physiology accompanying pregnancy underlie these susceptibilities. Emerging infectious diseases are defined as those whose incidence in humans has increased in the past two decades or threaten to increase in the near future. The past decade alone has witnessed many

Retinal disease course in Usher syndrome 1B due to MYO7A mutations.

PURPOSE. To determine the disease course in Usher syndrome type IB (USH1B) caused by myosin 7A (MYO7A) gene mutations. METHODS. USH1B patients (n = 33, ages 2-61) representing 25 different families were studied by ocular examination, kinetic and chromatic static perimetry, dark adaptometry, and optical coherence tomography (OCT). Consequences of the mutant alleles were predicted. RESULTS. All MYO7A patients had severely abnormal ERGs, but kinetic fields revealed regional patterns of visual loss that suggested a disease sequence. Rod-mediated vision could be lost to different degrees in the fir

Developmental profiles in preschool children with autism spectrum disorders referred for intervention.

The aim was to characterize the panorama of developmental disorders in 208 preschool children with a clinical diagnosis of autism spectrum disorder (ASD), referred to a specialized centre, the Autism Centre for Young Children (ACYC), for intervention. At the centre, a research team examined all children according to structured protocols and interviews. All available test data from their assessments prior to referral were scrutinized. The boy:girl ratio was 5.5:1. In 22% of the total group a period of regression, including speech and language, had occurred. Epilepsy had been diagnosed in 6% of

Interferons and autoimmune disorders.

Interferons are ubiquitous cytokines produced by all mononuclear cell types in response to infection by a DNA or RNA virus. There are three major classes of interferons: type I or nonimmune interferons consist chiefly of interferons alpha produced by leukocytes and of interferon beta produced by fibroblasts, although there are several other less important variants; type II or immune interferon is interferon gamma, which is mainly produced by NK cells and T cells; and type III consists of the lambda interferons. Each type is characterized by a specific receptor and signal transduction pathway.

Toward an evolutionary and syndromic psychiatry: Dogs as a comparative model for the neuroscience of psychopathology.

Psychiatric disorders arise from dysfunctions of neural systems that are deeply conserved across mammals, yet contemporary psychiatric classification remains fundamentally anthropocentric, relying heavily on subjective experience, symbolic language and self-report. This creates a major challenge for veterinarian psychiatrists and for comparative neuroscience: how can psychopathology be characterized across species when introspective access is unavailable? The domestic dog provides a uniquely powerful model for addressing this question, combining conserved affective and cognitive neural circuit

Immunomodulatory Roles of Probiotics: From Intestinal Barrier Regulation to Clinical Applications.

The mammalian gut is a highly dynamic physiological ecosystem that harbors an enormous number of commensal bacteria that play an essential role in the development of various cells of the immune system. The gut bacteria, both resident and diet-derived, share a habitat with immune cells located within the lamina propria layer of the intestinal villi. The intestinal epithelial cells do not come into direct contact with the gut microbiota, but microbial metabolites play a significant role in their maturation and appropriate function. According to the World Health Organization, probiotics are "live

Ectopic NMDAR expression in cancer unmasks germline-encoded autoimmunity.

Autoimmunity and anti-cancer immunity lie on the same biological continuum1,2, but their link remains obscure. The paraneoplastic neurological syndrome ANRE (anti-NMDA receptor (NMDAR) encephalitis) is a paradigm for their connectivity3, given that intratumoural NMDAR expression is correlated with the generation of anti-NMDAR antibodies4,5. Here we verify ectopic expression of GluN1 and GluN2B NMDAR subunits in triple-negative breast cancer (TNBC)6 and model this using orthotopic TNBC tumours with inducible expression of GluN1-GluN2B NMDARs. We show that NMDAR expression is sufficient to induc

Exploring the impact of Ramadan fasting on women's reproductive health: A narrative review.

Ramadan is the ninth month of the Islamic lunar calendar in which Muslims abstain from consumption of food and drink, smoking, and engaging in sexual activity during daylight hours for the full lunar month, approximately 29-30 days. Beyond its spiritual significance, fasting during this month represents a distinct physiologic and cultural practice that may influence women's reproductive health. This narrative review integrates findings from published studies to summarize current evidence on the impact of Ramadan fasting (RF) on the menstrual cycle, fertility, polycystic ovary syndrome, pregnan

Role of natural inter-individual variability in the different penetrance of congenital long QT syndromes (LQTS) types 1 and 2: an in silico approach.

Congenital Long QT Syndrome types 1 and 2 (LQT1 and LQT2) are caused bymutations in the KCNQ1 and KCNH2 genes, responsible for the IKs and IKr currents, respectively. However, the penetrance of these mutations is highly variable, since some carriers remain asymptomatic, while others exhibit severe clinical manifestations. To elucidate how physiological variability in other cardiac ionic currents may influence the arrhythmic phenotype, human ventricular action potentials were simulated in silico using the O'Hara-Rudy dynamic model implemented in the Virtual Assay software. Conductances of nine

Context-dependent regulation of IL-6 in exercise and colorectal cancer cachexia.

Colorectal cancer (CRC) is a common malignancy of the digestive system and remains a major cause of cancer-related morbidity and mortality worldwide. Its development and progression are influenced by genetic susceptibility, lifestyle factors, metabolic dysregulation, and chronic inflammatory states. Cancer cachexia is a multifactorial wasting syndrome characterized by weight loss, skeletal muscle atrophy, metabolic disturbances, and functional decline, which substantially impairs quality of life and survival outcomes in patients with CRC. In this context, interleukin-6 (IL-6) has emerged as a

Differential Causal Associations of Gut Microbiota, Blood Metabolites, and Immune Cell Phenotypes With Early- and Late-Onset Alzheimer's Disease: A Bidirectional Mendelian Randomization Analysis.

BACKGROUND: Alzheimer's disease (AD) is a heterogeneous syndrome with distinct genetic and clinical profiles between early-onset AD (EOAD) and late-onset AD (LOAD) subtypes. However, specific causal etiologies linking the gut microbiota-immune-metabolic axis to these subtypes remain poorly understood. METHODS: We employed a comprehensive bidirectional two-sample Mendelian randomization (MR) framework to systematically investigate the causal associations of gut microbiota, immune cell phenotypes, and blood metabolites with EOAD and LOAD. Large-scale genome-wide association study (GWAS) summary

Multimodal AI-driven Biomarker for Early Detection of Cancer Cachexia.

Cancer cachexia, a multifactorial metabolic syndrome characterized by severe muscle wasting and weight loss, contributes to poor outcomes across various cancer types but lacks a standardized, generalizable biomarker for early detection. We present a multimodal AI-based biomarker trained on real-world clinical, radiologic, laboratory, and unstructured clinical note data, leveraging foundation models and large language models (LLMs) to identify cachexia at the time of cancer diagnosis. Prediction accuracy improved with each added modality: 77% using clinical variables alone, 81% with added labor

Multi-OMICs analysis on tridimensional fibroblast spheroids to model vascular Ehlers-Danlos syndrome pathogenesis.

Three-dimensional (3D) spheroids are an innovative cellular model mimicking tissue-like properties for a more effective replication of physiological cellular environment. Vascular Ehlers-Danlos syndrome (vEDS) is a rare hereditary connective tissue disorder caused by heterozygous deleterious variants in COL3A1. Affected individuals are at increased risk of early death due to ruptures of arteries, large intestine, and gravid uterus. vEDS cellular pathogenesis is only partially understood and the disease remains without effective treatment. We integrated transcriptomic and proteomic data generat

Implementing total skin electron irradiation in radiotherapy: a structured change management approach.

BACKGROUND: Total skin electron irradiation (TSEI) is a specialized radiotherapy technique used to treat cutaneous T‑cell lymphoma, including mycosis fungoides and Sézary syndrome. Despite its clinical benefits, TSEI is rarely implemented in clinical practice due to significant technical and organizational challenges. This study explores a structured approach to integrating TSEI into clinical routines by applying a comprehensive change management framework in a German radiotherapy department. METHODS: The implementation process was based on Kotter's eight-step change management model and carr
