Direct gambling marketing, direct harm: A randomised experiment.

BACKGROUND AND AIMS: Whether gambling marketing has a causal effect on harm is of regulatory interest. Direct marketing offers (emails, push notifications and text messages) are frequently received by people with active gambling accounts, but they can opt out. This study aimed to test whether opting out of direct marketing reduces betting and short-term gambling harms in a real-world gambling environment. DESIGN: Stratified randomised field experiment with a between-participants design, embedded in a 14-day ecological momentary assessment (EMA). SETTING: Nationwide, Australia (July-August 20

Care in an opioid substitution therapy daycentre.

Care is getting increasing attention as important for harm reduction for people who use drugs (PWUD). As a contribution to the literature about the role that care can play in harm reduction, this article presents a study of the care provided at a daycentre for PWUD. Based on semi-structured interviews with PWUD and staff at the daycentre and field notes from researchers, the article shows how care is enacted through the social interaction between staff and users and how the daycentre is constituted as an enabling place. However, the article also shows how care involves power. Analytical tools …

The gender factor in meeting the needs of women who use drugs in Senegal.

Harm reduction (HR) services are developing in Africa. In Senegal, the overall HR service coverage ratio is one WUD for every 10 men. By analysing gender-specific initiatives developed by HR stakeholders and evaluating their limitations, we explored HR in Senegal through a gender lens to better understand how to meet the specific needs of WWUD. The data for this study were collected through interviews, observations, and the researchers' direct presence in drug use settings, as part of a social anthropology research project on the trajectories of WWUD in Senegal. The study reveals that in Seneg…

Radical hospitality: Innovative programming to build community and meet the needs of people who use drugs at a government-sanctioned overdose prevention site in San Francisco, California.

BACKGROUND: The Tenderloin Center (TLC), a multi-service center where people could receive or be connected to basic needs, behavioral health care, housing, and medical services, was open in San Francisco for 46 weeks in 2022. Within a week of operation, services expanded to include an overdose prevention site (OPS), also known as safe consumption site. OPSs have operated internationally for over three decades, but government-sanctioned OPSs have only recently been implemented in the United States. We used ethnographic methods to understand the ways in which a sanctioned OPS, situated in a mult

Opioid-related mortality in rural America: Geographic heterogeneity and intervention strategies.

Over the last two decades, opioid-related mortality rates have increased dramatically to become a serious public health concern in the United States. Opioid-related mortality has reached epidemic levels in certain rural areas of the U.S., such as Appalachia, New England, and the Mountain West, while remaining relatively low in others, such as the Delta South and Great Plains. Explanations for geographic variation in opioid mortality are unclear, contributing to ineffective policies and interventions. The goal of this article is to summarize the existing literature on the opioid epidemic in the

Could cannabis liberalisation lead to wider changes in drug policies and outcomes?

Cannabis policies are changing in some countries. This may have consequences that extend beyond cannabis-specific outcomes, such as an impac … icy perspective has inhibited investment in evidence based responses in areas such as treatment and harm reduction. The burden of disease associated with opioid use for example is large and this is an area in which treatment and harm reduction have been shown to deliver benefits. Thus if the changes in cannabis policies result in a greater willingness to invest …

"This place has given me a reason to care": Understanding 'managed alcohol programs' as enabling places in Canada.

For several decades, the emphasis on abstinence within homeless support systems has presented significant barriers to care for those who continue to use alcohol or drugs further marginalizing them in terms of housing and health/social services. In response, health care specialists and policymakers have recommended the integration of harm reduction philosophies and interventions into system-level responses to end homelessness. Managed alcohol programs (MAPs) have been developed to this end and have demonstrated positive results. While recent studies of MAPs have focused attention on reductions …

A qualitative exploration of the relevance of training provision in planning for implementation of managed alcohol programs within a third sector setting.

BACKGROUND: Managed Alcohol Programs (MAPs) are a harm reduction strategy for people experiencing homelessness and alcohol dependence. Despite a growing evidence base, resistance to MAPs is apparent due to limited knowledge of alcohol harm reduction and the cultural preference for abstinence-based approaches. To address this, service managers working in a not-for-profit organization in Scotland designed and delivered a program of alcohol-specific staff training as part of a larger study exploring the potential implementation of MAPs during the COVID-19 pandemic. METHODS: Semi-structured inter…

"There is a Place": impacts of managed alcohol programs for people experiencing severe alcohol dependence and homelessness.

BACKGROUND: The twin problems of severe alcohol dependence and homelessness are associated with precarious living and multiple acute, social and chronic harms. While much attention has been focused on harm reduction services for illicit drug use, there has been less attention to harm reduction for this group. Managed alcohol programs (MAPs) are harm reduction interventions that aim to reduce the harms of severe alcohol use, poverty and homelessness. MAPs typically provide accommodation, health and social supports alongside regularly administered sources of beverage alcohol to stabilize drinkin…

Consuming 'God Juice': Using 'ethnopharmacological-connoisseurship' to situate trenbolone use and knowledge among image and performance enhancing drug communities.

INTRODUCTION: Trenbolone, a potent anabolic-androgenic steroid (AAS), is used for its muscle growth benefits but poses significant health risks, including psychosocial issues. Existing research among humans is limited by lack of targeted investigation. This study addresses gaps in existing research by leveraging the ethnopharmacological expertise of trenbolone consumers. By engaging with the community's connoisseurship, we sought to enhance harm reduction strategies and foster collaboration between consumers, scholars and health practitioners. METHODS: An international sample of trenbolone co…

Implementation of minimum unit pricing for alcohol in Scotland and subsequent rates of hospital-admitted self-harm: controlled interrupted time series analysis.

BACKGROUND: Harmful alcohol use is a major modifiable risk factor for self-harm. We aimed to examine associations between minimum unit pricing for alcohol, introduced in Scotland in 2018, and rates of hospital-admitted self-harm. METHODS: Data were obtained from Public Health Scotland and English Hospital Episode Statistics. Interrupted time series (ITS) analysis compared pre- and post-intervention self-harm admission rates per 100 000 population, including controlled comparisons with England. RESULTS: The before-after slope reduction in Scotland corresponded to -5.6 (95% CI -7.5 to -3.7) ad
