A 68-year-old man with coronary artery disease was admitted for chest pain and ventricular tachycardia. After electric cardioversion, therapeutic heparinization was started for myocardial ischemia and nontransmural infarction. BACKGROUND: In the last years heparin surface-modified intraocular lenses (HSM IOLs) were used for special indications like uveitis, synechia, congenital cataract or pseudoexfoliation syndrome. A monolayer of heparin molecules on the surface of PMMA-IOLs causes a reduced postoperative inflammatory reaction and produces a higher long-term biocompatibility than PMMA-IOLs without surface-modification. We have studied the binding of 125I-heparin to human omental microvascular endothelial cells (HOME cells) and investigated its effect on cell proliferation. A case of severe vasospasm producing ischemia associated with the administration of dihydroergotamine and heparin for prophylaxis of deep venous thrombosis is presented. We feel that the intensity of the vasospasm was related to the ergot in combination with the use of pressors and the presence of an operative site contiguous to the spastic artery. Based on our experience and a review of the world's literature, we feel that risk of vasospasm in trauma patients outweighs the potential additional benefit of this combination of drugs over heparin prophylaxis alone.

We report on a 3 1/2 year old girl with purpura fulminans after varicella who made a complete recovery on treatment with an appropriate high-dose heparin regimen and other supporting measures. Several attempts to reduce the heparin dosage from the initial high level (85 IU/kg/hour) to the usual level (25 IU/kg/hour) were followed each time by an immediate deterioration in the clinical condition.
