[Reversed musculus biceps femoris flap for covering a defect of the distal thigh].

We report the case of a 30-year-old female patient, who had suffered a grade III open femur fracture in a motor vehicle accident 14 weeks prior to being transferred to the trauma department of the University Hospital in Bonn. Upon admission to our unit, posttraumatic osteitis, an unstable fracture following compression plating, and a soft tissue defect of the anterolateral distal thigh were discovered. Following removal of the hardware and stabilization of the fracture with external fixation, the infection was brought under control. Because the patient refused the time-consuming segmental tran

Double-blind randomized prospective study of the efficacy of antibiotic prophylaxis for open reduction and internal fixation of closed ankle fractures.

Antibioprophylaxis has been proven to be efficient for some orthopaedic procedures. However, its efficacy for clean limited procedures with … as hospitalized and treated with i.v. antibiotics. However, there was no osteomyelitis or premature hardware removal. The difference between the two groups was not statistically significant (chi 2 test of appreciation p = 0.33, two-tailed probabilities). Therefore, cephalothin prophylaxis does not seem justified in this patient population. A larger series is neede …

Ankle fractures involving the fibula proximal to the distal tibiofibular syndesmosis.

Thirty-two cases of ankle fractures associated with fibular fractures above the distal tibiofibular syndesmosis were studied. All were treat … r nonunions, one deep infection, and one recurrent superficial infection and wound dehiscence after hardware removal. A syndesmotic screw is usually needed in cases of fracture-dislocations. Two patients with occult fibular nonunions developed diastasis of the syndesmosis after removal of the syndesmotic screw. It was found that reduction and temporary pinning of …

Therapeutic Measures for Infections Originating from Scalp Incisions Following Deep Brain Stimulation in Patients with Parkinson's Disease.

BACKGROUND: Deep brain stimulation (DBS) is a well-established treatment for Parkinson's disease (PD). However, infection following DBS surgery is a serious complication that can lead to the recurrence and worsening of Parkinson's symptoms or related hardware reimplantation, causing considerable patient suffering and financial burden. OBJECTIVE: This study aims to compare the therapeutic efficiency of different treatment approaches for scalp incision infections after DBS surgery in PD patients. METHODS: We conducted a retrospective review of patients with PD who experienced scalp infections

Impact of preoperative osteoporosis on short- and long-term complications following ankle ORIF: A multi-institutional propensity-matched analysis.

BACKGROUND: Osteoporosis may impair fracture healing and fixation durability after ankle ORIF. This study evaluated postoperative complicati … and transfusion (1.9% vs. 1.1%; RR=1.63). At 2 years, device infection (2.6% vs. 1.4%; RR=1.80) and hardware removal (14.2% vs. 11.6%; RR=1.23) were elevated. CONCLUSIONS: Osteoporosis is associated with increased short- and long-term complications after ankle ORIF; because these injuries are treated acutely, bone health optimization and fixation suited to osteop …

Pseudo-Pulseless Electrical Activity With Anaphylaxis in a Pediatric Surgical Patient: A Case Report.

Pseudo-pulseless electrical activity (p-PEA) is a clinical condition where cardiac output drops severely enough to produce a non-palpable pulse in the setting of organized electrical activity of the heart, but imaging or monitors display evidence of cardiac motion. It may be seen in shock states such as anaphylactic shock. We describe a case where a previously healthy teenage patient presenting for operative removal of orthopedic hardware developed p-PEA after anaphylaxis to antibiotics. Despite loss of pulses and severe mottling of the skin, the patient's pulse oximetry (plethysmography) and

Walking the Tight Rope for a Diagnosis: A Case Report of a Dermatological Enigma of Surgical Device Nickel Allergy.

Metal allergies pose challenges in patients with implantable devices, yet no standardized protocols exist for preoperative metal patch testing. This case details a 53-year-old woman with systemic dermatitis and impaired hand function after left thumb trapeziectomy with Arthrex Mini TightRope placement. Despite treatment, including hardware removal, symptoms including rash, pruritus, and impaired mobility persisted for months. Metal patch testing confirmed hypersensitivity to nickel, chromium, and other metals, implicating type IV hypersensitivity. Nickel is the most common allergen, affecting …

Infective endocarditis presenting as ANCA-associated glomerulonephritis: a complex diagnostic challenge.

A 59-year-old woman with a history of permanent pacemaker placement, prior stroke, and deep vein thrombosis presented with persistent purpuric lesions, lower extremity edema, and acute kidney injury. Laboratory evaluation revealed hemolytic anemia, thrombocytopenia, and myeloperoxidase-antineutrophil cytoplasmic antibody (ANCA) positivity, initially suggesting ANCA-associated vasculitis. Although she received high-dose corticosteroids and a single intravenous dose of cyclophosphamide, the subsequent development of fever and splenic infarction led to a workup revealing Enterococcus faecalis bac

Novel Bone and Marrow Harvesting Approach Through Posterior Subtalar Screw Tunnel for Revision Talonavicular Joint Arthrodesis.

We present a case report of revision talonavicular joint arthrodesis following painful nonunion. Following the removal of the previously united subtalar joint arthrodesis hardware, the posterior screw tunnel was used to access the internal aspect of the calcaneus for bone and marrow harvesting avoiding a second graft site incision. A suction curettage system was then used to acquire adequate autologous cancellous bone graft and nondiluted bone marrow. The talonavicular joint was then prepared for revision arthrodesis, followed by augmentation with the autologous cancellous bone graft and rigid

Dorsal Translocation of the Flexor Pollicis Longus Tendon Following Pediatric Both Bone Forearm Fracture.

Tendon entrapment is a rare complication of closed forearm fractures. A 16-year-old boy sustained a type 1 open both bone forearm fracture after falling from a skateboard. The injury was initially managed with irrigation, debridement, and flexible intramedullary nailing. Seven weeks after surgery, a flexion contracture of the ipsilateral thumb interphalangeal joint was noted. Subsequent hardware removal and hand therapy failed to improve thumb extension. The patient was taken to the operating room for planned tenolysis and possible tendon reconstruction. Intraoperatively, the flexor pollicis l…

Retained Foreign Body in the Nasal Cavity After Oral Maxillofacial Surgery.

Retained foreign bodies sometimes occur in various surgical procedures and can lead to severe complications. Foreign bodies in the oral and maxillofacial region are not rare because of the use of many small items and the natural communication with the outside environment in some areas. We experienced a case of foreign body in the nasal cavity, which was discovered 1 year later at a second operation for hardware removal after maxillofacial surgery. A small, soft material is usually placed between the nasal endotracheal tube and nostril to avoid nasal pressure ulcer at the ala of nose after prol…

Open wound management of esophagocutaneous fistula in unstable cervical spine after corpectomy and multilevel laminectomy: A case report and review of the literature.

A 67-year-old female patient developed an esophagocutaneous fistula 4 mo after C4 and C5 partial corpectomy. Plain radiograph and computed tomography (CT) scan of cervical spine showed inferior screws pullout with plate migration that caused the esophageal perforation. Management included removal of anterior hardware, revision C4-5 corpectomy, iliac crest strut autograft and halo orthosis immobilization. The fistula was treated using antibiotics and a 10-french gauge rubber tube for daily irrigation and Penrose drain. At 3 mo, the esophagocutaneous fistula healed and the patient resumed oral f

Prefabricated supraclavicular flap as a salvage procedure for a failed microvascular hypopharyngeal defect closure in a tetraplegic patient.

Delayed hypopharyngeal perforations in tetraplegics are a rare but potentially life-threatening complication of anterior cervical spine instrumentation. To date, no established treatment regimen exists for these situations. A patient with traumatic tetraplegia sub-C4 was admitted to the hospital 14 days after anterior spinal fusion with an infected hypopharyngeal perforation. After hardware removal, the spine was restabilized with a composite free osteomusculocutaneous fibula with the flexor hallucis longus muscle closing the mucosal defect. However, it was lost because of external venous comp…
