[Transitional cell carcinoma of pelvis in a giant hydronephrotic kidney].

A 73-year-old man complained of macroscopic hematuria. Preoperative diagnosis was left giant hydronephrosis due to UPJ stenosis. Resected kidney unsuspectedly contained multiple papillary tumors. Ureterectomy and partial cystectomy was additionally performed. Histological examination showed transitional cell carcinoma. Urinary stasis in the dilated pelvis may have been one of the causes of carcinomatous changes in the epithelium in our case.

Combination therapy for anaplastic giant cell thyroid carcinoma.

Since 1981, 20 patients with anaplastic giant cell carcinoma of the thyroid have been prospectively treated according to a combination regimen of chemotherapy and external beam radiation therapy. Two types of chemotherapy were used every 4 weeks, depending on the patient's age. For those younger than 65 years, a combination of doxorubicin (60 mg/m2) and cisplatin (90 mg/m2) was given, and for older patients mitoxantrone (14 mg/m2) was used. Radiotherapy was carried out between Day 10 and Day 20 of the first four cycles of chemotherapy. It delivered 17.5 Gy in 7 fractions to the neck and the su

Noonan-like/multiple giant cell lesion syndrome.

A patient with the Noonan-like/multiple giant cell lesion syndrome is reported and the findings in 14 cases are reviewed. Impressive manifestations include short stature, low normal intelligence or developmental delay, ocular hypertelorism, prominent posteriorly angulated ears, giant cell lesions of bones, joints, and/or soft tissues, pectus excavatum, and pulmonic stenosis. It has been difficult to delineate the syndrome because problems in identifying the condition have resulted from incomplete or truncate ascertainment by various medical specialists.

Temporal artery morphology and morphometry in giant cell arteritis.

Paraffin sections and more than 7,000 plastic cross-sections of temporal arteries from 27 patients with a clinical diagnosis of polymyalgia … nternal elastic membrane (IEM) with a spatially correlated, mononuclear cell infiltration. Isolated giant cells were also found to attack the IEM in these cases. The majority of the biopsies displayed a different picture with a diffuse macrophage attack on media and intima with numerous and apparently macrophage-derived giant cells, which did not attack calci …

Giant-cell reparative granuloma of the hand and foot bones.

Giant-cell reparative granuloma (GCRG) is an uncommon benign reactive intraosseous lesion. It occurs in the skull, jaw, hand, foot, and facial bones and rarely in other skeletal sites. It is a solitary, lytic, expanded lesion and infrequently may extend into the surrounding soft tissue. Histologically, it is composed of fibrous stroma with spindle-shaped fibroblasts, multinucleated giant cells, and inflammatory mononuclear cells. Areas of hemorrhage are uniformly present. It may be difficult to distinguish this entity from an aneurysmal bone cyst, giant-cell tumor, or brown tumor of hyperparat…

Bilateral orbital involvement in fatal giant cell polymyositis.

The case of a young woman with giant cell polymyositis is described. She had bilateral, severe, midly painful proptosis and ophthalmoplegia. Extensive pharyngeal, laryngeal, and cardiac muscle involvement occurred 18 months later coincident with fatal cardiac arrhythmia. At autopsy, extensive muscle necrosis and giant cells were noted in extraocular, pharyngeal, laryngeal, and cardiac muscle with only minimal involvement of other striated muscles.

Cranial diabetes insipidus caused by giant cell arteritis.

A 78 year old woman was admitted with symptoms of diabetes insipidus and was found to have a high ESR, the cause of which was uncertain. Five months later she presented with blindness, and cranial arteritis was confirmed by temporal artery biopsy. The endocrine abnormality returned to normal after steroid therapy. The association between these two conditions is discussed.

Giant cell arteritis of the skin simulating erythema nodosum.

Cutaneous involvement in giant cell arteritis is quite uncommon. A patient is described who presented with pretibial skin lesions clinically indistinguishable from erythema nodosum which, on biopsy, showed subcutaneous pannicular giant cell vasculitis. Cutaneous manifestations of giant cell arteritis are subsequently reviewed.

Pulmonary artery obstruction due to giant cell arteritis.

Giant cell arteritis is often referred to in the context of polymyalgia rheumatica with temporal artery involvement. There are, however, more malignant forms of presentation of this necrotizing arteritis involving either the great vessels of the aorta or, occasionally, the pulmonary arteries. Our case relates to giant cell arteritis presenting as pulmonary artery obstruction in a patient without polymyalgia rheumatica or extensive aortic or proximal great vessel involvement.

Giant-cell arteritis. Histological, immunohistochemical and electronmicroscopic studies.

Biopsies from the temporal artery of 32 patients suspected of giant-cell arteritis were evaluated retrospectively by light microscopy, histochemical, and immunohistochemical methods, as well as by transmission electron microscopy (TEM). At the clinical follow-up the 32 patients included four clinical groups: temporal arteritis (8 patients), polymyalgia rheumatica (10 patients), rheumatoid arthritis (4 patients), and a group of miscellaneous diseases unrelated to inflammatory rheumatic diseases (10 patients). There were a number of similarities between age-related alterations in the arteries an

The following histologic parameters were assessed: fractional surface area occupied by giant cells (FSA), relative size index of giant cells (RSI), stromal characteristics, mitotic index, inflammatory cells, and hemosiderin content. Brain stem infarction as a complication of giant-cell arteritis.

Two cases of brain stem infarction as an early and fatal complication of giant-cell arteritis are reported. These complications occurred despite adequate treatment with corticosteroids. The findings at autopsy are compared with those of the literature. The possible pathogenetic mechanisms of vertebro basilar occlusion and the therapeutical implications are discussed.

Giant cell arteritis diagnosed following arm claudication.

A patient is described whose initial symptoms, though suggestive of polymyalgia rheumatica, were attributed to psoriatic arthritis, and whose subsequent development of claudication of the left arm led to the biopsy proven diagnosis of giant cell arteritis. Attention is drawn to widespread arterial involvement by giant cell arteritis which, though uncommon, must be considered in all patients with ischemic limb pain. In addition, the consequences of overlooking polymyalgia rheumatica as an early manifestation of giant cell arteritis are outlined.

[Giant cell arteritis: multiform disease of the elderly human].

Six biopsy proven cases of giant-cell arteritis in women beyond the age of 70 years are presented to illustrate the wide range of clinical manifestations of this disease. Besides polymyalgia rheumatica or cranial arteritis the illness may afflict various organs including eye, ear, skin, heart, cerebrum and liver or may mimic a chronic infectious or malignant disease. Diagnosis should be considered in any such cases, especially when laboratory data suggest a chronic infectious disease due to high sedimentation rate, anemia with low serum iron and normal total iron binding capacity and an increa

Lingual infarction in giant cell arteritis. A case report.

Giant cell arteritis is a disease of the elderly, affecting the temporal or other cranial arteries, often in patients with polymyalgia rheumatica. A case of giant cell arteritis with necrosis of the tongue in a 77-year-old woman is reported. It is important to be aware of this condition because of the serious complications. As the lesion is focally distributed, it is necessary to obtain a large biopsy specimen from the superficial temporal artery in order to make a correct diagnosis. Prompt treatment with corticosteroid in large doses followed by a lower maintenance dose is important to preven
