The purpose of this prospective study was to determine the early subjective and functional result and outcome after primary implantation of humeral endoprosthesis in severely comminuted humeral head fractures in elderly patients. From 1993 to 1995, 27 humeral head fractures (27 patients) were treated by primary implantation of a Neer II modular shoulder prosthesis. Of these, 23 patients were women, 4 patients were men. The average age of patients was 76 (48-91) years. Postoperatively, one shoulder had to be mobilized under general anesthesia. Of the 27 patients 21 could be clinically examined

Pathological fracture in histologically proven post-Paget osteosarcoma of the humerus is a rare complication. Due to individual requests as well as age and comorbidity, a course of primary palliative treatment was chosen in the present case. Survival time after diagnosis was 9 months and the patient died of a tumor-independent disease. Even in combined treatment, consisting of surgery and (neo-)adjuvant radio-/chemotherapy, prognosis of osteosarcomas secondary to Paget's disease remains very disappointing. Therefore, in treatment of this highly lethal tumor the patient's individual requests an

Internal rotation deformity may contribute to the onset of tardy ulnar nerve palsy after a supracondylar fracture. We measured the angle of deformity in five patients with nerve palsy using an electrogoniometer and compared it with that of patients without palsy. Varus deformity of patients with nerve palsy was 23 degrees +/- 15.2 degrees (mean +/- SD), and that of patients without palsy was 12 degrees +/- 10.9 degrees. No significant difference was seen between the two groups. Internal rotation deformity of patients with nerve palsy was 16 degrees +/- 4.2 degrees, and that of patients without

Experiences are presented with the surgical treatment of 33 patients with traumatic radial nerve injuries after humerus fracture. Diagnostic and surgical difficulties of primary orthopaedic humerus fragment union by osteosynthesis with simultaneous evaluation of the damaged nerve. In the group of patients who had been given primary orthopaedic treatment the causes were analysed of reinnervation failure. In another group of patients undergoing metal plate removal after full bone union had been achieved the own experiences are presented related to an improved method of operative approach and del

Displaced three- and four-part fractures of the humeral head detached from their blood supply are at high risk for a poor functional outcome after internal fixation. Primary prosthetic replacement can restore the pretraumatic anatomy, and early function is possible. Head replacement within 2 weeks after trauma, reapproximation of the tuberosities, and reconstruction of the rotator cuff tear establishes early stability and good functional results in up to 80%.

We analysed retrospectively 50 humerus fractures in the vicinity of elbow joint and 40 fractures of the proximal humerus with respect to possible injuries of the vessel pattern of the epiphyseal plate. The normal vessel pattern of the epiphyseal plate was examined postmortem on 8 foetus and newborns up to 6 months. We plastinated the affected regions and could compare symmetry of the vessel pattern in the epiphyseal plates as well as therapeutic consequences, especially after injuries of the elbow joint. Vascular disturbances in the case of young children, i.e. by fractures of the lateral oder

Salter-Harris type IV fractures of the lateral humeral condyle are well-recognized injuries that have been previously described and classified. The case of an 11-year-old boy who sustained an unusual fracture of the distal humerus with the free fragment composed of 75% of the articular cartilage of the capitellum is presented. The cartilage was attached to only a thin shell of subchondral capitellar bone in continuity with the lateral aspect of the distal humeral physis and metaphysis. Radiographs did not show the magnitude of the articular cartilage component of the fracture, although physica

BACKGROUND Open distal humeral fractures (DHFs) often lead to loss of elbow function, thereby seriously affecting patient quality of life. The aim of this study was to evaluate the treatment outcomes of 2 surgical techniques to determine the better method for repairing open DHFs. Both groups were treated with immediate debridement first, and then group I had only internal fixation (IF), while group II underwent initial external fixation (EF) followed by IF surgery. MATERIAL AND METHODS This retrospective study included 32 patients who had open DHFs between 2013 and 2018. Twelve patients underw…

BACKGROUND: Although most proximal humeral fractures are attributed to osteoporosis, they are usually not considered individually in osteoporotic studies because of their lower incidence. The purpose of this study was to evaluate the incidence of nondisplaced proximal humeral fractures in comparison with other outpatient-treated osteoporotic fractures, as well as to assess their functional impact and effects on patient-perceived quality of life.
MATERIALS AND METHODS: In this multicenter, cross-sectional, prospective study, all osteoporotic fractures in postmenopausal women aged 50 years or o…

INTRODUCTION: Isolated fractures of the humeral trochlea are exceedingly rare because of the trochlea's deep intra-articular position and anatomical protection from direct trauma. The articular notch formed by the olecranon and coronoid processes shields the trochlea, which also lacks direct muscular or ligamentous attachments. Such fractures typically occur in conjunction with injuries involving the capitellum or the medial and lateral condyles.
CASE REPORT: We report the case of a 35-year-old male who sustained an isolated trochlear fracture following a physical assault. Initial anteroposte

Proximal humerus fractures are very common injuries. In young people the treatment options include non-operative management, internal fixation, and in selected cases, shoulder arthroplasty. One of the most challenging complications after internal fixation is the fracture non-union that can lead to the dreaded implant breakage. In such a situation, the treatment depends on the patient's age, bone quality, bone loss, infection status, and functional demands, and consists of revision open reduction and internal fixation employing autologous bone grafting or conversion to shoulder arthroplasty. We

BACKGROUND: Osteoporosis increases fracture risk and adversely affects outcomes after shoulder arthroplasty. Preoperative bisphosphonate use has been linked to higher rates of periprosthetic fracture after lower extremity total joint arthroplasty, but data in shoulder arthroplasty are limited. The purpose of this study was to evaluate whether anti-remodeling agents such as bisphosphonates or anabolic osteoporotic therapy influenced 1-year outcomes, including periprosthetic humeral fracture, acromial stress fracture, and all-cause revision, following anatomic total shoulder arthroplasty (aTSA)

Humeral shaft fractures are prevalent, with the majority receiving nonoperative treatment using a functional brace. However, some patients necessitate surgical intervention either as an initial approach or in cases where nonunion arises despite conservative management. This case report presents a patient who initially sustained a closed humeral shaft fracture managed nonoperatively with a Sarmiento brace. Unfortunately, she progressed to nonunion, prompting the need for surgical intervention via open reduction and internal fixation. Following surgery, she experienced hardware failure but ultim

Background and Clinical Significance: Aneurysmal bone cyst (ABC) is a benign but locally aggressive osteolytic lesion that typically arises in the metaphysis of long bones in children and adolescents. Extension across an open proximal humeral physis into the epiphysis is uncommon and creates a diagnostic challenge because unicameral bone cyst and telangiectatic osteosarcoma may show overlapping radiological features. Case Presentation: A 12-year-old girl presented with pain and restricted motion of the left shoulder after a bicycle-related fall. Radiographs demonstrated a large expansile lytic

Background and Clinical Significance: Non-prosthetic peri-implant fractures (NPPIFs) are rare injuries occurring around internal fixation devices, and are distinct from periprosthetic fractures. While most studies focus on the femur, humeral NPPIFs remain poorly documented. This case illustrates a complex humeral NPPIF and highlights key surgical considerations. Case Presentation: A 62-year-old woman presented with a spiral humeral shaft fracture (AO 12B2) after a fall. Following closed reduction and antegrade intramedullary nailing, an intraoperative peri-implant fracture occurred at the dist

We report the case of a 43-year-old woman who presented with combined radial, median, and ulnar nerve palsies and a spiral humeral shaft fracture after falling down stairs. Postinjury, the patient did not recover full median, ulnar, and radial nerve function despite aggressive hand therapy. Electrodiagnostic studies and imaging findings were concerning for traction neuropraxia or transection, radial nerve neuroma, and scar encasement of the radial, median, and ulnar nerves. The patient underwent radial, median, ulnar, and musculocutaneous nerve neuroplasty, nerve wrap application, nerve stimul

The association of radial nerve palsy and humeral shaft fracture is well known. Primary exploration and fracture fixation is recommended for open fractures and vascular injury while expectant management remains the standard of care for closed injuries. In the absence of nerve recovery, exploration and reconstruction is recommended 3 to 5 months following injury. When direct repair or nerve grafting is unlikely to achieve a suitable outcome, nerve and tendon transfers are potential options for the restoration of wrist and finger extension.
