What leads to treatment failure among people living with HIV: first systematic review from the middle east and North Africa.

BACKGROUND: While antiretroviral therapy (ART) has significantly improved human immunodeficiency virus (HIV) outcomes globally, some individuals experience treatment failure (TF). Data on TF rates and determinants in the Middle East and North Africa (MENA) region are limited. METHODS: For the first time, a systematic review following PRISMA guidelines was conducted to synthesize evidence on TF from MENA countries. Electronic databases were searched from inception to May 2025. Studies reporting quantitative TF data among people living with HIV (PLWH) on ART in MENA were included. Failure was ev

Perceptions of dolutegravir-based regimen use among people living with HIV in Limpopo province: A qualitative study.

BACKGROUND: HIV and AIDS continues to be a critical public health challenge, with sub-Saharan Africa shouldering the heaviest burden. Progress towards disease management has been implemented over the years, and antiretroviral therapy (ART) has been the most significant milestone. The World Health Organization recommended dolutegravir as the second line of treatment for the disease. A dolutegravir-based regimen has been appreciated for its great benefits. While its effectiveness is apparent, reports of side effects have raised concerns about adherence, posing a challenge to long-term treatment

Risks and possible implications of weight gain after switch to dolutegravir in a large Ugandan cohort of people with HIV.

OBJECTIVES: We investigated weight gain amongst people with HIV (PWH) in Kampala after switching their antiretroviral therapy (ART) anchor drug from nevirapine, efavirenz, or a protease inhibitor to dolutegravir. DESIGN: This was a retrospective cohort analysis. METHODS: Data were extracted from electronic medical records of nonpregnant PWH who were virally suppressed on dolutegravir-based therapy for at least 12 months following switch. We studied the rate of weight gain using a piecewise spline random effects model and used Cox-proportional hazard models to describe risk factors for weight

Population Pharmacokinetic Modelling of Dolutegravir: A Narrative Review.

Dolutegravir-based regimens are part of the preferred regimens in all populations living with HIV, but dolutegravir's pharmacokinetics can vary greatly with factors such as drug-drug interactions (DDIs) and age. For example, DDIs between dolutegravir and rifamycins in countries with a high burden of HIV/tuberculosis co-infection present a major challenge. Attaining therapeutic concentrations is crucial for virologic success and to prevent resistance. Understanding dolutegravir pharmacokinetics and its sources of variability is essential to ensure adequate exposure in all individuals. Populatio

A lesson in embracing uncertainty in early safety signals from the Tsepamo study.

OBJECTIVE: We investigated how randomness may have contributed to the apparent decline in observed risk of neural tube defects (NTDs) following in-utero dolutegravir (DTG) exposure. We aimed to describe statistical approaches to uncertainty using accessible language for nonstatistical audiences. METHODS: We reanalyzed Tsepamo Study data using frequentist confidence intervals, repeated intervals accounting for group sequential monitoring, and Bayesian posterior and posterior predictive distributions. We estimated the probability of decision reversal using simulation. RESULTS: The initial Tsep

Bone Health in People Living with HIV: From Pathophysiology to Practical Management.

As the life expectancy of people living with HIV (PLWH) continues to rise with the success of antiretroviral therapy (ART), long-term complications such as reduced bone mineral density (BMD) and osteoporosis have become increasingly prevalent. PLWH exhibit a higher risk of bone loss and fractures compared to the general population, driven by multiple factors including chronic immune activation, systemic inflammation, and ART-related toxicity. Tenofovir disoproxil fumarate is known to induce proximal renal tubular dysfunction and hypophosphatemia, contributing to reduced BMD. Tenofovir alafenam

Analysis of mutational history of multidrug-resistant genotypes with a mutagenetic tree model.

Human immunodeficiency virus (HIV) can develop resistance to all antiretroviral drugs. Multidrug resistance, however, is a rare event in modern HIV treatment, but can be life-threatening, particular in patients with very long therapy histories and in areas with limited access to novel drugs. To understand the evolution of multidrug resistance, we analyzed the EuResist database to uncover the accumulation of mutations over time. We hypothesize that the accumulation of resistance mutations is not acquired simultaneously and randomly across viral genotypes but rather tends to follow a predetermin

Dolutegravir, abacavir and lamivudine as HIV therapy.

INTRODUCTION: Antiretroviral therapy has evolved dramatically and more potent, safer and convenient drugs have replaced former compounds. Dolutegravir (DTG) is the most recently approved integrase inhibitor. It displays attractive properties such as one pill once daily (QD) dosing, high barrier to resistance and clean safety profile. Moreover, it is planned to be marketed, co-formulated with abacavir (ABC) and lamivudine (3TC) as a single-tablet regimen (STR). The availability of this QD single pill represents a significant step further for a large number of HIV-infected persons. AREAS COVERE

Polycythemia in males living with HIV post dolutegravir and long term antiretroviral therapy.

BackgroundDolutegravir based antiretroviral therapy (ART) is widely recommended as first line treatment for people living with HIV because of its potent viral suppression, high genetic barrier to resistance, and a favorable tolerability profile. Haematological adverse effects of ART are well recognized, most commonly anemia, particularly with zidovudine-containing regimens. However, erythrocytosis or polycythaemia is rarely described in association with dolutegravir, and available literature on this possible association remains limited. Case-presentationsWe describe a case series of four male

Dolutegravir combined anti-retroviral agents as second-line drugs and virologic suppression of adults with HIV in Federal Teaching Hospital, Ido Ekiti - case series.

The transition to dolutegravir (DTG)-combined anti-retroviral therapy (ART) as a preferred option in both first- and second-line for all populations was adopted by the World Health Organization (WHO) due to encouraging safety data for women and adolescent girls during the peri-conception period. This case series aimed at determining the viral suppression of adults with HIV on Dolutegravir combined anti-retroviral agents as second-line drugs. Four clients' viral suppression were observed from ART initiation to switching to second line and later transiting to the DTG-combined regimen and the fin

Sex and gender differences in weight gain associated with second-generation integrase strand transfer inhibitors: A scoping review.

INTRODUCTION: Weight gain associated with antiretroviral therapy, particularly second-generation integrase strand transfer inhibitors (INSTIs), is an increasingly recognized concern. However, literature remains mixed regarding whether these effects differ by sex/gender. This scoping review aims to map the pattern, timing, severity and risk factors for weight gain among cis and trans women initiating or switching to second-generation INSTIs, such as dolutegravir, bictegravir or cabotegravir. METHODS: Four databases (EMBASE, MEDLINE, Web of Science and CINAHL) were searched for studies reportin

Older Age is Associated with Higher Dolutegravir Exposure in Plasma and Cerebrospinal Fluid of People Living with HIV.

BACKGROUND: People living with human immunodeficiency virus are ageing under combination antiretroviral treatments but data on drug exposure in serum and cerebrospinal fluid are limited. Dolutegravir is a widely used second-generation integrase strand transfer inhibitor: conflicting data suggest that neuropsychiatric side effects may present at a higher frequency in patients with higher dolutegravir serum concentrations. METHODS: We performed a retrospective analysis of our therapeutic drug monitoring registry identifying patients receiving once-daily dolutegravir without concomitant interact

Clinical pharmacology of the single tablet regimen bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF).

The fourth HIV strand-transfer integrase inhibitor (INSTI) has been released into the market as part of a single-tablet-regimen (STR) consisting of bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF). The newest component is thus BIC, a booster-free INSTI with pharmacological characteristics similar to those of dolutegravir (DTG), including high intrinsic antiretroviral potency. The BIC-containing STR underwent clinical development in both treatment-naive and virologically suppressed patients and was found non-inferior to DTG-based comparator arms. In the currently evolving therapeut

Hemeoxygenase-1 as a Novel Driver in Ritonavir-Induced Insulin Resistance in HIV-1-Infected Patients.

BACKGROUND: Hemeoxygenase-1 (HO-1) has recently been identified as a major driver of metaflammation and obesity-related insulin resistance (IR). Drug-induced IR increases cardiovascular risk within the HIV-1-infected population receiving antiretroviral therapy (ART). We therefore investigated a possible role of HO-1 in ART-induced IR. METHODS: Effects of HIV-1 protease inhibitor ritonavir and integrase inhibitor raltegravir (RAL) on expression levels of HO-1 and proinflammatory cytokines, including interleukin 1β (IL-1β), IL-6, IL-8, tumor necrosis factor-α (TNFα), chemokine (C-C motif) ligan

Differential adipose tissue remodeling and metabolic effects of dolutegravir and bictegravir: implications for HIV therapy.

Combined antiretroviral therapy (cART) has significantly improved the prognosis of people with HIV (PWH), but long-term metabolic side effects-particularly weight gain and adipose tissue dysfunction-remain an important concern. Among the components of cART, integrase strand transfer inhibitors (INSTI), especially dolutegravir (DTG) and bictegravir (BIC), have been related to adipose tissue remodeling and weight change. This study explores the direct effects of DTG and BIC on adipogenesis and adipose tissue function using 3T3-L1 preadipocyte cultures and mouse models. In vitro, DTG impaired adi
