Diagnosis, treatment, and analysis of long-term outcomes in idiopathic normal-pressure hydrocephalus.

OBJECTIVE: The response to shunt surgery for idiopathic normal-pressure hydrocephalus (INPH) is variable because INPH is difficult to distinguish from other conditions causing the same symptoms. To date, no clinical picture or diagnostic test can distinguish INPH or predict response to cerebrospinal fluid (CSF) shunt surgery. We reviewed our 10-year experience with INPH to characterize long-term outcome and to identify independent predictors of outcome after shunt surgery. METHODS: Patients were classified as having INPH only if they had: 1) ventriculomegaly, 2) two or more INPH clinical feat

Normal pressure hydrocephalus: cerebral hemodynamic, metabolism measurement, discharge score, and long-term outcome.

BACKGROUND: Regional CBF study has been reported effective in the selection of patient with NPH. However, controversial outcome had been reported. We sought to determine if the combination of rCBF measurement, cerebrovascular reactivity, and regional metabolism were positive predictors of shunt responsiveness in NPH syndrome. METHODS: Twenty-eight patients with clinical diagnosis of NPH were enrolled to study their rCBF in CSWM before and after the ACT challenge test, the regional CSWM metabolism by MRSI, and the clinical grading by the CSRIH defined by the Ministry of Health and Welfare of J

The pathophysiology of idiopathic normal pressure hydrocephalus: cerebral ischemia or altered venous hemodynamics?

BACKGROUND AND PURPOSE: Many theories of normal pressure hydrocephalus (NPH) stress the importance of ischemia in the deep white matter. Alternate theories stress a reduction in superficial venous compliance and changes in pulse-wave propagation. An overlap in the cerebral blood flow volumes measured between NPH and controls suggests that ischemia may not be a prerequisite for this condition. This study sought to compare blood flow and compliance measures in a cohort of patients with NPH selected for having arterial inflows above the normal range to see if deep brain ischemia or superficial he

Shunts in normal-pressure hydrocephalus: do we place too many or too few?

OBJECT: The average 65-year-old patient with moderate dementia can look forward to only 1.4 quality-adjusted life years (QALYs), that is, longevity times quality of life. Some of these patients suffer from normal-pressure hydrocephalus (NPH) and respond dramatically to shunt insertion. Currently, however, NPH cannot be diagnosed with certainty. The authors constructed a Markov decision analysis model to predict the outcome in patients with NPH treated with and without shunts. METHODS: Transition probabilities and health utilities were obtained from a review of the literature. A sensitivity an

Good outcome in patients with normal-pressure hydrocephalus and factors indicating poor prognosis.

OBJECT: The authors set out to describe the outcome in a subgroup of patients with normal-pressure hydrocephalus (NPH) in whom prognostic factors were poor. This subgroup of patients who had received shunts was selected according to strict criteria. METHODS: From a cohort of 56 patients with NPH in whom shunts were placed, the authors selected a subgroup with four of the factors traditionally considered to indicate poor prognosis: idiopathic type, cortical atrophy, longstanding symptoms, and presence of dementia in addition to old age. Twelve patients met the inclusion criteria. After receivi

Corpus callosum functioning in patients with normal pressure hydrocephalus before and after surgery.

OBJECTIVES: Our aim was to evaluate corpus callosum functioning in a group of patients with normal pressure hydrocephalus (NPH) before and after shunting. METHODS: Left ear-extinction under a dichotic listening task was evaluated in twenty-three patients with NPH, 30 patients with Alzheimer's disease and 30 aged controls. RESULTS: Patients with NPH had higher levels of left ear extinction than the control and Alzheimer's groups. Sixty-one percent of NPH patients exhibited left ear suppression, compared with 13% of Alzheimer's patients and 17% of controls. Following surgery, NPH patients show

Unifying concept for Alzheimer's disease, vascular dementia and normal pressure hydrocephalus - a hypothesis.

The three common forms of dementias in the elderly include Alzheimer's disease (AD), vascular dementia (VD) and normal pressure hydrocephalus (NPH). These disorders are distinguished by their specific pathological features. However, overlapping clinical and imaging features in a given case are not too uncommon. Based on alterations in CSF dynamics study, a unifying concept in the pathogenesis of AD and NPH has been proposed recently which may have therapeutic implications. Altered CSF dynamics by affecting the absorptive process may lead to hydrocephalic change. This may also affect clearance

Normal pressure hydrocephalus manifesting as transient prosopagnosia, topographical disorientation, and visual objective agnosia.

Usually, dementia, gait disturbance and urinary incontinence are an integral part of the clinical presentation of normal pressure hydrocephalus (NPH). However, NPH with transient visual cognitive disorders has not been reported previously. We herein describe an extremely rare case of NPH that presented with transient visual cognitive disorders and long lasting visual memory disturbances that subsided after CSF shunting. A 38-years-old man developed transient prosopagnosia, topographical disorientation, color vision disturbance, and visual objective agnosia that progressed over 5 years. Magneti

Acute subdural hematoma after lumboperitoneal shunt placement in patients with normal pressure hydrocephalus.

Acute subdural hematoma (SDH) is a rare but disastrous complication after lumboperitoneal shunt placement. Four of 206 adult patients with normal pressure hydrocephalus (1.9%) who underwent lumboperitoneal shunt placement suffered acute SDH following head trauma. The interval between shunt placement and acute SDH was one month to 7 years. Two patients had subdural effusion on computed tomography (CT) at 2- and 6-month follow up. All four patients required assistance in their daily activities before acute SDH onset. The traumatic event was a fall. On admission, CT revealed a large SDH that requ

Analysis of hearing loss after shunt placement in patients with normal-pressure hydrocephalus.

OBJECT: Following shunt placement for treatment of normal-pressure hydrocephalus (NPH), several patients suffered hearing loss. The authors undertook a study to analyze this outcome. METHODS: Sixteen patients in whom NPH was diagnosed were treated by placement of a ventriculoperitoneal shunt. Their hearing was assessed pre- and postoperatively by using pure tone audiometry. Two thirds of the ears tested showed a postoperative hearing loss of more than 10 dB. Recovery of the hearing loss occurred 6 to 12 weeks after shunt placement in 75% of the ears examined. CONCLUSIONS: Although shunt inse

[Diagnostic and therapeutic criteria of adult normal-pressure hydrocephalus. Descriptive multicenter national study].

INTRODUCTION: There are some controversial items concerning normal pressure hydrocephalus (NPH). The incidence of NPH is unknown since there is not agreement concerning diagnostic criteria. The choice of shunt location and valve type may be difficult if we consider the wide range of options available. We present a descriptive multicentric study which pretends evaluate the consensus between different centers. MATERIAL AND METHODS: We have requested 19 neurosurgical departments about diagnostic criteria, shunt location, valve type, cases number per year and therapeutic results. RESULTS: The st

Probabilistic diagnosis of normal pressure hydrocephalus and other treatable cerebral lesions in dementia.

Clinical profiles of dementia patients, differing with respect to age, presence of gait abnormalities and urinary incontinence are discussed. Epidemiological data, subjective probabilities and clinical reasoning are used to predict a treatable cerebral lesion, i.e., an intracranial space occupying lesion or normal pressure hydrocephalus (NPH). Our calculations help in distinguishing clinically between demented patients who are, and who are not likely to benefit from computed tomography (CT) investigation for treatable lesions and, eventually, from cerebrospinal fluid shunting for NPH. Utility

Cerebral blood flow in patients with normal-pressure hydrocephalus before and after shunting.

Cerebral blood flow (CBF) was measured by xenon-133 inhalation and single photon emission tomography in 17 demented patients with normal-pressure hydrocephalus before and after shunt treatment. All patients had a decreased conductance to outflow (C out) of cerebrospinal fluid as measured by lumboventricular perfusion (C out less than 0.12 ml X mm Hg-1 X min-1). Computerized tomography (CT) scanning, clinical assessment, and neuropsychological grading were performed pre- and postoperatively. The preoperative CBF studies revealed abnormal flow patterns in all patients. Fourteen patients showed m

Monoamine acid metabolites and cerebrospinal fluid dynamics in normal pressure hydrocephalus: preliminary results.

Lumbar and ventricular CSF concentration of homovanillic acid (HVA) and 5-hydroxy-indole-acetic acid (5-HIAA) have been determined in 13 patients admitted to hospital for suspected normal pressure hydrocephalus. Low values of HVA in lumbar CSF were found in all patients with reduced CSF absorption and CSF flow inversion. The HVA lumbar concentration remained low after shunt procedure; it increased if obstruction of the shunt occurred. The ventricular CSF concentration of HVA was normal before surgery; it became higher, in two cases, after surgery. No important variations were found in the lumb

Can Shunt Response in Patients with Idiopathic Normal Pressure Hydrocephalus Be Predicted from Preoperative Brain Imaging? A Retrospective Study of the Diagnostic Use of the Normal Pressure Hydrocephalus Radscale in 119 Patients.

BACKGROUND AND PURPOSE: The Normal Pressure Hydrocephalus Radscale is a combined scoring of 7 different structural imaging markers on preoperative brain CT or MR imaging in patients with idiopathic normal pressure hydrocephalus: callosal angle, Evans Index, Sylvian fissure dilation, apical sulcal narrowing, mean temporal horn diameter, periventricular WM lesions, and focal sulcal dilation. The purpose of this retrospective study was to assess the performance of the Normal Pressure Hydrocephalus Radscale in distinguishing idiopathic normal pressure hydrocephalus shunt responders from nonrespond

Relationship between valve opening pressure, body position, and intracranial pressure in normal pressure hydrocephalus: paradigm for selection of programmable valve pressure setting.

OBJECTIVE: There is little scientific basis for guidance in selecting the optimal valve for the treatment of normal pressure hydrocephalus. The aim of this study was to determine the programmable valve opening pressure setting that would result in a slight reduction in intracranial pressure (ICP) after a ventriculoperitoneal shunt is implanted. We also assessed whether shunt-induced ICP could be predicted on the basis of a simple hydrodynamic equation. METHODS: In this prospective study of 11 patients with normal pressure hydrocephalus, ICP was measured before and after implantation of a shun

Sensor-derived heel pressure metrics capture reversible gait dysfunction beyond conventional gait measures in normal pressure hydrocephalus.

BACKGROUND: The response to the cerebrospinal fluid (CSF) tap test helps determine the surgical eligibility of individuals with idiopathic normal pressure hydrocephalus (iNPH); however, conventional gait assessments have limited specificity and low negative predictive value. We investigated whether commercially available, low-cost sensor-embedded insoles could offer sensitive gait biomarkers for detecting changes in gait following the CSF tap test. METHODS: Seventy-seven individuals with iNPH were prospectively enrolled. Participants performed conventional gait assessments, including the 10-m
