Spontaneous Synchronous Tubal Pregnancy with Prior Successful in Vitro Fertilization and Embryo Transfer. A Case Report.

BACKGROUND: Synchronous (bilateral) ectopic preg- nancy is a very rare gynecological entity resulting in most cases from assisted reproduction techniques. Al- though few cases of bilater- al ectopic pregnancy have been reported in Nigeria, spontaneous bilateral tubal pregnancy in a woman with a diagnosis of bilateral tubal blockage and prior success- ful in vitro fertilization and embryo transfer (IVF-E T) is paradoxically rare and, to the best of our knowledge, has not been reported in Nigeria. CASE: A 37-year-old Nigerian woman presented with ruptured ectopic pregnancy associated with hemop

Atraumatic splenic rupture: an atypical presentation in a previously healthy patient.

UNLABELLED: Atraumatic splenic rupture represents a life-threatening abdominal event that requires immediate diagnosis and prompt surgical treatment to ensure the survival of the patient. Atraumatic rupture is relatively uncommon and can occur either in pathological spleens or, more rarely, in normal ones. It has a high morbidity, frequently with few and non specific signs suggesting its presence, can be associated to other pathologies incidentally discovered by imaging. We present a case, successfully treated, of a 51-year-old man, previously healthy, that referred to our hospital for arteria

Is laparoscopic surgery safe in patients with an elevated shock index due to ruptured ectopic pregnancy?

PURPOSE: To evaluate the effectiveness of laparoscopic surgery in patients with elevated shock index (SI), which is a unique determinant of acute hemorrhage. MATERIALS AND METHODS: A retrospective chart review of all patients treated for ectopic pregnancy (EP) in the present gynaecology department between January 2007 and March 2011 was performed. For each measurement of heart rate (HR) and systolic blood pressure (SBP), a SI was calculated by dividing HR by SBP (normal, 0.5 - 0.7). RESULTS: One hundred sixty patients were selected as SI above 0.7. There were 111 (69.4%) patients in the lapa

Laparoscopic management of a 16-week ruptured rudimentary horn pregnancy: a case and literature review.

INTRODUCTION: Pregnancy in a rudimentary horn is although rare, but is associated with the risk of rupture and life threatening hemorrhage. With the use of ultrasound the diagnosis can be made before symptoms occur. Management usually consists of excision of the rudimentary horn along with the pregnancy and the ipsilateral tube, traditionally by laparotomy. MATERIALS AND METHODS: We present a case of a 16-week ruptured rudimentary horn pregnancy diagnosed intraoperatively and managed laparoscopically in the presence of massive haemoperitoneum, which is first of its kind along with literature

[Maternal death after spontaneous rupture of a uterine artery immediately following delivery: a case report].

We report the case of a maternal death occurring after spontaneous rupture of a uterine artery immediately following delivery. The patient presented abdominal pain and a collapsus one hour after a normal delivery. Laparotomy revealed massive haemoperitoneum and intraperitoneal bleeding from the right uterine artery. Ligature of the uterine artery and hemostasis hysterectomy were performed but the patient died of multivisceral failure 18 h after the delivery. This is the first case report of maternal death occurring after spontaneous rupture of a uterine artery.

Intraperitoneal hemorrhage during and after percutaneous radiofrequency ablation of hepatic tumors: reasons and management.

BACKGROUND: Intraperitoneal hemorrhage is one of the most common complications of radiofrequency (RF) ablation of hepatic tumors. This study was designed to investigate the reason and management of intraperitoneal hemorrhage occurred during or after percutaneous RF ablation of hepatic tumors. METHODS: Three hundred and fifty-six patients with hepatic tumors have been treated at 592 procedures of ultrasound guided RF ablation. Intraperitoneal hemorrhage occurred in 5 patients (0.8%). The reasons and management of intraperitoneal hemorrhage in these 5 cases were retrospectively analyzed. RESUL

[Acute abdomen caused by rupture of aneurysms of the pancreatico-duodenal arteries. Apropos of a clinical case].

A case of acute abdominal hemorrhage caused by the rupture of several aneurysms of the pancreatoduodenal artery is presented. We discuss the difficulties of the diagnosis and point out the importance of the arteriographic study, as always in vascular lesions. We emphasize the difficulties of the treatment; most cases require surgical approach, going from suture of the aneurysm to cephalic duodenopancreatectomy.

Intermittent omental torsion--an unusual cause of recurrent abdominal pain?

A case of haemoperitoneum arising from an omental haematoma is described. Histology suggests that this resulted from an omental torsion. The omentum, however, was not torted at the time of laparotomy. Furthermore, there was a history of similar episodes of pain over the previous eighteen months. This suggests that spontaneous derotation of an omental torsion may occur, and may be an unusual cause of recurrent abdominal pain.

[The diagnosis of haemoperitoneum by "minimal" abdominal tap (author's transl)].

A simple diagnostic procedure in haemoperitoneum, using a single drop of fluid collected by abdominal tap and placed on a glass slide, is described. It is possible in this way to demonstrate the presence of minimal bloody effusions and to radically decrease the number of false negative results of paracentesis. Taps considered to be blood-free thus fall within the category of positive results. The actual procedure is improved since it is no longer necessary to collect a relatively large volume in order to ensure the diagnosis of haemoperitoneum. Furthermore, more complicated methods using cathe

Abdominal Pregnancy at Advanced Gestational Age in Lagos, Nigeria.

Abdominal ectopic pregnancy is an uncommon form of ectopic pregnancy with few cases reported in Nigeria. We present a case report of abdominal ectopic pregnancy with haemoperitoneum in an unbooked multipara at advanced gestational age. She subsequently had an emergency exploratory laparotomy and right salpingectomy. Diagnosis of this condition can be challenging due to non-specific clinical symptoms, while definitive management can also pose a challenge due to the site of placental attachment.
