Reverse Shoulder Arthroplasty for the treatment of Proximal humeral fractures in the Elderly (ReShAPE trial) : study protocol for a multicentre combined randomised controlled and observational trial.

BACKGROUND: Proximal humeral fractures are common in older patients. The majority are minimally displaced and are associated with good outcomes after nonoperative treatment. Poorer outcomes are associated with displaced, multipart fractures. There is no clear benefit from surgical fracture fixation compared to nonoperative treatment. Replacement of the fractured humeral head with a hemiarthroplasty is another treatment option, but has not been shown to be clearly superior to nonoperative treatment or internal fixation. Recently, reverse total shoulder arthroplasty has been used to treat these …

Postoperative malrotation of humeral shaft fracture after plating compared with intramedullary nailing.

OBJECTIVE: We supposed difference of rotation alignment of postoperative humeral shaft fracutres between open reduction and internal fixatio … . Group I had lower functional scores than group II (P < .05). Internal malrotation of 20° or more (humeral head internally rotated) was found in 27.2% of group I patients, but there was no malrotation in group II. The internal and external range of motion of the involved shoulder was significantly correlated with the degree of malrotation (P < .01). DISCUSSI …

Axillary shoulder with exaggerated rotation: the Hill-Sachs defect.

One of the most common fractures of the humeral head resulting from an anterior dislocation is the Hill-Sachs defect. Other special radiographic positions to demonstrate this injury may prove difficult for the patient to assume and maintain. An axillary shoulder projection with exaggerated external rotation is easy to position and clearly demonstrates the Hill-Sachs defect.

Fatigue-induced glenohumeral and scapulothoracic kinematic variability: Implications for subacromial space reduction.

Superior humeral head translation and scapula reorientation can reduce the subacromial space. While these kinematic abnormalities exist in injured populations, the effect of muscle fatigue is unclear. Additionally, these mechanisms were typically studied independently, thereby neglecting potential covariance. This research evaluated the influence of upper extremity muscle fatigue on glenohumeral and scapulothoracic kinematics and defined their relationship. Radiography and motion tracking systems captured these kinematic relationships, during scapula plane elevation, both before and after fati…

Management of Posttraumatic Avascular Necrosis of the Proximal Humerus.

The etiology of avascular necrosis (AVN) of the proximal humerus can be classified as idiopathic or posttraumatic, the latter being mainly due to proximal humerus fractures. Evidence suggests that posttraumatic AVN may require surgical intervention more often than idiopathic AVN of the proximal humerus. This article provides a comprehensive review of the management of posttraumatic AVN of the proximal humerus. Early stage AVN (stages 1 to 3) is commonly treated with nonoperative intervention or core decompression of the humeral head, whereas later stage disease (stages 4 and 5) may require hem…

Editorial Commentary: Superior Capsular Reconstruction Works Biomechanically but Should Be Used Selectively for Genuinely Irreparable Tears.

Superior capsular reconstruction (SCR) has undergone extensive biomechanical and clinical study. Systematic review of biomechanical studies has shown that SCR functions to depress the humeral head and improve the glenohumeral register, which provides proof of concept. Clinical studies have shown the short-term effectiveness of SCR, but this is influenced by graft type and thickness, as well as surgical technique. These studies are mostly flawed because follow-up is brief, postoperative imaging is often lacking, and the results may not be generalizable. Multiple systematic reviews pooling the r…

[Primary anterior shoulder dislocation].

Anterior dislocations of the shoulder are common among young males following trauma. Dislocations may be associated with fractures, rotator cuff tears as well as nerve and vascular injuries. However, recurrent instability is the major complication following primary dislocation of the shoulder. After physical examination and radiologic evaluation the humeral head may be reduced using a variety of methods. The methods differ in sedation, analgesia, body and arm position, type of manipulation, number of staff required for manipulation and finally, rates of success and complications. Following suc…

[Posttraumatic shoulder stiffness].

Posttraumatic stiffness of the shoulder joint is a frequent and socioeconomically relevant result of injury; however, prior to making the diagnosis as the only cause of a persisting impairment of motion, it is necessary to rule out other sequelae of trauma. Even intensive conservative treatment is mostly accompanied by the stagnation of the impairments of movement. In many cases the treatment of choice is early arthroscopic arthrolysis with a standardized approach and intensive follow-up treatment. In the surgical treatment of proximal humeral fractures with humeral head preservation, arthrosc…

Anterior shoulder instability: the role of advanced shoulder imaging in preoperative planning.

Despite advances in the understanding of anterior shoulder instability, recurrence rates after arthroscopic and open surgery have been reported to be as high as 30%. A successful operative outcome for patients with anterior shoulder instability requires the surgeon to perform a complete preoperative evaluation based on a thorough physical examination and advanced imaging techniques. In addition to the Bankart lesion, the treating surgeon must be aware of other copathologies, such as bony lesions of the glenoid or humeral head, humeral avulsion of the glenohumeral ligament, and articular cartil…

A prospective study of a new locking plate for proximal humeral fracture.

Displaced and unstable extra-articular fractures of the proximal humerus are treated by operative reduction and fixation using various techniques such as fixed plates and intramedullary implants. This prospective study evaluates the clinical efficacy of a novel implant that is attached to the humeral head with fixed-angle locking screws. Between October 2002 and November 2005, 99 people (mean age 62.8 years) with proximal humeral fractures received treatment involving this plate in our department. The mean follow-up time was 17.8 months. Outcome was assessed with radiography, the Constant-Murl…

[Evaluation of the treatment of the unicameral bone cyst in children (author's transl)].

Solitary bone cyst is characterized by its tenacity and the risk of recurrence. This fact led the authors to use a surgical procedure very different of the classic ways of treatment as they excluded any bone graft to the benefit of a muscle graft with wide resection. 35 cases have been operated on at several steps of childhood. The recurrence's rate decreased plainly and is no more than 9%. They concern only the wide humeral cysts which were active and close to the fertile plate of the humeral head. These recurrences have been very well tolerated and have not predisposed the child to some path…

Osteochondritis dissecans with associated secondary chondromatosis in an adolescent shoulder.

Osteochondritis dissecans (OCD) of the humeral head is rare and usually occurs in adolescents. Secondary synovial chondromatosis occasionally occurs in the shoulder but has only been reported twice in an adolescent. We describe the case and management of an adolescent male who presented with features of OCD and secondary chondromatosis in his shoulder occurring simultaneously.

A study of diagnostic reliability in inferior shoulder instability: Measurement of inferior humeral displacement using arthroscopy.

PURPOSE: The purpose of this study was to define arthroscopic measurement of inferior humeral displacement, which allows an objectively estimated reduction of capsular volume using arthroscopic techniques. TYPE OF STUDY: Prospective controlled study. METHODS: Based on a 3-grade classification, inferior displacement caused by axial traction was measured arthroscopically at 15 degree of forward flexion and 70 degree of abduction and neutral rotation over the humeral head in 143 patients (146 shoulders) who underwent surgery consecutively; 99 shoulders because of recurrent dislocations and 47 b…

[Imaging assessment and treatment of Charcot arthropathy of the shoulder assisted by NGH classification].

Charcot arthropathy of the shoulder is a rare destructive joint disorder that is most commonly secondary to syringomyelia and often leads to osteolysis of the shoulder joint and severe upper limb dysfunction. The diagnostic and therapeutic challenges of this condition lie not merely in identifying joint destruction, but in the nonspecific nature of its early clinical manifestations, which may mimic infection, tumor, or inflammatory arthropathy. In addition, the extent of bone destruction in the glenoid and humeral head may vary considerably, making conventional imaging descriptions insufficien…

Calcar screws and adequate reduction reduced the risk of fixation failure in proximal humeral fractures treated with a locking plate: 190 patients followed for a mean of 3 years.

BACKGROUND: Fixation of proximal humeral fractures (PHF) with locking plates has gained popularity over conservative treatment, but surgery may be complicated with infection, non-union, avascular necrosis (AVN) of the humeral head and fixation failure. Failure to achieve structural support of the medial column has been suggested to be an important risk factor for fixation failure. The aims of this study were to examine the effect of calcar screws and fracture reduction on the risk of fixation failure and to assess long-term shoulder pain and function. METHODS: This was a single-centre retrosp…
