We describe studies undertaken in a diabetic patient with acquired sustained severe hypernatraemia. Arginine vasopressin levels and thirst scores were grossly subnormal in the presence of marked hypernatraemia but arginine vasopressin increased normally under the influence of negative pressure-induced hypovolaemia. Despite very low levels of arginine vasopressin, polyuria was not a feature suggesting acquired renal hyper-responsiveness. This patient is an additional case of acquired osmoregulatory dysfunction, whose features do not fall neatly into previously described categories.

Twenty-one hepatic Echinococcus granulosus cysts (maximal diameter, 7.5 cm +/- 4.0) in 12 patients were aspirated and irrigated with hypertonic (20%) saline under sonographic guidance. All patients had signs and symptoms of a hepatic mass caused by the cysts, which had a prominent fluid component that appeared anechoic or hypoechoic, with marked enhancement of back wall echoes. 14 children with anorectal prolapse have been treated by injection therapy with 25% saline solution. Experimental studies have shown no noticeable reactions between rectum and surrounding area, as far as adhesion and thus rectal fixation are concerned. Manometric examinations show an increased contraction tendency of the Musculus puborectalis after injection, which should lead to a depression of the kink at the rectosigmoidal junction and to a healing of the prolapse. The associated stool-regulating measures contribute a lot to remedy the prolapsed rectum.

Five patients with severe hyponatraemia and epileptiform seizures were given 50 ml of 29.2% saline (250 mmol) through a central venous catheter over 10 minutes to control seizures rapidly, reduce cerebral oedema, and diminish the incidence of permanent neuronal damage. The saline controlled seizures in all patients, increasing the mean serum sodium concentration by 7.4 (SD 1.14) mmol(mEq)/l and decreasing the mean serum potassium concentration by 0.62 (0.5) mmol(mEq)/l. Further saline and frusemide were then administered over 10 (2) hours, raising the serum sodium concentration by 2.14 (0.49)

Severe symptomatic hyponatremia (serum sodium level below 120 meq/liter) is often a life-threatening emergency that can result in permanent neurologic damage or death if left untreated. Early recognition and rapid correction to mildly hyponatremic levels by the administration of hypertonic saline are important in order to reduce the potential mortality and morbidity. If the serum sodium level is more than 105 meq/liter, it can be corrected to a value of 125 to 130 meq/liter. The blood-aqueous humor barrier in adult rhesus monkeys was opened by intracarotid infusion of a 25% mannitol solution. Each monkey had one to four infusions into the same artery, with at least one week between subsequent procedures. The monkeys were observed clinically for 3 to 212 days. The effect of age, sex and salt intake on the hypertension produced in homozygous (DI) and heterozygous (non-DI) Brattleboro rats and Long Evans rats was studied. The left kidney was removed at the age of 25, 35, 50 and 80 days (UNX 25, UNX 35, UNX 50, UNX 80), and 0.6% NaCl solution or water was offered as drinking fluid.
