We present two cases of recurrent gynecomastia in men enrolled in a placebo-controlled trial evaluating the efficacy of finasteride in treating benign prostatic hyperplasia. When the pharmacologic records were examined, it was apparent that the breast tissue hyperplasia diminished when the patients become noncompliant with their study medication and then resumed therapy. A case of right mammary invasive ductal carcinoma in young man with atypical ductal hyperplasia and gynecomastia of the second breast is described. The review of the existing literature stress the rarity of this observation. Nevertheless, in all cases of male mammary carcinoma the accurate examination must always include the contralateral breast to search the possible precancerous lesions or etiologic factors.

A 66-year-old man developed right painful gynecomastia following resection of a well-differentiated squamous cell carcinoma from the right upper lobe. In 1979, he had a well-differentiated squamous cell carcinoma resected from the left lower lobe. Extensive investigation did not reveal any definite indication of metastases or residual carcinoma. Radiation therapy is an effective means of preventing the development of hormone-induced gynecomastia in men with cancer of the prostate. The efficacy and morbidity of this type of radiation was studied in a retrospective analysis of 87 patients referred for treatment from 1972 to 1982. Patients receiving DES as treatment for prostate carcinoma were treated with irradiation to the breast tissue. Patients were treated with 4 MV, 60Co superficial X rays. The case of an 11-year-old boy with prepubertal gynecomastia is presented. Endocrine workup, including sex steroid and pituitary hormone concentrations, as well as chromosome analysis, was normal. Microscopy of the mastectomy specimen demonstrated, in addition to ductal proliferation and fibrosis, formation of lobules and acini, simulating female breast tissue. A case of Leydig cell tumor is reported and discussed. The tumor, 1.5 cm in diameter, was located in the capsule of the right testis. Gynecomastia present for 3 months was almost absent 4 months postoperatively. Preoperative hormone analysis revealed a low concentration of serum testosterone, which was normalized 1 week postoperatively. In electron microscopic examination, several intracytoplasmic microbody-like structures and 'tigroid' mitochondria were noted. These findings are similar to the three earlier descriptions of Leydig cell tumors.

A normally-virilized 24-year-old white man who had gynecomastia was found to have a Leydig-cell tumor of the testis, without demonstrable metastases. Hormonal evaluation revealed negative serum and urine tests for human chorionic gonadotropin, and normal steroid excretion. Sixty-six male patients with leprosy were studied. Only those with the lepromatous type developed testicular and epididymal changes. Nine of the 38 patients showed decreased sexual function, and 7 developed gynecomastia. These changes were believed to be due to the altered gonadal state.
