The role of intraabdominal drain placement in minimal-invasive right hemicolectomy with complete mesocolic excision - a propensity score matched single center analysis.

BACKGROUND: The role of intraabdominal drains in minimally invasive right hemicolectomy with complete mesocolic excision (CME) remains controversial. This study evaluates the impact of drain placement on perioperative outcomes using a propensity score-matched analysis in a single-center cohort. METHODS: Data from 185 patients who underwent minimally invasive right hemicolectomy with complete mesocolic excision and central vascular ligation at our institution from 2016 to November 2024 were analyzed, including 62 without drains and 123 with drains. After propensity score matching, 50 patients

The degree of local inflammatory response after colonic resection depends on the surgical approach: an observational study in 61 patients.

BACKGROUND: Clinical data indicate that laparoscopic surgery reduces postoperative inflammatory response and benefits patient recovery. Little is known about the mechanisms involved in reduced systemic and local inflammation and the contribution of reduced trauma to the abdominal wall and the parietal peritoneum. METHODS: Included were 61 patients, who underwent elective colorectal resection without intraabdominal complications; 17 received a completely laparoscopic, 13 a laparoscopically- assisted procedure and 31 open surgery. Local inflammatory response was quantified by measurement of int

[Sense or nonsense of a prophylactic drainage after laparoscopic colorectal surgery - a prospective study].

INTRODUCTION: A growing number of studies do not show an advantage of prophylactic drainage in intraabdominal surgery any more. Especially against the background of "fast-track" surgery, this study aimed at an analysis of the influence of drainage on the patient's outcome in elective laparoscopic colorectal surgery. METHOD: Within a 50-month period, 569 laparoscopic colorectal operations were carried out at the Klinikum Bremerhaven Reinkenheide, a centre for minimally invasive surgery. Of these, 505 patients were operated by one surgeon. For this prospective study, the data of 299 elective co

Comparative effectiveness of ertapenem versus other empirical antibiotics in elderly patients with complicated intra-abdominal infection: a real-world inverse probability of treatment weighting study.

BACKGROUND: Elderly adults with complicated intra-abdominal infection (cIAI) represent a functionally immunocompromised population due to immunosenescence, multimorbidity, and frailty. Optimizing empirical antibiotic therapy in this group is essential to improve outcomes while minimizing unnecessary broad-spectrum antimicrobial exposure and antimicrobial resistance (AMR) selection pressure. Ertapenem is a once-daily carbapenem with favorable pharmacological properties, yet contemporary real-world comparative data in older adults are limited. METHODS: We conducted a retrospective, real-world c

Isolation and Epidemiology of Respiratory Bacteria From Unusual Sites: Demographic, Seasonal, and Geographic Distribution in a Laboratory-Based Study From a Tertiary Care Hospital in Karachi, Pakistan.

Introduction Respiratory pathogens such as Streptococcus pneumoniae, Haemophilus influenzae, and Haemophilus parainfluenzae primarily infect respiratory and oropharyngeal sites. However, they are increasingly being isolated from unusual anatomic sites, such as urogenital and abdominal sources. These atypical occurrences necessitate an understanding of their epidemiology and clinical implications. Methods This study aimed to evaluate the seasonal prevalence, gender distribution, and clinical characteristics of Streptococcus pneumoniae, Haemophilus influenzae, and Haemophilus parainfluenzae isol

Discrepancy between genotypic and phenotypic extended-spectrum β-lactamase rates in Escherichia coli from intra-abdominal infections in the USA.

Varying rates of false-positive results of phenotypic extended-spectrum β-lactamase (ESBL) tests have been reported for different methods in different settings, species and geographic locations. This report describes discrepancies in Escherichia coli genotypic and phenotypic ESBL rates observed in a surveillance study of 29 US hospitals that participated in the Study for Monitoring Antimicrobial Resistance Trends (SMART). The ESBL phenotype was determined with the Clinical and Laboratory Standards Institute confirmatory broth microdilution test using cefotaxime and ceftazidime with and without

Antibiotic Timing and Progression to Septic Shock Among Patients in the ED With Suspected Infection.

BACKGROUND: Recent medical society opinions have questioned the use of early antimicrobials in patients with sepsis, but without septic shock. RESEARCH QUESTION: Is time from ED presentation to administration of antibiotics associated with progression to septic shock among patients with suspected infection? STUDY DESIGN AND METHODS: This was a retrospective cohort study from March 2007 through March 2020. All adults with suspected infection and first antimicrobial administered within 24 h of triage were included. Patients with shock on presentation were excluded. We performed univariate and

Laparoscopic versus open appendectomy for complicated appendicitis.

BACKGROUND: Laparoscopic appendectomy has been widely practiced for uncomplicated appendicitis; various reports demonstrated its merits in assisting diagnosis, reducing postoperative pain, analgesic requirement, and incidence of wound infection. The role of laparoscopy in management of complicated appendicitis, ie, gangrenous, perforated appendicitis and appendiceal abscess, remains undefined. Currently, the choice of operative approach is mostly at the surgeons' discretion. A retrospective study was conducted in our institution to review the feasibility, safety, and efficacy of laparoscopic a

Fluconazole versus micafungin for initial antifungal prophylaxis against Candida in pancreas transplant recipients: A comparative study of two consecutive periods.

BACKGROUND: Invasive fungal infection, particularly intraabdominal candidiasis, exerts a negative impact on the outcome of pancreas transplant recipients (PTRs). Optimal antifungal prophylaxis in this context remains unclear. METHODS: We performed a single-centre retrospective study to compare the incidence of invasive candidiasis during the first 6 post-transplant months in a cohort of 218 PTRs over two periods in which different agents for antifungal prophylaxis were used: fluconazole (Fluco-Px) from March 1995 to June 2012, and micafungin followed by fluconazole (Mica-Px) from July 2012 to

Prevalence, Microbiological Cultures, and Outcomes of Intra-abdominal Sepsis in Intensive Care Unit Trauma Patients at Charlotte Maxeke Johannesburg Academic Hospital.

INTRODUCTION: Intra-abdominal infections following trauma-related relook laparotomies are associated with significant morbidity and mortality, especially in low- and middle-income countries where interpersonal violence drives high rates of penetrating injuries. Managing these infections effectively requires understanding pathogen profiles and antibiotic resistance patterns. This study aimed to identify the microorganisms present in trauma-related relook laparotomies, evaluate their antibiotic sensitivity, and assess the outcome of intra-abdominal infections and sepsis on 28-d mortality and hos

Imipenem-Relebactam and Meropenem-Vaborbactam: Two Novel Carbapenem-β-Lactamase Inhibitor Combinations.

Relebactam (formerly known as MK-7655) is a non-β-lactam, bicyclic diazabicyclooctane, β-lactamase inhibitor that is structurally related to avibactam, differing by the addition of a piperidine ring to the 2-position carbonyl group. Vaborbactam (formerly known as RPX7009) is a non-β-lactam, cyclic, boronic acid-based, β-lactamase inhibitor. The structure of vaborbactam is unlike any other currently marketed β-lactamase inhibitor. Both inhibitors display activity against Ambler class A [including extended-spectrum β-lactamases (ESBLs), Klebsiella pneumoniae carbapenemases (KPCs)] and class C β-
