Natural Language Processing for Cohort Discovery in a Discharge Prediction Model for the Neonatal ICU.

OBJECTIVES: Discharging patients from the Neonatal Intensive Care Unit (NICU) can be delayed for non-medical reasons including the procurement of home medical equipment, parental education, and the need for children's services. We previously created a model to identify patients that will be medically ready for discharge in the subsequent 2-10 days. In this study we use Natural Language Processing to improve upon that model and discern why the model performed poorly on certain patients. METHODS: We retrospectively examined the text of the Assessment and Plan section from daily progress notes o

Chiral cyclodextrin-modified micellar electrokinetic chromatography and chemometric techniques for green tea samples origin discrimination.

Catechins and methylxanthines were determined in 92 green tea (GT) samples originating from Japan and China by using micellar electrokinetic chromatography with the addition of (2-hydroxypropyl)-β-cyclodextrin. GT samples showed high concentrations of (-)-epigallocatechin gallate and caffeine, with (-)-epigallocatechin, (-)-epicatechin gallate and (-)-epicatechin in relevant content and (+)-catechin, (-)-catechin and theobromine in much lower amounts. The amount of all the considered compounds was higher for Chinese GTs, with the exception of (-)-epicatechin gallate. Pattern recognition method

Implementation of Health Information Technology in Routine Care for Fibromyalgia: Pilot Study.

Fibromyalgia management remains complicated and challenging. Health information technology is an evidence-based, nonpharmacological self and symptom management strategy, but few studies have evaluated its feasibility for managing fibromyalgia patients in clinical practice. FibroGuide is an example of an evidence-based, interactive, and computer-based program comprised of 10 educational modules on fibromyalgia. Study aims were to: (1) develop a process for implementing FibroGuide into the routine care of patients with fibromyalgia, (2) evaluate the overall impact on fibromyalgia before and afte

WISDOM-II: screening against multiple targets implicated in malaria using computational grid infrastructures.

BACKGROUND: Despite continuous efforts of the international community to reduce the impact of malaria on developing countries, no significant progress has been made in the recent years and the discovery of new drugs is more than ever needed. Out of the many proteins involved in the metabolic activities of the Plasmodium parasite, some are promising targets to carry out rational drug discovery. MOTIVATION: Recent years have witnessed the emergence of grids, which are highly distributed computing infrastructures particularly well fitted for embarrassingly parallel computations like docking. A reliable and efficient method for cerebral ventricular volumetry in pediatric neuroimaging.

OBJECTIVES: Cerebral ventricular volume has the potential to become an important parameter in quantitative neurological diagnosis. However, no accepted methodology for routine clinical use exists to date. We sought a robust, reproducible, and fast technique to evaluate cerebral ventricular volume in young children. METHODS: We describe a novel volumetric methodology to segment and visualize intracerebral fluid spaces and to quantify ventricular volumes. The method is based on broadly available T1 weighted volumetric magnetic resonance (MR) imaging, an interactive watershed transform, and a fu

Syndromic surveillance using minimum transfer of identifiable data: the example of the National Bioterrorism Syndromic Surveillance Demonstration Program.

Several health plans and other organizations are collaborating with the Centers for Disease Control and Prevention to develop a syndromic surveillance system with national coverage that includes more than 20 million people. A principal design feature of this system is reliance on daily reporting of counts of individuals with syndromes of interest in specified geographic regions rather than reporting of individual encounter-level information. On request from public health agencies, health plans and telephone triage services provide additional information regarding individuals who are part of ap

Improving parents' early recognition and understanding of infant cranial abnormalities through Web-based 2-D animations of 3-D structures.

A prototype Web site, "HeadStart: a craniosynostosis and positional plagiocephaly resource," was developed to help parents of children with cranial abnormalities access information about their child's condition. An on-line survey of 30 parents confirmed the need for information regarding the early diagnosis and treatment of cranial abnormalities. Three methods of creating the illusion of 3-D in a 2-D interactive animation were investigated. The final prototype Web site was developed using a novel approach to rendering 3-D models for use on the Web using a non-photorealistic rendering technique

Verbal autopsy of 48 000 adult deaths attributable to medical causes in Chennai (formerly Madras), India.

BACKGROUND: In the city of Chennai, India, registration of the fact of death is almost complete but the cause of death is often inadequately recorded on the death certificate. A special verbal autopsy (VA) study of 48 000 adult deaths in Chennai during 1995-97 was conducted to arrive at the probable underlying cause of death and to measure cause specific mortality rates for Chennai. METHODS: Trained non-medical graduates with at least 15 years of formal education interviewed the surviving family members or an associate of the deceased to write a report on the complaints, symptoms, signs, dura

Patient preferences for visualization of longitudinal patient-reported outcomes data.

OBJECTIVE: The study sought to design symptom reports of longitudinal patient-reported outcomes data that are understandable and meaningful to end users. MATERIALS AND METHODS: We completed a 2-phase iterative design and evaluation process. In phase I, we developed symptom reports and refined them according to expert input. End users then completed a survey containing demographics, a measure of health literacy, and items to assess visualization preferences and comprehension of reports. We then collected participants' perspectives on reports through semistructured interviews and modified them

Usability testing of Avoiding Diabetes Thru Action Plan Targeting (ADAPT) decision support for integrating care-based counseling of pre-diabetes in an electronic health record.

PURPOSE: Usability testing can be used to evaluate human-computer interaction (HCI) and communication in shared decision making (SDM) for patient-provider behavioral change and behavioral contracting. Traditional evaluations of usability using scripted or mock patient scenarios with think-aloud protocol analysis provide a way to identify HCI issues. In this paper we describe the application of these methods in the evaluation of the Avoiding Diabetes Thru Action Plan Targeting (ADAPT) tool, and test the usability of the tool to support the ADAPT framework for integrated care counseling of pre-d

A 'web app' for diagnosing hemianopia.

BACKGROUND: Hemianopia commonly complicates stroke and, less frequently, head injury and brain tumours. Patients' activities of daily living are often affected although these can be ameliorated by appropriate behavioural therapy. Identifying a field defect is the first step in the rehabilitation process. An online visual field test (an 'app') was developed as part of a free to use web based therapy site for patients with hemianopic alexia, called Read-Right (http://www.readright.ucl.ac.uk). This study is an attempt to validate this test by comparing with a clinical 'gold standard'-the Humphrey

A bivalent multi-epitope vaccine targeting CSP and TRAP of Plasmodium vivax designed through immunoinformatics and structural bioinformatics.

Malaria associated with Plasmodium vivax is still one of the main public health concerns due to relapse-associated infections. Moreover, the absence of a broadly effective licensed vaccine makes the situation more alarming. In this scenario, epitope-based vaccine design helps to improve safety, immunogenicity, and population coverage. The integrated workflow of immune informatics and structural bioinformatics was used to establish a bivalent multi-epitope vaccine that targets pre-erythrocytic antigens, circumsporozoite protein (CSP), and thrombospondin-related adhesive protein (TRAP). Antigeni…

CircadiOmics: bioinformatics platform for circadian rhythms.

MOTIVATION: Circadian rhythms are fundamental to biology: they occur at the molecular level across all species and are critical to homeostasis and biological functions. They are also essential for health and medicine, as disruption of circadian regulation has been linked to numerous health problems, and many drug targets exhibit circadian oscillations, highlighting the importance of molecular-level circadian insight for precision medicine. Despite the rapid growth of circadian omic time series datasets, informatics tools for processing and analyzing this growing wealth of data remain limited. …
