Mutational analysis of the LDL receptor and APOB genes in Mexican individuals with autosomal dominant hypercholesterolemia.

The goal of this project was to identify families with autosomal dominant hypercholesterolemia (ADH) to facilitate early detection and treatment and to provide genetic counselling as well as to approximate the mutational diversity of ADH in Mexico. Mutational analysis of the LDLR and APOB genes in 62 index cases with a clinical and/or biochemical diagnosis of ADH was performed. Twenty-five mutations (24 LDLR, 1 APOB) were identified in 38 index cases. A total of 162 individuals with ADH were identified using familial segregation analysis performed in 269 relatives of the index cases. In additi…

Nitroxide derivatives for imaging of hypercholesterolemia-induced kidney dysfunction and assessing the effectiveness of antilipidemic drugs.

The present study was designed to clarify the possibility for application of nitroxide derivatives in magnetic resonance imaging (MRI) of hypercholesterolemia-mediated renal dysfunction in mice, as well as to assess the effectiveness of antilipidemic drugs (cholestyramine and ezetimibe). The mice were separated in four groups: (i) on a normal diet (ND) without medication (control); (ii) on a high cholesterol diet (CD) without medication; (iii) CD mice receiving cholestyramine; and (iv) CD mice receiving ezetimibe. In CD mice without medication, a hypercholesterolemia was developed, detected by…

[C-reactive protein is asociated to carotid intima media thickness in patients with isolated hypercholesterolemia].

Studies have shown that high concentrations of total cholesterol at the expense of LDL cholesterol and markers of inflammation are linked with subclinical atherosclerosis. This study was aimed to associate the carotid intima media thickness (IMT) and high-sensitivity C-reactive protein (hs-CRP) in patients with isolated hypercholesterolemia. The investigation was descriptive, transverse and included 100 patients divided into a control group (normocholesterolemic) and a study group (hypercholesterolemic). There were no significant differences in age and gender distribution among the groups. The…

An apparent inconsistency in parent to offspring transmission of point mutations of LDLR gene in familial hypercholesterolemia.

BACKGROUND: Familial Hypercholesterolemia (FH), the most common form of autosomal co-dominant hypercholesterolemia, is due to mutations in the LDLR gene, mostly minute or point mutations in the coding sequence. METHODS: Analysis of LDLR gene was performed by direct resequencing and multiplex ligation-dependent probe amplification (MLPA). RESULTS: LDLR gene resequencing showed that proband I.G., with the clinical diagnosis of homozygous FH, was homozygous for a mutation in exon 12 (c.1775 G>A, G571E) known to be pathogenic, and heterozygous for a mutation in intron 14 (c.2140 +5G>A). Proband'…

Treatment of hypercholesterolemia in patients with primary biliary cirrhosis might be more beneficial than indicated.

Cholesterol circulating levels are elevated in most of the patients with primary biliary cirrhosis. This review questions whether hypercholesterolaemia represents a cardiovascular risk in primary biliary cirrhosis and whether it should be treated. The published evidence indicates that hypercholesterolaemia in patients with primary biliary cirrhosis should be considered a cardiovascular risk factor only when other factors are present. Ursodeoxycholic acid the standard treatment of primary biliary cirrhosis improves the cholestasis and hereby lowers circulating levels of cholesterol. Primary bil

Effects of allopurinol, a xanthine oxidase inhibitor, on renal injury in hypercholesterolemia-induced hypertensive rats.

To investigate if increased lipid peroxidation is involved in hypercholesterolemia-induced hypertension and renal injury, we examined the effects of allopurinol, a xanthine oxidase inhibitor, on these conditions. Groups of male Sprague--Dawley rats were fed for 8 weeks with a high-cholesterol diet (4% cholesterol), a high-cholesterol plus allopurinol (10 mg/kgBW/day) diet or a normal diet. Systolic blood pressure (SBP), serum lipids, uric acid (UA) and malondialdehyde (MDA) as a measure of lipid peroxides, and urinary excretion of protein (UP) were measured after 0, 4 and 8 weeks. Urinary excr…

Influence of hypercholesterolemia and acute graft rejection on chronic nephropathy development in renal transplant recipients.

Graft endothelial lesions resulting from acute rejection may be sustained by concomitant hypercholesterolemia, thus increasing the risk of chronic graft failure. The present study was undertaken to examine the influence of hypercholesterolemia and acute graft rejection (AGR) episodes on graft function and graft loss due to chronic nephropathy. A cohort of 336 patients transplanted between 1993 and 2000 having graft function at 12 months after transplantation were examined. Immunosuppressive therapy consisted of CsA, azathioprine, and corticosteroids in 90% with 10% of patients receiving mycoph…

Differences in the phenotype between children with familial defective apolipoprotein B-100 and familial hypercholesterolemia.

Familial defective apolipoprotein B-100 (FDB) is a dominantly inherited genetic disorder resulting from a point mutation in the apolipoprotein (apo) B gene and is associated with significantly elevated plasma total and LDL cholesterol levels. Despite numerous descriptions outlining the phenotype of children with familial hypercholesterolemia (FH), no study has described the biochemical and clinical phenotype in a cohort of children with FDB. The phenotypes of FH and FDB, therefore, have not been compared in children. We have studied a cohort of 38 Dutch children (all <20 years old) with FDB fr…

The effect of combined deendothelialization and hypercholesterolemia on some arterial lysosomal and glycolytic enzymes and lactate in rabbits.

In eight New Zealand white male rabbits the abdominal aorta and one iliofemoral artery was balloon deendothelialized (group A). After 2 weeks they were kept for 6 weeks on a high cholesterol diet together with eight unoperated rabbits (group B). Eight more rabbits were kept on a commercial diet only (group C). The degree of atherosclerosis was much higher in the deendothelialized Group A vessels than in the uninjured group B vessels. The activity of lactate dehydrogenase and of the rate-limiting glycolytic pyruvate kinase was significantly increased and the activity of lipoamide dehydrogenase

Single-cell dissection of cholesterol metabolism reveals cell-type-specific genes and hypercholesterolemia-associated remodeling.

This study was aimed at systematically characterizing cell-type-specific cholesterol metabolism in human blood samples and investigating the impact of hypercholesterolemia on the composition and gene expression of immune and stromal populations. Single-cell RNA sequencing (scRNA-seq) data from 31 human blood samples, encompassing 472,204 high-quality circulating cells, were analyzed. Pathway activities were assessed via gene set variation analysis (GSVA), and cell-type-specific markers were identified using differential expression analysis. Clinical cholesterol levels were used to stratify sam…

Systematic evaluation of mobile element insertions in autosomal dominant hypercholesterolemia genes using short-read sequencing.

BACKGROUND AND AIMS: Autosomal dominant hypercholesterolemia (ADH) is a common monogenic disorder conferring markedly increased cardiovascular risk. While short-read next-generation sequencing (NGS) efficiently detects small nucleotide variants, it often misses mobile element insertions (MEIs), recently recognized as an additional pathogenic mechanism in LDLR-related ADH. This study aimed to systematically evaluate the contribution of MEIs to ADH in a large, well-characterized cohort. METHODS: NGS data from 2393 unrelated probands were retrospectively screened for MEIs using MELT software. Ca…

Reverse Cascade Genetic Screening for Revealing New Cases of Familial Hypercholesterolemia in Russia: Pilot Project.

BACKGROUND: Reverse cascade genetic screening, following initial lipid testing, is an effective strategy for the early diagnosis of familial hypercholesterolemia (FH) and related hereditary dyslipidemias. In this study, we present the results of a pilot project in Russia. METHODS: This was a hybrid retrospective-prospective cross-sectional study. Analysis of 1897 lipid screenings of children referred to the main pediatric centers in St. Petersburg, Russia, demonstrated that 60 (3.2%) met the Simon Broome criteria for FH, defined as total cholesterol (TC) ≥6.5 mmol/L or low-density lipoprotein…

Real-World Outcomes of Lipoprotein Apheresis in Homozygous Familial Hypercholesterolemia: A Single-Center Longitudinal Experience.

Background: Lipoprotein apheresis (LA) remains an important treatment option for patients with homozygous familial hypercholesterolemia (HoFH) who have persistently severe low- density lipoprotein cholesterol (LDL-C) elevation despite intensive lipid-lowering therapy. Therapeutic plasma exchange (TPE) may serve as a temporary alternative when selective LA is unavailable. Methods: This retrospective, single-center longitudinal observational study evaluated six patients with HoFH treated with double-filtration plasmapheresis (DFPP) between 2010 and 2023. During a temporary interruption in DFPP a…

Ischemic stroke and intracranial large-artery disease risk in familial hypercholesterolemia: A prospective Japanese study.

BACKGROUND AND AIMS: The contribution of dyslipidemia to stroke risk is generally considered weaker than that of hypertension and diabetes. However, some previous studies suggest that dyslipidemia is associated with intracranial major artery stenosis (ICAS) and ischemic stroke, particularly in Asians populations, who are more susceptible to ICAS than Caucasians. We hypothesized that dyslipidemia, and specifically familial hypercholesterolemia (FH), may play a different role in the development of stroke in Asian populations. This study aimed to assess the prevalence and incidence of symptomatic…

Multimodal Therapy Achieves Secondary Prevention LDL-C Targets in LDL-Receptor Null Homozygous Familial Hypercholesterolemia.

BACKGROUND: Homozygous familial hypercholesterolemia is a rare condition most commonly associated with pathogenic variants in the LDLR gene that leads to mortality before age 20 if not treated. CASE SUMMARY: A 4-year-old boy of Lebanese origin with multiple skin xanthomas was found to have untreated low-density lipoprotein cholesterol (LDL-C) of 1005 mg/dL (26 mM). Gene analysis revealed biallelic identical LDLR variants with <2% residual LDLR activity (LDLR-null). DISCUSSION: With combination therapy including maximum dose rosuvastatin, ezetimibe, plasma exchange, lomitapide, and evinacumab…

Cascade screening of a Pakistani consanguineous familial hypercholesterolemia cohort: Identification of seven new homozygous patients.

BACKGROUND AND AIMS: Familial hypercholesterolemia (FH) is a genetic disorder characterized by elevated low-density lipoprotein cholesterol (LDL-C) levels from birth, significantly increasing the risk of premature cardiac events and mortality. In Pakistan, despite the potential burden of FH, comprehensive studies evaluating its genetic characteristics, cascade screening significance, and lipoprotein (a) [Lp(a)] levels remain scarce. Understanding these factors is crucial for effective diagnosis, risk assessment, and management of FH in the Pakistani population. METHODS: After the identificati…

Zwitterionic lipid nanoparticles for efficient siRNA delivery and hypercholesterolemia therapy with rational charge self-transformation.

Rationale: Effective delivery of small interfering RNA (siRNA) remains a significant challenge in treating hypercholesterolemia due to biocompatibility, cellular uptake, and endosomal escape issues. Rational regulation of carrier surface charge contributes to efficient siRNA delivery in vivo. Methods: This study introduces zwitterionic lipid nanoparticles (ZwiLNPs) as a novel solution to these challenges. By leveraging the unique properties of zwitterionic polymers, we achieved robust siRNA encapsulation and targeted delivery. The design of ZwiLNPs facilitates charge self-transformation in res…
