Distinct Cell-Intrinsic Functional Differences Between Iliac Crest and Distal Radius Autografts.

PURPOSE: Autologous bone grafts demonstrate osteoconductive, osteoinductive, and osteogenic properties. Hand surgeons commonly augment surgical fixation with autografts to promote fracture healing. This study compared the intrinsic stem cell-like properties of 2 commonly used autograft sources in hand surgery: the iliac crest and distal radius. METHODS: A total of 9 subjects who received an iliac crest bone graft and distal radius bone graft harvest as a part of the standard care of distal radius malunion or nonunion correction or scaphoid nonunion open reduction and internal fixation were en

Monoaxial distraction of ulna to second metacarpal followed by single bone forearm in massive post infective radial bone loss.

INTRODUCTION: Radial bone loss associated with gross manus valgus deformity can be managed by open reduction internal fixation using intervening strut bone graft, callus distraction using ring or monoaxial fixator, and achieving union by distraction histogenesis. These methods are particularly suitable when bone loss is small. Single or staged procedure is described for congenital as well as in acquired extensive bone loss of radius. Distraction through radial proximal to distal segments, to achieve reduction of distal radio-ulnar joint (DRUJ), is also described in acquired cases. In the prese

Neonate auditory brainstem response repeatability with controlled force gauge bone-conducted stimulus delivery.

OBJECTIVE: The feasibility and repeatability of neonate auditory brainstem responses (ABRs) with a controlled hand-held applied force gauge for bone-conducted stimulus delivery was examined. DESIGN: A repeated measures test-retest design was employed. STUDY SAMPLE: Participants were 27 healthy neonates. A 4000 Hz bone-conducted CE-Chirp octave band stimulus evoked the ABRs. Intra- and intertester conditions were employed with a prototype hand-held applied force gauge (Etymōtic Research) attached to the superior aspect of the bone vibrator. The bone vibrator was placed in a superoposterior au

Mirror Hand Disorder's Surgical Management with Metacarpal Wedging.

Ulnar dimelia, also known as "mirror hand disorder", is a rare developmental disorder affecting the upper limb. Primarily, it involves the duplication of fingers, carpal bones, metacarpals, or ulna along the sagittal axis, and is often accompanied by the absence of the radius or thumb. The anomaly presents challenges in both bone and soft tissue development, impacting limb functionality and affecting a child's quality of life. We present the case of a one-year-old girl with unilateral ulnar dimelia. Surgical intervention was considered to address functional and aesthetic concerns. The surgery

Early total reconstruction for a Gustilo type IIIB open forearm fracture associated with avulsion injury of multiple extensor tendons: A case report.

Gustilo type IIIB open forearm fractures associated with avulsion injuries of multiple extensor tendons are difficult to reconstruct. Not only are bones, nerves, blood vessels, and soft tissues injured, but also tendons directly related to hand function. A 74-year-old man sustained an injury to his dominant right hand after being hit by a heavy pulley. The patient was diagnosed with a Gustilo type IIIB open forearm fracture, and multiple extensor tendons were avulsed from the musculotendinous junction. On the day of injury, the radius was fixed using a volar locking plate, and the ulnar head w

ACR Appropriateness Criteria® Chronic Hand and Wrist Pain: 2023 Update.

Chronic hand and wrist pain is a common presenting complaint. The intricate anatomy results in a variety of pain generators-multiple bones, articular cartilage, intrinsic ligaments, triangular fibrocartilage complex, joint capsules and synovium, tendons and tendon sheaths, muscles, and nerves-in a compact space. The need for imaging and the choice of the appropriate imaging modality are best determined by the patient's presentation, physical examination, and the clinician's working differential diagnosis. Radiography is usually appropriate as the initial imaging study in the evaluation of chro

Trapezoid Fracture Associated with Scaphoid Fracture in a Football Goalkeeper.

INTRODUCTION: Trapezoid fractures are uncommon in sports. We presented a rare case of a trapezoid fracture associated with a scaphoid fracture caused by punching a ball in a football goalkeeper. CASE PRESENTATION: A 19-year-old male who played as a football goalkeeper visited our hospital with complaints of sustained pain from the right wrist to the hand after punching a ball. Scaphoid fracture was diagnosed on plain radiographs, whereas trapezoid fracture was overlooked. Computed tomography revealed a displaced trapezoid fracture associated with a scaphoid fracture. Both fractures were succe

Chondrodysplasia with multiple dislocations: comprehensive study of a series of 30 cases.

The group of chondrodysplasia with multiple dislocations includes several entities, characterized by short stature, dislocation of large joints, hand and/or vertebral anomalies. Other features, such as epiphyseal or metaphyseal changes, cleft palate, intellectual disability are also often part of the phenotype. In addition, several conditions with overlapping features are related to this group and broaden the spectrum. The majority of these disorders have been linked to pathogenic variants in genes encoding proteins implicated in the synthesis or sulfation of proteoglycans (PG). In a series of

TRANSPLANTATION OF VASCULARIZED COMPOSITE ALLOGRAFTS. REVIEW OF CURRENT KNOWLEDGE.

Transplantation in reconstructive surgery has been performed for more than 20 years, although the official beginning of these procedures is considered the first successful transplantation of the hand in 1998. The original name "Composite Tissue Transplantation" has been used less frequently; more common is the term "Vascularized Composite Allotransplant", which better describes the basis of the procedure. There have been so far approximately 180 transplantations performed, the exact number is difficult to find. The most known transplantation from this group include transplantation of the hand

You've Got to Hand It to Them: Assessing Final Year Medical Students Knowledge of Hand Anatomy and Pathology.

BACKGROUND: Knowledge of hand anatomy and pathology is important for final year medical students as it frequently appears in examinations as a short case or in a written paper. Studies have shown that doctors in the Emergency Department have a deficient grasp of musculoskeletal anatomy secondary to a lack of dedicated teaching in medical school. METHODS: A questionnaire was handed out to 111 final year medical students. Students were asked to fill it out before and after dedicated upper limb teaching sessions during their rotation. The questionnaire consisted of an anatomical and pathological

[Osteocytes and the pathways of mechanical homeostasis optimization from the point of view of functional osteology].

The aim of this work was to determine, on the basis of the results of authors' own research and literature data, the main pathways of osteocyte (OC) influence on the mechanical homeostasis of the skeleton. The following pathways of reorganization of the architecture of bone structures are postulated: at the ultrastructural level without direct cell participation, through the bone matrix synthesis by osteoblasts and OC, through bone matrix resorption by osteoclasts and OC, the latter being able to resorb the surrounding mineral and organic matrix both separately, and conjointly. This reorganiza

Systematic Evaluation of IMU Sensors for Application in Smart Glove System for Remote Monitoring of Hand Differences.

Human hands have over 20 degrees of freedom, enabled by a complex system of bones, muscles, and joints. Hand differences can significantly impair dexterity and independence in daily activities. Accurate assessment of hand function, particularly digit movement, is vital for effective intervention and rehabilitation. However, current clinical methods rely on subjective observations and limited tests. Smart gloves with inertial measurement unit (IMU) sensors have emerged as tools for capturing digit movements, yet their sensor accuracy remains underexplored. This study developed and validated an

Multifunctional polydopamine modified nano-hydroxyapatite doped sodium alginate/gelatin dual network hydrogel for bone tissue engineering scaffolds.

Hydrogels have the structure and composition of extracellular matrix, but single component hydrogels are often difficult to meet the needs of multi-function. Therefore, in view of the common requirements of anti-infection and promoting bone regeneration in bone tissue, this paper, based on GelMA modified by gelatin, added sodium alginate to blend, and constructed a double network hydrogel of photocrosslinking/ion crosslinking. At the same time, nano hydroxyapatite modified by polydopamine is added to enhance the mechanical properties of the hydrogel on the one hand, and improve the function of

Genetic analysis of a congenital split‑hand/split‑foot malformation 4 pedigree.

In the present study whole-exome sequencing using the Complete Genomics platform was employed to scan a proband from a split‑hand/split‑foot malformation (SHFM) 4 family. The missense mutation c.728G>A (p.Arg243Gln) in the TP63 gene was revealed to be associated with SHFM. Sanger sequencing confirmed the sequences of the proband and his father. The father was diagnosed with SHFM and harbored a CGG‑to‑CAG mutation in exon 5, which produced a R243Q substitution in the zinc binding site and dimerization site of TP63. The R243Q mutation was predicted to be pathogenic by PolyPhen‑2. The proband, wh

Long-term Follow-up Study for Fractured and Non-Fractured Hand Enchondromas Treated by Sole Curettage.

Background  Enchondromas are the most common primary tumors in the small tubular bones of the hand, and fractures are often the result of thinned cortical bone. The main question was whether fractured enchondromas influence long-term clinical and radiological outcomes. Methods  Between 2000 and 2019, 57 patients with previously treated fractured (group I) and non-fractured (group II) hand enchondromas (34 female, 23 male; mean age 39.4 ± 13.7 years) were evaluated for clinical and radiological treatment outcomes. Occult renal cell carcinoma with acrometastasis and ipsilateral juxta-articular knee lesions mimicking acute inflammation.

Generally, skeletal peripheral metastases below the elbow and the knee are rare. Skeletal metastases to the hand or foot are very rare; but when they do it may be a revealing clinical finding. Purely lytic lesions are commonly seen in metastases from lung, renal, and thyroid tumors, but they are also known to occur in primary myeloma, brown tumor and lymphomas. A 70-year-old man was brought to the emergency department with acute painful swelling involving his right hand and the right knee. Due to significant accompanying soft tissue swellings cellulitis, acute osteomyelitis and gouty arthropat

Clinical evaluation of irreversible data compression for computed radiography of the hand.

We evaluated the clinical utility of high data compression for computed radiography (CR) of the hand. Paired original and compressed CR images were obtained using an FCR 9000 system in 35 patients with various abnormalities of the hand. For the paired original and compressed images, we compared the depiction of the bone cortex, bone trabeculae, soft-tissue, and the margin of the abnormalities. The overall impressions for detectability of the abnormalities and for diagnostic capability were also evaluated. Poorer image quality of the compressed images was noted for most of the structures, but t
