Triangular body-cover model of the vocal folds with coordinated activation of the five intrinsic laryngeal muscles.

Poor laryngeal muscle coordination that results in abnormal glottal posturing is believed to be a primary etiologic factor in common voice d … g finite element models, excised larynges, and clinical studies in sustained and time-varying vocal gestures. Simulations of vocal fold posturing obtained with distinct antagonistic muscle activation yield clear differences in kinematic, aerodynamic, and acoustic measures. The proposed tool is deemed sufficiently accurate and flexible for future comprehens …

The Representation of Objects in Apraxia: From Action Execution to Error Awareness.

Apraxia is a well-known syndrome characterized by the sufferer's inability to perform routine gestures. In an attempt to understand the syndrome better, various different theories have been developed and a number of classifications of different subtypes have been proposed. In this article review, we will address these theories with a specific focus on how the use of objects helps us to better understand upper limb apraxia. With this aim, we will consider transitive vs. intransitive action dissociation as well as less frequent types of apraxia involving objects, i.e., constructive apraxia and m…

Two French-speaking cases of foreign accent syndrome: an acoustic-phonetic analysis.

Foreign accent syndrome (FAS) is an acquired neurologic disorder in which an individual suddenly and unintentionally speaks with an accent which is perceived as being different from his/her usual accent. This study presents an acoustic-phonetic description of two Quebec French-speaking cases. The first speaker presents a perceived accent shift to Acadian French (French spoken in the easternmost provinces of Canada), whereas the second acquired an accent identified as Germanic. Speech seems affected by constraints on the coordination of articulatory gestures, expressed by distortions in the pro…

Altered connectivity and action model formation in autism is autism.

Internal action models refer to sensory-motor programs that form the brain basis for a wide range of skilled behavior and for understanding others' actions. Development of these action models, particularly those reliant on visual cues from the external world, depends on connectivity between distant brain regions. Studies of children with autism reveal anomalous patterns of motor learning and impaired execution of skilled motor gestures. These findings robustly correlate with measures of social and communicative function, suggesting that anomalous action model formation may contribute to impair…

A model-based approach to long-term recovery of limb apraxia after stroke.

Limb apraxia is a disorder affecting performance of gestures on verbal command (pantomime), on imitation, and/or in tool and action recognition. We aimed to examine recovery on tasks assessing both conceptual and production aspects of limb praxis in left (n = 22) and right (n = 15) stroke patients. Patients were assessed longitudinally on four conceptual tasks (action identification, tool naming by action, tool identification, and tool naming) and five production tasks (pantomime, pantomime by picture, concurrent imitation, delayed imitation, and tool use). They were grouped as impaired or not…

Hyoid-bolus transit latencies in normal swallow.

Pharyngeal swallow delay is frequently found in dysphagic patients and is thought to be a factor in a range of swallowing problems, including aspiration. Implicit in notions of swallow "delay" is a temporal interval between two events that is longer than normal. However, there appears to be little agreement about which referent events should be considered in determining delay. A number of pharyngeal bolus transit points and various pharyngeal gestures have been used in delays determined from fluoroscopic evidence, and other referents have been used in electromyographic and manometric studies o…

Suicide by overdoses of medicines among the aged.

The current literature on suicide by medicinal overdose among the elderly population of the United States and Britain is discussed. The olde … ly is complicated by the fact that any attempt at suicide usually is serious and rarely preceded by gestures. Depression, based on social factors, is common among older people. Some of them face problems of declining income and prestige, as well as a loss of physical and mental powers. A change in the attitude of society towards the elderly may help to all …

Nasal emission as a component of the misarticulation of sibilants and affricates.

Nasal emission confined solely to the sibilants or sibilants and affricates may be encountered as an articulatory phenomenon in speakers who … rossly deviant, perhaps owing to such factors as malocclusion and earlier compensatory articulation gestures. Many such cases prove impervious to extensive articulation therapy, yet physical management may constitute "over-correction" with undesirable sequelae. Inventive approaches to articulation therapy may be required in these cases, since correction of …

Entry Point Localisation for Percutaneous Surgical Robots Based on Concentric Fiducial Patches and Robust Surface Fitting.

BACKGROUND: Accurate entry point localisation is critical for the safety of robot-assisted percutaneous interventions (RAPI). However, medical optical tracking systems for the traditional fiducial patch are cost-prohibitive. To address these challenges, this paper proposes a robust entry point and gesture localisation framework based on an inexpensive concentric fiducial patch and robust surface fitting. METHODS: We introduce an anisotropic iteratively reweighted least squares (IRLS) algorithm integrated with a Tukey biweight M-estimator, employing a deterministic soft-weighting strategy to e

Modelling the early expressive communicative trajectories of infants/toddlers with early cochlear implants.

For children with normal hearing (NH), early communication skills predict vocabulary, a precursor to grammar. Growth in early communication skills of infants with cochlear implants (CIs) was investigated using the Early Communication Indicator (ECI), a play-based observation measure. Multilevel linear growth modelling on data from six ECI sessions held at three-monthly intervals revealed significant growth overall, with a non-significant slower growth rate than that of children with NH (comparison age centred at 18 months). Analyses of gesture use and of nonword vocalisations revealed the CI g

Committing Suicide With Handgun Wounds in the Head Using a Defective Firearm: About 2 Unusual Autopsy Cases.

We report 2 unusual autopsy cases with head handgun wounds using defective firearms in a suicidal context. In the first case, a 78-year-old man was found hanged from a tree, with a Lefaucheux revolver lying on the tree. In the right temporal region, there was an entrance wound with a bullet still in place, responsible for a mild cerebral contusion, without bone defect. The main cause of death was hanging. In the second case, a 60-year-old man died at home after shooting himself in the head several times with a 22-long rifle revolver. The autopsy showed 13 entrance wounds on the scalp. Most of

Motor System Pathology in Psychosis.

PURPOSE OF REVIEW: Motor abnormalities are an intrinsic feature of psychosis. Neurological soft signs, Parkinsonism, dyskinesia, and other motor phenomena are frequently observed in subjects at clinical or genetic risk for psychosis as well as first-episode patients, chronic patients. Here, we review the most recent literature on motor assessments and pathophysiology in psychosis. RECENT FINDINGS: Instrumental measures of fine motor performance, balance, spontaneous motor activity, and gesture indicated motor abnormalities in subjects at risk and across stages of schizophrenia. Motor phenomen

The speed of articulatory movements involved in speech production in children with dyslexia.

A group of children with dyslexia (mean ages 9 and 14 years) was studied, together with group of children without dyslexia matched for age. Participants were monolingual native speakers of the Bosnian language with transparent orthography. In total, the diagnostic tests were performed with 41 children with dyslexia and 41 nondyslexic children. The participants were asked to produce monosyllables, /pa/, /ta/, and /ka/, and the trisyllable /pataka/, as fast as possible. Analysis was undertaken in four ways: (1) time of occlusion duration for plosives (duration of stop), (2) voice onset time for

Prosody in apraxia of speech.

Prosody is a complex process that involves modulation of pitch, loudness, duration, and linearity in the acoustic stream to serve linguistic and affective communication goals. It arises from the interaction of distributed neural networks that may be anatomically and functionally lateralized. Intrinsic prosody is mediated largely through left hemisphere mechanisms and encompasses those elements of linguistic microstructure (e.g., syllabic magnitudes and durations, basic consonantal and vocalic gesture specifications, and so) that yield the segmental aspects of speech. Extrinsic prosody is proce

Neural bases of imitation and pantomime in acute stroke patients: distinct streams for praxis.

Apraxia is a cognitive disorder of skilled movements that characteristically affects the ability to imitate meaningless gestures, or to pantomime the use of tools. Despite substantial research, the neural underpinnings of imitation and pantomime have remained debated. An influential model states that higher motor functions are supported by different processing streams. A dorso-dorsal stream may mediate movements based on physical object properties, like reaching or grasping, whereas skilled tool use or pantomime rely on action representations stored within a ventro-dorsal stream. However, give…

Experiences of loneliness among older deaf adults who use sign language.

Loneliness is recognized worldwide as a risk to well-being and health. It emerges from the discrepancy between our expectations of social relationships and the reality. However, very little is known about the experiences of loneliness among older deaf adults. The aim of the study was to describe the experiences and expressions of loneliness among 12 older deaf adults who use sign language. We interviewed them, and the interview transcripts were analyzed inductively. Loneliness manifested itself in a variety of ways in the narratives, facial expressions, postures, movements, gestures, and the v…
