Clinical Significance of Ventricular Tachyarrhythmias in Patients Undergoing Valve Replacement: A Nationwide Population-Based Study.

Background: The clinical significance and outcomes of ventricular tachyarrhythmias (VTa) in patients undergoing valve replacement have rarely been reported. Objective: This study aimed to investigate the incidence and outcome of VTa after surgical valve replacement. Methods: We conducted a population-based retrospective cohort study using data obtained from the Taiwan National Health Insurance Research Database. In total, 10,212 patients were selected after 1:1 propensity-score matching based on the type of prosthetic valve used (mechanical vs. bioprosthetic). Various outcomes during long-term

The TRIVIA Cohort for Surgical Management of Tetralogy of Fallot: Merging Population and Clinical Data for Real-World Scientific Evidence.

BACKGROUND: Guidelines for surgical management of tetralogy of Fallot (TOF) are often based on low-quality evidence due to the many challenges of congenital heart disease: heterogeneous cardiac anatomy, consequences from surgical interventions arising years later, and scarcity of hard outcomes. The overarching goal of the Tetralogy of Fallot Research for Improvement of Valve replacement Intervention: A Bridge Across the Knowledge Gap (TRIVIA) study is to evaluate the long-term impact of the surgical management strategies in TOF. The specific objectives are: (1) to describe the long-term outcom

Peer influence on physicians in adopting opportunistic salpingectomy at the time of hysterectomy.

BACKGROUND: Since professional societies recommended counseling patients about opportunistic salpingectomy for ovarian cancer risk reduction, use of opportunistic salpingectomy has increased overall. However, physicians varied in their adoption of this new cancer prevention strategy. OBJECTIVE: To examine peer influence among physicians as a possible factor affecting their adoption of opportunistic salpingectomy at the time of hysterectomy. STUDY DESIGN: Using insurance claims data from the Blue Cross Blue Shield Axis database across the United States, we identified female patients aged 18 t

Socioeconomic and Racial Disparities in Surgical Management of Adolescent Idiopathic Scoliosis: Vertebral Body Tethering Versus Posterior Spinal Fusion.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) affects 2% to 3% of children aged 10 to 18, and progressive cases often require surgery. Posterior spinal fusion (PSF) remains the standard treatment but restricts mobility, while vertebral body tethering (VBT) offers a motion-preserving alternative. However, VBT is costly, concentrated in specialized centers, and inconsistently reimbursed, raising concerns that access is limited to higher socioeconomic status (SES) populations. This study examined patterns of VBT utilization compared with PSF across SES and hospital characteristics. METHODS:

[Gender correction surgery in transsexualism from a legal perspective].

As understood by the German "Transsexuellengesetz" (Gender Recognition Act) transsexualism is to be recognized as a gender identity disorder if a long-term diagnostic-therapeutic process has shown that the transposition of gender roles is irreversible and if, besides psychotherapeutic monitoring, only hormonal and surgical procedures are suitable to relieve the patient's distress related to his gender identity. The German Gender Recognition Act does not contain provisions regarding the diagnosis or medical treatment of transsexualism. Commissioned by the German Sexuality Research Association,

[PPI very long-term treatment: is the problem only economic?].

Proton pump inhibitors are among the most prescribed drugs in Belgium, due to the high prevalence of gastrooesophageal reflux disease and significant impairment of health-related well-being. Short-term and long-term efficacy is superior to that of H2-receptor antagonists. Long-term safety is established. Empirical short-term treatment of symptomatic patients under 50 years of age without alarm signs is acceptable, but endoscopic investigation should always be performed before starting a long-term treatment. Doses for management of low-grade oesophagitis may be tailored to the patient's respons

The role of epilepsy surgery in southern Africa.

Epilepsy surgery plays an important role in the management of patients with medically refractory temporal lobe epilepsy and lesional epilepsies. When performed early in the course of the illness, surgery can render many patients seizure-free and greatly improve their quality of life. In southern Africa, as in many developing nations, thousands of patients could benefit from epilepsy surgery, but inequalities in healthcare provision mean that ready access to surgical facilities is largely restricted to those with private healthcare insurance. Over the past 5 years, more than 250 patients have h

Isolation perfusion for malignant melanoma of the extremity: a review.

The role of perfusion, normothermic and hyperthermic, in the curative treatment of melanoma remains controversial. Survival appears to be somewhat improved over that of surgery alone in all stages, especially with hyperthermic perfusion, but all comparisons have been retrospective and uncontrolled. In addition to the usual problems with historical controls, melanoma presents its own special problems because of its unpredictable natural history in any given individual and the multiplicity of factors known to affect prognosis. It is unfortunate that hundreds of patients have been treated in unco

Creating a Surgical Biobank: The Hershey Medical Center Experience.

BACKGROUND: Tissue harvesting at the time of surgery offers surgeons and scientists a unique opportunity to discover and better understand disease pathophysiology. Tissue biobanking presents challenges in patient consents, specimen collection, preparation, and storage, but the potential for scientific discovery justifies the effort. Although the number of tissue biobanks is increasing worldwide, information regarding necessary infrastructure, process flow, and management of expected obstacles is lacking. OBJECTIVE: To provide a framework and motivation for clinician scientists intending to st

[Preoperative management for malnourished patients in abdominal surgery. Practical treatment regimen for reduction of perioperative morbidity].

The incidence of malnutrition in oncological and visceral surgical units can be high. The screening of malnourished patients is very important, especially in the preoperative setting. The available published literature provides crucial knowledge about the risks inherent to malnutrition and subsequent perioperative morbidity and mortality. The preoperative screening of malnourished patients followed by a subsequent renutrition is the key to decreasing rates of postoperative morbidity and mortality. The data and guidelines given by the European Society of Parenteral and Enteral Nutrition (ESPEN)

Impact of social factors on outcomes following pediatric neuro-oncology surgery in the United States: a systematic review and meta-analysis.

To use AI-assisted software to identify gender, racial, and socioeconomic differences in post-operative outcomes following pediatric neuro-oncology surgery. PubMed, Scopus, Web of Science, and Ovid were queried using a semi-automated software. Meta-analyses quantified pooled odds ratios for overall survival (OS), gross total resection (GTR), recurrence, 30-day readmission, medical complications (neurologic and endocrinologic), and surgical complications. Where possible, subanalyses were performed based on study type and cranial tumor location. Sixty-seven studies were included. Across all stud

Addressing Barriers to Care in Odontogenic Infections: The Impact of Timely Surgical Intervention on Reducing Hospital Readmissions in Vulnerable Populations.

Odontogenic infections are a leading cause of deep neck infections, with their incidence expected to rise due to the prevalence of contributing risk factors, such as uncontrolled diabetes mellitus and obesity. While the existing literature primarily focuses on the management of odontogenic infections in admitted patients, this study uniquely assesses clinical outcomes across all care settings and among a predominantly low socioeconomic status (SES) patient population. These data showed that definitive surgical source control (SC) was associated with an 88% reduction in the relative risk of 30-

Delayed Tricep Repair Using Dermal Allograft: Technique and Case Examples.

An acellular dermal matrix is a biological graft composed of several components present in the dermis such as collagen fibers, elastin, fibronectin, and hyaluronic acid that serve as support for cellular repopulation that will gradually become vascularized. This provides mechanical resistance and improves suture retention. With this article we aim to present 2 cases, and our technique, in which a rupture of the distal triceps tendon occurred, patients presented late due to usual insurance authorization delays, and then required delayed surgical repair by means of dermal acellular matrix grafti
