Psychedelic drugs for psychiatric disorders.

Existing pharmacological treatments for psychiatric disorders have demonstrated limited efficacy, delayed onset of action, and significant burden of side effects. Recent findings from human studies with psychedelics have shown promise, demonstrating rapid and sustained clinical benefits of these compounds for a variety of psychiatric disorders. Classical psychedelics have a rich history and some of these compounds have been used in shamanic and spiritual ceremonies for millennia. The psychoactive effects of these drugs, particularly on human consciousness, have generated great scientific curio

Toward Mapping Neurobehavioral Heterogeneity of Psychedelic Neurobiology in Humans.

Precision psychiatry aims to identify markers of interindividual variability that allow for predicting the right treatment for each patient. However, bridging the gap between molecular-level manipulations and neural systems-level functional alterations remains an unsolved problem in psychiatry. After decades of low success rates in pharmaceutical research and development for psychiatric drugs, multiple studies now point to the potential of psychedelics as a promising, fast-acting, and long-lasting treatment for some psychiatric symptoms. Yet, given the highly psychoactive nature of these subst

Psychedelic medicines for mood disorders: current evidence and clinical considerations.

PURPOSE OF REVIEW: Despite advances in treatment modalities for mood disorders over recent decades, further therapeutic options are still required. Increased research is occurring, with the pursuit of psychedelic-based pharmacotherapies for a range of mood disorders and other conditions. RECENT FINDINGS: Serotonergic psychedelics have been found to modulate brain networks underlying various psychiatric disorders, as well promoting neurogenesis and neuroplasticity. Randomized placebo-controlled trials have found psilocybin with psychological support effective at treating depression, including

Assessing the risk-benefit profile of classical psychedelics: a clinical review of second-wave psychedelic research.

RATIONALE: A broad reassessment of the potential benefits of psychedelic drugs has led to the initiation of multiple major clinical trials in an effort to advance their status to become FDA-approved medications, as well as local legislative efforts to legalize or decriminalize their use. OBJECTIVES: To use recently published data to assess potential risks and benefits of psychedelic drugs as therapeutics, as well as to synthesize what is currently known in order to generate fruitful future research directions. METHODS: A review of studies conducted since 1991 identified 14 clinical trials of

Delta-8 THC use in US adults: Sociodemographic characteristics and correlates.

BACKGROUND: Use of novel cannabis products whose primary active compound is Delta-8 tetrahydrocannabinol (Delta-8 THC), an isomer of Delta-9 THC has recently surged. While Delta-8 THC has psychoactive effects and is potentially harmful, little is known about its use. We examined sociodemographic characteristics, motivations, and consumption patterns of Delta-8 THC in US adult cannabis users. METHODS: Cannabis-using adult online survey participants (N = 4,348) provided information on Delta-8 use, and other characteristics. We assessed frequencies of sociodemographics, patterns, and correlates

Signaling snapshots of a serotonin receptor activated by the prototypical psychedelic LSD.

Serotonin (5-hydroxytryptamine [5-HT]) 5-HT2-family receptors represent essential targets for lysergic acid diethylamide (LSD) and all other psychedelic drugs. Although the primary psychedelic drug effects are mediated by the 5-HT2A serotonin receptor (HTR2A), the 5-HT2B serotonin receptor (HTR2B) has been used as a model receptor to study the activation mechanisms of psychedelic drugs due to its high expression and similarity to HTR2A. In this study, we determined the cryo-EM structures of LSD-bound HTR2B in the transducer-free, Gq-protein-coupled, and β-arrestin-1-coupled states. These struc

Psilocybin modulation of time-varying functional connectivity is associated with plasma psilocin and subjective effects.

BACKGROUND: Psilocin, the neuroactive metabolite of psilocybin, is a serotonergic psychedelic that induces an acute altered state of consciousness, evokes lasting changes in mood and personality in healthy individuals, and has potential as an antidepressant treatment. Examining the acute effects of psilocin on resting-state time-varying functional connectivity implicates network-level connectivity motifs that may underlie acute and lasting behavioral and clinical effects. AIM: Evaluate the association between resting-state time-varying functional connectivity (tvFC) characteristics and plasma

Perceptions about THC and CBD effects among adults with and without prior cannabis experience.

BACKGROUND: Cannabis is associated with a range of therapeutic and non-therapeutic, positive and negative effects. While some benefits and harms may be specific to individual cannabinoid constituents (THC, CBD), individual expectancies may also play a role. OBJECTIVES: Evaluate the extent to which individuals hold expectancies about the effects of CBD, THC, and THC & CBD combined, and whether this differs with prior cannabis experience. METHODS: Canadian adults (N = 345; n = 58 no prior cannabis use, n = 287 prior cannabis use) completed a Qualtrics survey. Participants provided information

Classical Psychedelics as Therapeutics in Psychiatry - Current Clinical Evidence and Potential Therapeutic Mechanisms in Substance Use and Mood Disorders.

Classical psychedelics, primarily psilocybin and lysergic acid diethylamide (LSD), have been used and extensively studied in Western medicine as part of substance-assisted psychotherapy in the 1950s and 1960s. Modern clinical research is currently gaining momentum and provides new evidence for the safety and efficacy of classical psychedelics (primarily psilocybin, but also LSD and ayahuasca) in the treatment of different psychiatric conditions, including substance use and mood disorders. In this review article, we outline common pathological mechanisms of substance use disorders (SUD) and unip

The epidemiology of mescaline use: Pattern of use, motivations for consumption, and perceived consequences, benefits, and acute and enduring subjective effects.

BACKGROUND: Mescaline is a naturally occurring psychoactive phenethylamine found in several cacti and historically used ceremonially by Indigenous and Latin American populations. Broader recognition of its possible therapeutic value in Western science began in the 1950s; however, knowledge of the safety profile of mescaline and the extent of its use remains limited. The primary aim of this study is to examine the epidemiology of mescaline use among English-speaking adults. METHODS: About 452 respondents completed a web-based survey designed to assess their previous experience with mescaline (

Psychotomimetic symptoms after a moderate dose of a synthetic cannabinoid (JWH-018): implications for psychosis.

BACKGROUND: Synthetic cannabinoids (SCs) are the largest class of novel psychoactive substances (NPS) and are associated with an increased risk of overdosing and adverse events such as psychosis. JWH-018 is one of the earliest SCs and still widely available in large parts of the world. Controlled studies to assess the safety and behavioural profiles of SCs are extremely scarce. AIM: The current study was designed to assess the psychotomimetic effects of a moderate dose of JWH-018. METHODS: Twenty-four healthy participants (10 males, 14 females) entered a placebo-controlled, double blind, wit

The Biochemistry of Phytocannabinoids and Metabolic Engineering of Their Production in Heterologous Systems.

The medicinal properties of cannabis and the its legal status in several countries and jurisdictions has spurred the massive growth of the cannabis economy around the globe. The value of cannabis stems from its euphoric activity offered by the unique phytocannabinoid tetrahydrocannabinol (THC). However, this is rapidly expanding beyond THC owing to other non-psychoactive phytocannabinoids with new bioactivities that will contribute to their development into clinically useful drugs. The discovery of the biosynthesis of major phytocannabinoids has allowed the exploration of their heterologous pr

Why did New Zealand's referendum to legalise recreational cannabis fail?

New Zealand recently held the world's first national referendum on cannabis legalisation involving a detailed bill to regulate retail sale and supply of cannabis for recreational (non-medical) adult use (i.e. the Cannabis Legalisation and Control Bill; CLCB). The referendum generated significant domestic debate and lobbying from both sides of the issue. The CLCB was narrowly defeated, with 48.4% voting to support versus 50.7% opposed. In this commentary, we discuss the referendum campaigns and other political and social factors that may have contributed to the result. Voting patterns appeared

A comparative analysis of laws on recreational cannabis edibles between Canada and the United States of America.

BACKGROUND: The Canadian government has amended the Cannabis Act and Cannabis Regulations to legalize use of recreational cannabis edibles. Besides Canada, the United States of America (U.S.A.) is the only other country with experience regulating cannabis edibles, albeit at the state-level. Because of the potential health risks associated with consuming THC, we sought to compare laws (within Canada and the U.S. states that have legalized the recreational use of cannabis edible) that govern THC-infused cannabis edibles; and to identify any associated gaps in Canada's cannabis regulatory framewo

Is cannabis use associated with prescription psychotropic and pain reliever medication and other substance use among individuals aged 50+ with mental illness?

BACKGROUND: Despite increasing rates of nonmedical and/or medical cannabis use in the 50+ age group, scant research exists on the associations between cannabis use and prescription medication use. In this study, we examined associations of use of prescription tranquilizers, sedatives, stimulants, and pain relievers, tobacco products, any/binge/heavy alcohol, and illicit drugs with cannabis use and use characteristics among U.S. adults aged 50+ years with past-year mental illness (n = 6454). METHODS: Data are from the 2015-2019 National Survey on Drug Use and Health (NSDUH). We used logistic r

[Legal access to cannabis and pilot projects in Switzerland].

On March 31, 2021, the Swiss Federal Council accepted the Ordinance on Pilot Trials under the Narcotics Law, which will come into force on May 15, 2021 and will authorize pilot trials of recreational cannabis use by adults. The purpose of these trials will be to provide a scientific basis for future policy decisions on cannabis regulation. This ordinance is based on the idea that the legislator must give himself the possibility to experiment, as it was already the case in the 1990s with the pilot projects of heroin prescription, which proved to be useful in the legislative process, leading to

[The development of a rational national MDMA policy and its relevance for psychiatry].

BACKGROUND: MDMA (ecstasy) is a relatively safe drug and induces little dependence, but is nevertheless scheduled as a hard drug (Dutch Opium Act, List 1). Concerns about MDMA-related crime, health incidents and possible inappropriate listing of MDMA on List I have led to an ongoing debate about current Dutch ecstasy policy. AIM: To develop a rational MDMA policy that takes into account all aspects related to production, sale and use of MDMA. METHOD: An interdisciplinary group of 18 experts formulates a science-based MDMA policy by assessing the expected effects of 95 policy options on 25 ou
