Dapsone-induced agranulocytosis is a rare but potentially fatal adverse effect of treatment for tuberculoid leprosy. Publications distributed by the WHO Leprosy Elimination Campaign for patient information on leprosy do not contain any advice or guidelines for post-dapsone therapy follow-up. As a result of this major deficiency, the local anti-leprosy campaign in Sri Lanka has no such guidelines on dapsone therapy for leprosy patients. This prospective cohort study examined the health-seeking behaviour of patients diagnosed with genital warts and vulvodynia who presented to Manly Sexual Health Service from March to June 2000. A self-administered questionnaire was used to assess patients' perceptions of their condition and their use of complementary therapies. We report a case of a 31-year-old healthy man with marathon-induced pulmonary edema. Chest radiograph revealed pulmonary edema without cardiomegaly. Contrast-enhanced chest computed tomography (CT) revealed transient pulmonary edema without filling-defect in pulmonary arteries. As marathon running increases in popularity, radiologists and emergency physicians should be familiar with diagnosis of this entity on chest radiograph, avoiding unnecessary CT examination without additional clinical information.

The fact that the incidence of infertile marriages is on the increase throughout the world has stimulated a greater interest in identifying possible effects of the environment on the fertility of both man and woman. Over the last 50 years, the average number of spermatozoa in the ejaculate has decreased by about a half. The present review takes a look at recognized noxae and their effect on fertility, with the aim of providing clinically active physicians with up-to-date information about the major noxious substances, thus enabling them to be avoided.

A case report of a mola destruens is presented including the clinical monitoring and the therapeutical conclusions. Control parameters were the HCG, ultrasound and DSA. The DSA in a new and improved technique, provided helpful information during a critical phase of the disease, when changing from mono- to polychemotherapy was necessary. Furthermore, the excellent vascular mapping enables planning of radiological interventions. In case of tumour bleeding hysterectomy can be avoided by embolisation. There may thus be more time for conservative treatment by chemotherapy.

Concurrent alcohol and opioid withdrawal syndrome is a common and challenging clinical scenario with little published evidence or guidance to inform pharmacotherapy strategies. Concurrent use of benzodiazepines and opioid agonists, which are considered first-line agents for management of each withdrawal syndrome independently, is controversial and often avoided in clinical practice. Strategies to provide effective, simultaneous medication treatment of alcohol and opioid withdrawal while optimizing patient safety are demonstrated through 3 patient cases.

When a patient presents with spine problems, the spine surgeon would do well to avoid use of, reliance on, and acceptance of radiographs as the sole or primary source of information. Measurement of pelvic incidence and lumbar lordosis, although crucial, does not take into account the effort the patient must make to move, the level of involvement of other parts of the body, and the history of previous procedures and outcomes. Radiographs may show pathology that is not consistent with the appearance of the patient. How should we assess this situation?

One of the most significant changes to the section on Feeding and Eating Disorders in the DSM-5 was that feeding disorder of infancy or early childhood was renamed avoidant/restrictive food intake disorder or ARFID, and the criteria was significantly expanded. This diagnostic category was informed by field studies, analysis, and expert opinion. To date, research exploring the prevalence, clinical characteristics and utility have helped substantiate the recognition and need for the diagnostic category of AFRID as a distinct eating disorder diagnosis in the DSM-5.

Certain skin characteristics, such as altered vascular reactivity, appear to be common among patients with rosacea. This may partly explain the observation that these patients appear to have increased sensitivity to certain components of commonly used topical agents. Accordingly, patients with rosacea should be educated regarding which general skin care products to use and to avoid. This review summarizes information regarding 3 classes of these products--cleansers, moisturizers, and photoprotectants--with emphasis on barrier function and skin irritation.

Aglomerular segmental hypoplasia of the kidney is a disease that is uncommon in the adult; the average age at appearance of symptoms is between ten and 15 years. A clear female predominance has been noted, and this disease is often misdiagnosed as chronic pyelonephritis. However, the radiological and pathological characteristics are different to the latter. The etiology of aglomerular segmental hypoplasia has not yet been determined. It is important to acquire further knowledge regarding this disease, as in certain cases this information could help to avoid unnecessary nephrectomy.

A description is given of a pair of iris retractors which avoid certain shortcomings of other instruments or of other methods of retracting the iris in connection with cryo-extraction, particularly in case where the pupil cannot be dilated or only insufficiently. This pair of iris retractors has proved useful. In addition, information is given about our own technique to achieve a round functional pupil, after complicated lens-extractions with vitreous prolapse, by peripheral iridotomy parallel to the limbus and discission of the body.
