Bacteriology of dental infections.

The most common dental diseases, periodontal disease and dental caries, are chronic infections caused by bacteria of normal oral flora. When these bacteria increase in number and irritation exceeds the host defence threshold, disease arises. The human oral flora comprises more than 300 different bacteria. During the last decade approximately 10 species, mainly Gram-negative anaerobes, have been noted as putative pathogens in periodontal disease. The Gram-positive and facultatively anaerobic mutans streptococci are aetiologically the most important bacteria in dental caries. Data have rapidly i…

[Viral infections and asthma].

Respiratory tract viruses have been held responsible for the genesis and aggravation of asthma in both children and adults. The relationship between asthma and viruses is looser in adults than in children, but it has been demonstrated that viral infections can generate a peripheral obstructive disorder on the one hand, and a transient non-specific bronchial hyperreactivity in healthy subjects on the other hand. Current data are insufficient to define clearly the alterations induced by viral infections and responsible for bronchial inflammation and non-specific bronchial hyperreactivity. Numero…

[Infections after cesarean section].

Post-operative infectious morbidity was recorded in 390 patients who delivered by Caesarean section. The patients were observed closely while in hospital. Upon leaving the hospital, each was given a questionnaire, which was answered by 350 (89.7%). 10% of the patients had experienced post-operative infections, the most frequent being infections of the urinary tract or of wounds. There was no significant difference in post-operative infectious morbidity in the case of elective as against emergency Caesarean section. This may have been due to the use of prophylactic antibiotics in many cases of …

Unusual infections in humans.

Nine cases of unusual infections in humans are presented. In each case, we present the clinical history, histopathologic changes (if indicated), morphologic features of the causative organism, diagnosis, discussion, differential diagnosis, therapy, and current literature. All of the cases are illustrated with pertinent photographs. The nine cases are as follows: (i) acanthocephaliasis, the first acquired human infection by Moniliformis moniliformis in the United States; (ii) dipylidiasis, an uncommon infection caused by the dog tapeworm, Dipylidium caninum; (iii) granulomatous amebic encephali…

Fluoroquinolones in bronchopulmonary infections.

In recent years, Streptococcus pneumoniae and Haemophilus influenzae, two of the most common pathogens causing bronchopulmonary infections, have developed resistance towards beta-lactam antibiotics in many areas of the globe. In some countries, resistance rates are so high that treatment with penicillin can not be recommended as the therapy of choice. Unfortunately, many S. pneumoniae strains resistant to penicillins are also resistant to co-trimoxazole and erythromycin, and even to the novel macrolides. Present fluoroquinolones may have to be used in such resistant cases. The fluoroquinolones…

Quinolones in intracellular infections.

Intracellular parasites are those which spend most of their lives within host cells. The fluoroquinolones demonstrate favourable intracellular pharmacokinetics for the treatment of intracellular infections; these agents diffuse and accumulate in the phagocytes, mainly in the cytosol, and do not associate with cellular organelles. The fluoroquinolones are generally active against Salmonella spp. in vitro, and have been used successfully in the treatment of typhoid fever, Salmonella bacteraemia in patients with AIDS, and chronic enteric carriage. Fluoroquinolone monotherapy has also been found s…

Infections other than AIDS.

Neurologic manifestations of severe infectious complications of drug abuse and chronic alcoholism are reviewed in this article. Portals of entry from cutaneous postinjection infections and multiple vascular injection sites may lead to pyomyositis, tetanus, infective endocarditis, meningitis, brain abscesses, and vertebral osteomyelitis. Chronic intranasal abuse of cocaine may be followed by frontal osteomyelitis, botulism, brain abscess, and visual loss. Problems of hepatitis, malaria, and syphilis in drug abusers are discussed also.

Pseudomonas toe web infections.

We present the case of a patient with a presumed tinea pedis infection who was referred to us after being treated with oral griseofulvin for two weeks with no improvement. We used a simple Wood's light examination and culture to establish the diagnosis of Pseudomonas toe web infection and achieved rapid resolution with oral ciprofloxacin treatment and local application of Castellani's paint.

[Cytomegalovirus infections and pregnancy].

Infection by Cytomegalovirus is one of the most common viral infections in obstetrics and gynaecology. Cytomegalovirus can be transmitted across placenta at any time during gestation, producing a spectrum of diseases within the newborn. The fetus is mainly involved through haematogenous spread of the virus from the mother during primary infection. Even if the mother has an antibody against cytomegalovirus it does not prevent congenital infection. First trimester infection is associated with significant congenital anomalies affecting the central nervous system and sensorial nerves. Infection of…

[Osteoarticular infections in children].

We presented a retrospective review of osteoarticular infections diagnosed at Sant Joan de Deu Hospital from Barcelona, in the last 10 years (1981-1990). A total of 127 arthritis and 113 osteomyelitis were recorded in children aged from a few days to 15 years. This represents a 3.5% of all infectious diseases patients during the study period. The most common microorganism was S. aureus (52.3%) followed by Brucella (8.2%) and Salmonella as well as Streptococcus agalactiae (7.3% each). The most common involved joint were hip and knee in arthritis cases, but long bones (tibia and humerus) in oste…

Carbohydrates in herpesvirus infections.

Carbohydrate structures constitute important factors in the pathogenesis of herpesvirus infections for three reasons: (i) Human herpesviruses specify several biologically active glycoproteins, where co-translational addition of carbohydrate units is essential for generation of functioning viral glycoproteins. (ii) Cellular carbohydrate structures are parts of the cellular receptor complex at least for some herpes-viruses. (iii) Peripheral carbohydrate structures are engaged in modulation of the immunological properties of herpesvirus glycoproteins. The present review will consider all these th…

[Urogenital Chlamydia trachomatis infections].

Urogenital Chlamydia trachomatis infections are the most common sexually transmitted diseases which have shown a significant increase in the last few years. This organism presents a major health problem in many countries. It is recognized as the cause of both complicated and uncomplicated infections in men, women and infants passing through an infected birth canal. Tissue culture is accepted as the gold standard in the diagnosis of chlamydial infections. But, since this technique is expensive and cumbersome, antigen detection tests such as DFA and EIA are more widely used. Tetracyclines and er…

Complicated urinary tract infections.

Complicated urinary tract infections (UTIs) are defined as those infections causing severe complications, such as urosepsis, renal scarring, and end-stage renal disease. Major complicating factors are infection stones, infected atrophic kidney, renal papillary necrosis, indwelling catheters, and other causes of bacterial persistence, as well as vesicoureteral reflux and urinary tract obstruction in recurrent UTI. Operative therapy is mandatory in all types of persistent UTI besides chronic bacterial prostatitis, as well as in UTI associated with urinary tract obstruction. Recurrent UTI complic…

[Incidence of congenital infections].

The discussions on the pros and cons of obstetric screening for connatal infections have been going on for years. We, therefore, conducted a prevalence study of the most common connatal infections. HIV infection, rubella and syphilis were not subjects of this study. We analysed the relevance of these infections in 512 pregnant women and their newborn infants at the moment of delivery. Further serological tests were run three months post partum, if necessary even for a longer period. Cytomegaly IgG antibodies were found in 46% of the examined women, IgM antibodies in 1.3%. Women under the age o…

[Infections in war injuries].

Between April and September 1991, 415 injured patients were treated at the University Hospital Rebro, Zagreb, 362 at the Department of Surgery and 53 at the Department of Neurosurgery. Infections developed in 15.7% of the injured patients (wound infections in 14.6% and sepsis or meningitis in 1.1% of the injured patients). 88.2% of wound infections as well as all sepsis and meningitis were hospital-acquired infections, while 7.95 of wound infections occurred within 48 h of injuring. The major pathogens, in 90% of cases, were the aerobic bacteria (Enterobacteriaceae, Pseudomonas aeruginosa, Sta…
