Gastrointestinal CancerEmerging treatment modalities for gastric cancer with peritoneal metastases: a systematic review.

INTRODUCTION: Gastric cancer remains associated with a high mortality, since patients often present with advanced-stage disease such as peri … retrieved from 9 different countries, investigating systemic therapy, intraperitoneal therapy (IP), hyperthermic intraperitoneal chemotherapy (HIPEC) with or without cytoreductive surgery (CRS), and pressurized intraperitoneal aerosol chemotherapy (PIPAC). The epidemiology, current evidence and controversies in diagnosis and management of patients with colorectal peritoneal metastases.

BACKGROUND: Peritoneal metastases (PM) are a common site of spread in colorectal cancer (CRC) and are associated with poor survival outcomes. The true burden of disease is difficult to quantify due to limitations in imaging and limited symptoms until advanced disease. Systemic therapy has limited benefits and there is an increasing interest in the use of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). This narrative review summarises the current literature on epidemiology of PM in CRC and evidence based current management strategies. METHODS: PubMed was sear…

These neoplasms … the same time, it has standardized the perioperative management of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC), as well as the clinical application of the pre-rehabilitation system, to enhance the practical operability. Review of Options to Traditional HIPEC for Prevention and Treatment of Peritoneal Metastases.

Cytoreductive surgery with HIPEC has definite application to the management of selected patients with peritoneal metastases. Patients who profit most have a complete cytoreductive surgery. Higher-grade tumors such as colorectal cancer, gastric cancer, and ovarian malignancy are benefited by CRS and HIPEC only under limited circumstances. High-grade tumor invades subperitoneal lymphatics where HIPEC is not effective. Options to traditional HIPEC for treatment of invasive intraabdominal malignancies with peritoneal metastases must be explored.

Prospects of the surgical management of colorectal peritoneal metastasis.

BACKGROUND: Colorectal cancer (CRC) is a common malignancy with the propensity to metastasize. Common sites of metastasis include the liver, lungs, and peritoneum, with peritoneal metastases (PMs) having the worst prognosis. Unfortunately, systemic chemotherapy is often less effective in the treatment of PMs. Therefore, removal of all visible tumor via cytoreductive surgery along with intraperitoneal (IP) therapies has been used. METHODS: A comprehensive review of the literature was conducted using MEDLINE/PubMed and Web of Science with an end date of September 1, 2024, regarding cytoreductiv

[Similarities and differences between Chinese protocols and international practices in the management of peritoneal tumors].

Peritoneal surface malignancies represent a metastatic pattern of advanced gastrointestinal and other malignant tumors, which are difficult to treat and associated with poor prognosis. Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (CRS+HIPEC) is the standard treatment for peritoneal tumors recommended by the Peritoneal Surface Oncology Group International (PSOGI). In recent years, peritoneal tumors have evolved from a "cold" field to a "hot" topic in oncological academia. An increasing number of prospective clinical studies have been published worldwide, providi…

Different approaches of intraperitoneal chemotherapy such as hyperthermic intraperitoneal chemotherapy and early postoperative intraperitoneal chemotherapy have shown promising results as prophylactic treatments aiming to prevent peritoneal recurrence. CONCLUSION: Future studies are warranted to find safe and effective treatments to prevent peritone …

Peritoneal Carcinomatosis in Colorectal Cancer: Review and Update of Current Clinical Data.

The peritoneal metastasized colorectal cancer (pmCRC) represents a serious health problem worldwide with a special emphasis in the developed countries. Several guidelines recognize the role of multimodal therapy consisting of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) in the treatment of pmCRC. New data suggests that some other factors, eg, tumor biology, immune profile, neoadjuvant chemotherapy may play a predictive role for the oncological outcome of these patients.

Peritoneal carcinomatosis with urachal signet-cell adenocarcinoma.

Urachal adenocarcinoma is an uncommon clinicopathologic entity associated with a dismal prognosis. We report a case of peritoneal carcinomatosis from urachal adenocarcinoma (signet cell type) treated with cytoreductive surgery and intraperitoneal hyperthermic chemotherapy (IPHC). Prior to treatment, disease had progressed with systemic chemotherapy. The patient remained free of symptomatic peritoneal disease or local recurrence but eventually died 23 months after IPHC and 31 months after diagnosis due to widespread bone metastases.

Locally advanced perforated appendiceal cancer: Case report and review.

Appendiceal cancers may be difficult to diagnose even after comprehensive investigation. This report of locally advanced perforated appendiceal adenocarcinoma attached to the terminal ileum, cecum, and rectosigmoid illustrates the management challenges that require comprehensive knowledge of pathologic variations and range from simple appendectomy to cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

Anaesthesia for intraperitoneal hyperthermic perfusion.

Intraperitoneal hyperthermic perfusion is a method of regional chemotherapy shown to be effective in the prevention and treatment of peritoneal metastases. General anaesthesia for a patient who required this procedure is described. Guidelines for management are suggested, with particular emphasis on temperature control, fluid and electrolyte balance and postoperative care.

[Recent progress and future prospects of treatment for peritoneal metastasis in gastric cancer].

Peritoneal metastasis is one of the most frequent patterns of metastasis in gastric cancer, and remains a major unmet clinical problem. Thus, systemic chemotherapy remains the mainstay of treatment for gastric cancer with peritoneal metastasis. In well-selected patients, the reasonable combination of cytoreductive surgery, hyperthermic intraperitoneal chemotherapy (HIPEC), and neoadjuvant intraperitoneal chemotherapy with systemic chemotherapy will bring significant survival benefits to patients with gastric cancer peritoneal metastasis. In patients with high-risk factors, prophylactic therapy…

Role of prophylactic HIPEC in non-metastatic, serosa-invasive gastric cancer: a literature review.

The role of prophylactic hyperthermic intraperitoneal chemotherapy (p-HIPEC) in serosa invasive gastric cancers without gross or microscopic peritoneal disease, to reduce the rate of peritoneal relapse is an area of ongoing research. Although p-HIPEC is effective in reducing the rate of peritoneal relapse and improving disease free and overall survival with or without adjuvant chemotherapy, when added to curative surgery in locally advanced, non-metastatic gastric cancers, the available literature is at best, heterogeneous, centre-specific and skewed. Apart from that, variations in the systemi…

Hyperthermic intraoperative thoracoabdominal chemotherapy.

Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment option for selected patients with pseudomyxoma peritonei (PMP) and diffuse malignant peritoneal mesothelioma (DMPM). Tumor infiltration of the hemidiaphragm requiring partial resection occurs as a result of large volume and/or invasive disease at this anatomic site. Transmission of disease from abdomen to chest is a great danger in this group of patients. From a prospective database, patients who had diaphragm resection and then hyperthermic thoracoabdominal chemotherapy (HITAC) as a component …
