Early diet in preterm infants and later cognition: 10-year follow-up of a randomized controlled trial.

BACKGROUND: Achieving adequate nutrition in preterm infants is challenging. The post-discharge period may be critical for influencing growth and cognitive outcomes. We studied the effects of post-discharge nutrition on childhood cognition. METHODS: Preterm-born children were randomized at ~36 weeks corrected age (CGA) to either preterm formula (PTF) or term formula (TF) until 6 months, or PTF until 40 weeks CGA, then TF until 6 months (crossover group). Childhood cognition was assessed using the short form Wechsler Intelligence Scale for Children III, allowing computation of full-scale intell

[Gastrointestinal hormones during minimal enteral feeding of sick premature infants].

OBJECTIVE: To measure gastrointestinal hormonal response (GHR) with minimal enteral feeding (MEF) in sick premature infants. METHODS: Forty-one babies birth weight < 1800 g receiving total parenteral nutrition or intravenous solutions entered the study. They were distributed in two groups: group I: 26 infants (early enteral feeding < or = 5 d) and group II = 15 infants (late enteral feeding = 10-14 d). A diluted special formula was used as MEF starting with 1 mL hourly with daily increments of 1 mL up to 120 mL. Basal and final determinations of GHR were done before and after the MEF. RESULT

Persistent morbidity and mortality of protein calorie malnutrition in young infants with CF.

Young infants with cystic fibrosis (CF) are at risk for developing symptomatic protein-calorie malnutrition (PCM) characterized by hypoalbuminemia, edema, and anemia. We reviewed the hospital charts of all infants less than 12 months of age referred to our CF clinic between 1979 and 1982 and found nine patients with PCM (13%). Their courses were characterized by age less than 7 months (nine patients), frequent use of soy formula (eight patients), presence of gastrointestinal or respiratory symptoms before being diagnosed with CF (eight patients), severe respiratory distress often requiring mec

Modulation of Pharyngoesophageal Motility Characteristics During Concurrent Oral Feeding in Infants: Novel Pilot Observations.

Effect of interventions or diseases on the mechanisms of swallowing difficulties, and indications to change nipples, flows, or formula thickness in high-risk infants remains unclear. We evaluated and compared the effects of nipple flows and/or formula thickness on pharyngoesophageal timing and amplitude characteristics during oral milk challenge (OMC). A total of 48 OMC trials were undertaken in 12 infants [born at 36.4 ± 3.8 weeks gestation, studied at 48.0 ± 5.6 weeks postmenstrual age (PMA)] concurrent with high resolution pharyngoesophageal manometry (HRPM). We evaluated timing and amplitu

Iron status of one-year-olds and association with breast milk, cow's milk or formula in late infancy.

PURPOSE: Studies on iron status in infancy and early childhood have shown contradicting results concerning prolonged breast-feeding and cow's milk intake. The aim of the present study was to investigate associations between iron status among one-year-olds and feeding, with focus on the type of milk. METHODS: Randomly selected healthy infants were prospectively investigated until 1 year of age in two cohorts born 1995-1996 (n = 114) and 2005 (n = 140). Information on birth data, feeding and growth until 12 months and iron status at 12 months was collected. Data from the two cohorts were pooled

The Australasian Society of Clinical Immunology and Allergy position statement: Summary of allergy prevention in children.

A family history of allergy and asthma identifies children at high risk of allergic disease. Dietary restrictions in pregnancy are not recommended. Avoiding inhalant allergens during pregnancy has not been shown to reduce allergic disease, and is not recommended. Breastfeeding should be recommended because of other beneficial effects, but if breast feeding is not possible, a hydrolysed formula is recommended (rather than conventional cow's milk formulas) in high-risk infants only. Maternal dietary restrictions during breastfeeding are not recommended. Soy formulas and other formulas (eg, goat'

Risk Assessment of Dietary Exposure to Fluoride from Follow-On Milk Consumption.

Breastfeeding based on the use of follow-on milk may contain traces of xenobiotic elements that could pose a risk to the health of the vulnerable population for which it is intended. Fluorine is a non-essential element that, at high concentrations, can produce adverse health effects such as dental fluorosis, decreased IQ (intelligence quotient), thyroid alterations, and kidney damage. Given the vulnerability of infants and the possible presence of fluoride in this type of product, the content of this anion was determined in a total of 46 samples of follow-on milk from different brands and type

Risk factors for enamel fluorosis in a nonfluoridated population.

The purpose of this case-control investigation was to investigate the possible association between mild-to-moderate enamel fluorosis and exposure during early childhood to fluoride supplements, fluoride toothpaste, and/or infant formula use in nonfluoridated communities. Analysis was performed on 460 10- to 13-year-old children, born after 1979, who were residents of six nonfluoridated communities in Massachusetts and Connecticut. The fluorosis status of the subjects was determined on the basis of a clinical dental examination using the Fluorosis Risk Index (FRI). Risk factor exposure was asce…

Hydrophilic guardians: Putative core proteins unaltered by 22 gene deletions mediate desiccation resistance in Cronobacter sakazakii.

Cronobacter sakazakii is a notorious foodborne pathogen linked to severe infections in neonates, with powdered infant formula serving as a common transmission route due to its ability to survive under desiccated conditions. Understanding the molecular mechanisms underlying its desiccation resistance is crucial for developing effective control strategies. In this study, we employed comparative proteomics and hydropathy profiling to investigate putative core proteins potentially involved in desiccation resilience. Proteomic analysis of wild-type and 22 knockout strains identified 1354 proteins, …

Exploring the complexities of infant feeding decisions for immigrant pregnant people living with HIV in Ontario, Canada: a qualitative study.

North American societies recommend that Pregnant People Living with Human Immunodeficiency Virus (PLWHIV) use formula to feed their infants to eliminate the risk of perinatal HIV transmission. However, many Canadian PLWHIV have migrated from HIV-endemic countries, where the World Health Organization recommends exclusive breastfeeding. These opposing recommendations, along with the complex interplay of social, cultural, and personal factors, create tension when making decisions regarding infant feeding. This study describes the experiences of PLWHIV in making decisions to inform healthcare prov

"I love my baby more than they do": infant feeding decision-making among mothers living with HIV in the United States.

In the US and other high-income countries, national guidelines traditionally recommended against breastfeeding infants born to women living with HIV (WLHIV), due to the risk of HIV transmission to the infant. In 2023, US guidelines were changed to recommend collaborative decision-making and support for WLHIV who choose to breastfeed. Little is known regarding how WLHIV make the decision of how to feed their babies, leaving clinicians lacking guidance on how to provide effective counseling. We therefore conducted a semi-structured interview study of pregnant and recently postpartum WLHIV in the

Sex differences in preterm nutrition and growth: the evidence from human milk associated studies.

Sexual dimorphism of the fetus manifests itself even during pregnancy. Preterm births are more common in pregnancies with male fetuses. Intrauterine and postnatal growth nomograms are sex-specific. The human milk composition in term infants appears to be sex-specific. Early nutrition has sex-specific effects and neurodevelopmental outcomes. A large same-sex twin study suggests that a mother's own milk (MOM) provides sex-specific growth advantages probably related to the calibration of a mother's milk based on her newborn's sex. Formula composition does not vary with infant sex, which may be on

Feeding modalities and the onset of the neonatal abstinence syndrome.

Breast milk has been reported to ameliorate the severity and outcome of neonatal abstinence syndrome (NAS). The mechanism of this beneficial effect of breast milk on NAS remains unclear, as the negligible amount of methadone transmitted via breast milk is unlikely to have an impact on NAS. The aim of this study was to compare the impact of different feeding modalities on the onset of NAS. A retrospective medical record review was conducted on 194 methadone-maintained mother/infant dyads. Infants were categorized on the first 2 days of life as predominantly breastfed, fed expressed human breast

Are diet and feeding behaviours associated with the onset of and recovery from slow weight gain in early infancy?

Infants with slow weight gain cause concern in parents and professionals, but it is difficult to be certain whether such infants are genetically small or whether their energy intake is insufficient. The aim of the present study was to assess the impact of diet and feeding behaviours on slow weight gain early in infancy. The sample was 11 499 term infants from the Avon Longitudinal Study of Parents and Children (ALSPAC). A total of 507 cases of slow weight gain from birth to 8 weeks were identified and the remaining 10 992 infants were used as controls. It was found that infants who gained weig

Disadvantages of a weight estimation formula for macrosomic fetuses: the Hart formula from a clinical perspective.

PURPOSE: Sonographic fetal weight (FW) estimation to detect macrosomic fetuses is an essential part of everyday routine work in obstetrics departments. Most of the commonly used weight estimation formulas underestimate FW when the actual birth weight (BW) exceeds 4000 g. One of the best-established weight estimation formulas is the Hadlock formula. In an effort to improve the detection rates of macrosomic infants, Hart et al. published a specially designed formula including maternal weight at booking. The usefulness of the Hart formula was tested. METHODS: Retrospective study of 3304 singleto
