Patient-ventilator synchrony during pressure-targeted versus flow-targeted small tidal volume assisted ventilation.

PURPOSE: Low tidal volume (V(T)) delivered by flow-targeted breaths reduces ventilator-induced lung injury but may increase patient breathing effort because of limited flow. We hypothesized that a variable-flow, pressure-targeted breath would improve breathing effort versus a fixed flow-targeted breath. MATERIALS AND METHODS: We compared pressure assist-control ventilation and volume assist-control ventilation (VACV) in 12 patients with acute respiratory failure receiving 6 to 8 mL/kg V(T). Backup frequency, V(T), inspiratory time, applied positive end-expiratory pressure and fraction of insp

Barotrauma not related to type of positive pressure ventilation during severe asthma exacerbations in children.

BACKGROUND: Children with impending respiratory failure due to severe asthma may be treated with endotracheal intubation and mechanical ventilation. Barotrauma occurs in a significant number of these children. Non-invasive positive pressure ventilation (NPPV) has been used as an alternative intermediary therapy and potentially prevents intubation. However, the comparative risk of barotrauma associated with the use of NPPV has not been evaluated in this population. OBJECTIVE: To determine if the mode of positive pressure delivery per se affects the likelihood of development of barotrauma. MET

Spectrum of oxygen dependency in surviving infants weighing 600 to 1000 grams: decreased incidence of severe chronic lung disease.

Incidence and factors predisposing to chronic lung disease (CLD) were studied in a group of 110 infants still alive at the age of 1 year (69.2%) of an initial group of 159 infants born with a birthweight between 600 and 1000 gm from 1983 to 1988. Low peak inspiratory pressure (PIP), high initial ventilatory rates, slow weaning from respirator and partial arterial oxygen pressure and carbon dioxide pressure values in the range of 40 to 55 torr were used. Oxygen dependency had a bimodal distribution with a high number of infants needing oxygen supplement on day 1 (75.9%) and on day 28 (69.1%) co

The Effect of a Pediatric Intensive Care Severity-Tiered Pathway for Status Asthmaticus on Quality Measures and Outcomes.

Background: Patients with status asthmaticus admitted to the hospital contribute tremendous costs to the healthcare system. Treatment protocols directed at improving care to pediatric inpatients have improved both clinical and financial outcomes; however, there are limited data demonstrating the impact of goal-driven therapy for patients with status asthmaticus in the pediatric intensive care unit (PICU). The purpose of this initiative was to standardize care of children with status asthmaticus in the PICU to improve clinical outcomes. Materials and Methods: A goal-directed clinical pathway us

Invasive versus non-invasive ventilation for acute respiratory failure in neuromuscular disease and chest wall disorders.

BACKGROUND: Acute respiratory failure is a common life-threatening complication of acute onset neuromuscular diseases, and may exacerbate chronic hypoventilation in patients with neuromuscular disease or chest wall disorders. Standard management includes oxygen supplementation, physiotherapy, cough assistance, and, whenever needed, antibiotics and intermittent positive pressure ventilation. Non-invasive mechanical ventilation (NIV) via nasal, buccal or full-face devices has become routine practice in many centres. OBJECTIVES: The primary objective of this review was to compare the efficacy of

Application of active subglottic airway humidification in intermittent subglottic secretion drainage in patients with tracheostomy: a randomized controlled trial.

BACKGROUND: Intermittent aspiration of subglottic secretions (IASS) is commonly used to alleviate retention on the cuff, but inappropriate subglottic secretion drainage may lead to adverse effects, and fewer studies have been conducted on active subglottic airway humidification to improve the safety and effectiveness of IASS. METHODS: A randomized controlled trial was conducted from August 2023 to July 2024, involving 90 patients with flushable tracheostomy tubes: 48 patients received 30-min pre-drainage active humidification (intervention group), while 42 patients received sterile water flus

Congenital Diaphragmatic Hernia With Poor Clinical Outcome: Key Lessons To Be Learned.

Congenital diaphragmatic hernia (CDH) refers to the abnormal protrusion of abdominal contents (stomach, intestine) into the thoracic cavity, leading to the underdevelopment of the lungs (pulmonary hypoplasia). It is a critical neonatal condition that presents significant challenges in both diagnosis and management, especially in resource-limited countries. This case report describes a term female baby born via normal vaginal delivery to a mother with no prenatal care. The baby showed no respiratory effort and required resuscitation. Despite intubation, the baby's air entry remained poor, and s

The Effect of Intermittent Pneumatic Compression on Hemodynamics and Regional Cerebral Oxygen Saturation in Laparoscopic Bariatric Surgery with Mild Hypercapnia in the Steep Reverse Trendelenburg Position.

Obesity negatively affects hemodynamics and cerebral physiology. We investigated the effect of the utilization of an intermittent pneumatic compression (IPC) device on hemodynamics and cerebral physiology in patients undergoing laparoscopic bariatric surgery under general anesthesia with lung-protective ventilation. Sixty-four patients (body mass index > 30 kg/m2) were randomly assigned to groups that received an IPC device (IPC group, n = 32) and did not (control group, n = 32). The mean arterial pressure (MAP), heart rate (HR), need for vasopressors, cerebral oxygen saturation (rSO2), and ce

Reintubation rates after extubation to different non-invasive ventilation modes in preterm infants.

INTRODUCTION: Respiratory Distress Syndrome (RDS) is a common cause of neonatal morbidity and mortality in premature newborns. In this study, we aim to compare the reintubation rate in preterm babies with RDS who were extubated to Nasal Continuous Positive Airway Pressure (NCPAP) versus those extubated to Nasal Intermittent Positive Pressure Ventilation (NIPPV). METHODS: This is a retrospective study conducted in the Neonatal Intensive Care Unit (NICU) of Women's Wellness and Research Center (WWRC), Doha, Qatar. The medical files (n = 220) of ventilated preterm infants with gestational age ra

Methylxanthine treatment for apnea in preterm infants.

BACKGROUND: Recurrent apnea is common in preterm infants, particularly at very early gestational ages. These episodes of loss of effective breathing can lead to hypoxemia and bradycardia which may be severe enough to require resuscitation including use of positive pressure ventilation. Methylxanthines have been used to stimulate breathing and prevent apnea and its consequences. OBJECTIVES: The objective of this review is to determine if methylxanthine treatment in preterm infants with recurrent apnea leads to a clinically important reduction in apnea and use of intermittent positive pressure

Diaphragmatic ultrasound and thoracic fluid content for prediction of non-invasive ventilation failure in neonates: a randomized controlled trial.

UNLABELLED: Non-invasive ventilation (NIV) remains the standard of care for preterm infants with respiratory distress syndrome (RDS); however, NIV failure is frequent and often linked to adverse outcomes. This study evaluated diaphragmatic ultrasound and thoracic fluid content (TFC) as early predictors of NIV failure in preterm neonates. In this prospective randomized controlled trial, 90 preterm neonates (28-34 gestational weeks) with RDS requiring NIV as initial respiratory support were randomized into three groups: (1) nasal continuous positive airway pressure (nCPAP), (2) nasal intermitten

Randomized trial examining cerebral haemodynamics following artificial or animal surfactant.

To determine the effects of animal and artificial surfactants on cerebral haemodynamics, 20 premature babies receiving mechanical ventilation were randomized to receive Curosurf or Exosurf surfactant. Anterior cerebral artery blood flow velocity (CABFV) was measured using Doppler ultrasound before and up to 2 h after treatment. Following animal surfactant there was a rapid reduction in CABFV (median -36%, range -43% to +8%, p < 0.01), whereas artificial surfactant resulted in a slower rise which was less marked (median +20%, range -7% to +62%, p < 0.05). There were no significant changes in bl

[Acute gastric distension necessitating gastrostomy after anesthetic induction for surgical correction of type III esophageal atresia].

IPPV during anaesthesia for management of oesophageal atresia with tracheo-oesophageal fistula (TOF) can cause gastric insufflation. We report such a complication in a one-day-old newborn, who developed, 15 min after induction, a distension of the abdomen, hypoxia and bracdycardia. An emergency gastrostomy was performed. His status improved rapidly and surgery could be completed. TOF was located at the carina and had a large calibre. To avoid gastric distension in such cases, the tip of the tube is located just proximal to the carina, but distal to the fistula to prevent intubation of the latt
