A longitudinal analysis of factors associated with morbidity in cocaine abusers with psychiatric illness.

We conducted a study to characterize a population of cocaine users who were referred to a state psychiatric institution, identify treatment problems including reasons for relapse, and develop strategies to improve treatment outcome. Using a data base system from a tertiary-care hospital emergency department, we identified a sample of 80 patients with a cocaine-related presentation who came to the emergency department and were referred to the psychiatric facility. Forty-six percent had consumed crack cocaine, and 65% reported ingesting cocaine with other drugs, half of them with alcohol. Suicid

The so-called "inappropriate" psychiatric consultation request on a medical or surgical ward.

Seven cases of psychiatric consultations on medical and surgical wards are reviewed to show how intrapsychic conflicts in the staff may make the consultation request appear inappropriate. On deeper examination, such requests may signify staff dysfunction caused by arousal of conflictual feelings about the behavior of illness of the patient. Mutilated, mute patients appear to arouse fear of agression in their caregivers, who in turn reject such patients, see them as alien and violent, and become illogical in their management. Patients who publicly display sexual behaviors appear to arouse shame

Implementation of comfort rooms to reduce seclusion, restraint use, and acting-out behaviors.

The use of seclusion and restraint as methods to control acting-out behaviors by individuals with mental illness continues, despite deaths and other negative outcomes to both the clients themselves and the staff members applying these techniques. Additionally, client-to-client and client-to-staff assaults and self-injurious behaviors can lead to injury or possible fatalities to both parties. Thus, there is a need to find alternative approaches to mitigate or even eliminate some of these behaviors. The use of comfort rooms is one such approach. With the institution of comfort rooms, one small, r

Neonatal abstinence syndrome: transitioning methadone-treated infants from an inpatient to an outpatient setting.

OBJECTIVE: Each year in the US ∼50 000 neonates receive inpatient pharmacotherapy for the treatment of neonatal abstinence syndrome (NAS). The objective of this study is to compare the safety and efficacy of a traditional inpatient only approach with a combined inpatient and outpatient methadone treatment program. STUDY DESIGN: Retrospective review (2007 to 2009). Infants were born to mothers maintained on methadone in an antenatal substance abuse program. All infants received methadone for NAS treatment as inpatient. Methadone weaning for the traditional group (75 patients) was inpatient, wh

Predictors of burnout in professional and paraprofessional nurses working in hospitals and nursing homes.

Burnout is a phenomenon in which the cumulative effects of a stressful work environment gradually overwhelm the defenses of staff members, forcing them to psychologically withdraw. To understand the experience of professional and paraprofessional nurses suffering from burnout requires a close examination of the environments in which they function. This study examined interpersonal, intrapersonal and situational factors expected to contribute to the six dimensions of burnout among nursing staff who worked in acute care and long-term care health facilities. The sample included 312 professional a

Housing Insecurity, Behavioral Health Disorders, and Acute Care Utilization: A Cross-Sectional Study of Medicaid Members in the Portland, Oregon Metropolitan Area.

Housing insecurity and substance use disorder (SUD) are critical public health issues in the USA, with significant implications for health outcomes. This study evaluated the intersection of housing insecurity, SUD, and psychosis among Medicaid enrollees in an urban center in Oregon. Using data from Health Share of Oregon, we identified three distinct cohorts-individuals with housing insecurity, those with SUD/psychosis, and those with both conditions. Key outcomes included inpatient admission rates, average lengths of stay, and 30-day readmissions. The findings indicate that housing-insecure i

Residential care in hospital and in the community--quality of care and quality of life.

BACKGROUND: The reduction of beds in long-stay hospitals has led to concerns over the quality of care offered to the remaining residents as well as that provided in the community. This study seeks to compare the quality of care and quality of life (reported satisfaction) from residents in both types of setting. METHOD: A cross-sectional comparison was made of community residential homes and hospital wards drawn randomly from lists provided by local authorities in the outer London area. Samples were drawn from all the main provider types (local authority, housing association, private and joint

Communication to facilitate care of the obstetric surgical patient in a postanesthesia care setting.

Perinatal clientele have become more complex in their presentation and care today than ever before, requiring care in units other than the traditional obstetric or perinatal unit. Therefore, it is extremely important that the nurses caring for these patients know the associated standards of care, understand the importance of communication, and work to ensure the safety of the patient. This article provides an overview of the standards of ASPAN and the Association of Women's Health, Obstetrics, and Neonatal Nurses (AWHONN), guidance on initial and ongoing assessments of this population, and emp

The impact of trauma activations on the care of emergency department patients with potential acute coronary syndromes.

STUDY OBJECTIVE: Trauma systems improve the care of trauma patients; however, it is possible that prioritizing the emergency care of trauma patients might adversely affect other potentially ill patients requiring the same resources. We seek to determine whether the presence of a concurrent trauma activation negatively affects processes of care and outcomes for patients with potential acute coronary syndromes. METHODS: Patients who presented to the emergency department (ED) with a potential acute coronary syndrome from July 2003 to June 2004 were stratified according to whether they presented

Communication of incidental findings in the ED: Patient perspectives.

OBJECTIVES: Incidental findings are commonly identified on emergency department (ED) imaging, often unrelated to the reason for encounter. While system-level interventions have aimed to improve notification and follow-up, less is known about patients' experiences of receiving these unexpected results. We sought to explore ED patients' perspectives on incidental finding notification. METHODS: A qualitative analysis was conducted of semi-structured interviews of 16 adult ED patients undergoing abdominal CT or ultrasound imaging at a large academic ED. Interviews were originally collected for a

Urinary tract infections in patients admitted to rehabilitation from acute care settings: a descriptive research study.

The use of an indwelling urinary catheter comes with associated risks. At a hospital in southern California, nurses on the acute rehabilitation unit suspected their patients were arriving from acute care with undiagnosed urinary tract infections (UTIs). This descriptive research study quantified the incidence of UTI on admission to a rehabilitation unit and correlations with catheter use. During the study period, 132 patients were admitted to acute rehabilitation from an acute care setting, and 123 met criteria to participate in the study. Among participants, 12% had a UTI upon admission. Ques

Dueling priorities.

The Institute of Medicine report on medical errors and the resulting attention from the public and health care purchasers pushed patients safety to the forefront of providers' quality initiatives. Now, some health care leaders say the preoccupation with safety is wrongheaded and could detract from broad efforts to improve care.

Pelvic splenosis implanted in Douglas pouch mimics pseudo-endometriosis symptoms: a case report study.

INTRODUCTION AND IMPORTANCE: Pelvic splenosis is characterized by the autotransplantation of spleen fragments following tissue rupture. Since the fragments are mostly implanted into the Douglas pouch, endometriosis-like symptoms are developed, clinically. Thus, this case report demonstrated a successful surgical procedure for pelvic splenosis management. CLINICAL PRESENTATION: A 43-year-old woman with chronic pelvic pain was admitted to hospital. Medical history showed a car accident (at the age of 5 years) and severe spleen trauma followed by splenectomy. Patient assessment and sonography pr

Construction of a neovagina for patients with a flat perineum.

Vaginal dilatation affords an acceptable functional success rate for women with vaginal agenesis, provided there is an introitus with a pouch or dimple. In contrast, women with a flat perineum who lack labia majora, labia minora, and vaginal introitus have no alternative to development of a vagina-like orifice other than surgical vaginoplasty. This study addresses the anatomic and functional success rate of these women after undergoing modified McIndoe technique. Thirty-four patients with microphallus were assigned the female gender as the sex of rearing at the Johns Hopkins Hospital between 1

Admitted patients' recall of their treating doctors' names in adult wards of a South African district hospital.

BACKGROUND: Best practice guidelines suggest that patients should be well acquainted with their healthcare provider, and that doctors should introduce themselves or be introduced by another member of the patient's healthcare team. This aligns with the National Patients' Rights Charter by the National Department of Health, which states that every patient has the right to be treated by a named healthcare provider. Nevertheless, many patients have trouble recalling the name of their doctor. This study aimed to determine whether patients admitted to the adult ward of the National District Hospital
