Anesthetic management for transoral robotic surgery.

PURPOSE OF REVIEW: The incidence of oropharyngeal cancer is increasing. Traditional treatments, including major open surgical resection, chemotherapy, and radiotherapy, are associated with significant patient morbidity - affecting the cosmesis, structure, and function of the head and neck. Transoral robotic surgery (TORS) is a minimally invasive technique that maximizes surgical visualization, accessibility, and maneuverability, avoiding transfacial or transmandibular access procedures (often involving tracheostomy and gastrostomy insertion), potentially resulting in improved long-term patient

Gallbladder volvulus in an elderly patient presenting as acute calculous cholecystitis: a case report.

INTRODUCTION AND IMPORTANCE: Gallbladder volvulus is a rare, life-threatening emergency resulting from torsion of the gallbladder on its vascular pedicle. Its rarity and clinical similarity to acute cholecystitis make diagnosis difficult. This is the first documented case in the Philippines, highlighting its significance for local surgical practice. PRESENTATION OF CASE: A 74-year-old woman presented with acute right upper quadrant pain, nausea, vomiting, and bloating. Laboratory workup revealed leukocytosis with neutrophilia, and imaging suggested acute calculous cholecystitis. At surgery, c

Urine leak after robotic radical prostatectomy: not all urine leaks come from the anastomosis.

Radical prostatectomy is the gold standard in patients that are surgical candidates with localized prostate cancer. While most postoperative urine leaks are from vesico-urethral anastomosis, urologists must be aware that a small percentage of patients may have a urine leak from other sites that may have been inadvertently injured during the procedure. We propose a systematic workup to evaluate the source of the urinary leak as well as appropriate management of such injuries. The mid-ureter can be injured during lymph node dissection. The distal ureter is at risk of injury when performing the M

Scalpel Sign in Spine Pathology: Presentation in 3 Different Rare Diagnoses.

BACKGROUND: The scalpel sign is a radiological finding observed on sagittal magnetic resonance imaging and computed tomography myelography corresponding to an indentation in the dorsal aspect of the spinal cord resembling a surgical scalpel blade. It is said to be a pathognomonic imaging discovery linked to dorsal arachnoid webs. However, other spine-related conditions may mimic dorsal arachnoid webs on magnetic resonance imaging, such as spinal arachnoid cysts or ventral spinal cord herniation, leading to misdiagnosis. METHODS: A retrospective review was performed of cases involving 3 differ

Pneumomediastinum and pneumopericardium 11 days after Whipple procedure. A case report and review if the literature.

INTRODUCTION: Spontaneous pneumomediastinum (SPM) is a rare condition classified as free air in the mediastinum in the absence of any precipitating cause. This is the first time that a synchronous presence of pneumopericardium is described. To date, there are no clear guidelines for diagnosis and treatment. PRESENTATION OF A CASE: A 34-year old Caucasian male patient presented to our institution with a recently diagnosed pancreatic adenocarcinoma. He underwent an uneventful pancreaticoduodenectomy (Whipple procedure). Preoperative and intraoperative chest X-Ray after a central line placement

An Unusual Cause of Vertebrobasilar Insufficiency in a Case of Atlantoaxial Dislocation with Anomalous Vertebral Artery.

BACKGROUND: Posterior circulation stroke resulting from atlantoaxial dislocation (AAD), although uncommon, is a well-described entity. The normally coursed V3 segment of the vertebral artery (VA) is likely to be stretched because of C1-C2 dislocation and further compromised by the C1-C2 translational mobility. The persistent first intersegmental artery (PFIA), an anomalous variant does not course through the C1 transverse foramen, but rather crosses the posterior C1-C2 joints and is unlikely to be affected by the C1-C2 dislocation. Therefore, a patient with AAD and anomalous VA presenting with

A case of Jackhammer esophagus caused by eosinophilic esophagitis in which per-oral endoscopic myotomy resulted in symptom improvement.

A 73-year-old female with a 6-month history of progressive dysphagia and chest pain was referred to our hospital. She underwent esophagogastroduodenoscopy, which revealed abnormally strong contractions in the distal esophagus. Esophageal biopsy specimens showed massive eosinophil infiltration into the epithelium, and high-resolution manometry (HRM) also demonstrated abnormally strong contractions in the distal esophagus. Based on these results, she was diagnosed with Jackhammer esophagus (JHE) due to eosinophilic esophagitis (EoE). Treatment was started with 5 mg/day of prednisolone (PSL), and

Congenital absence of lumbosacral articular facet joint associated with conjoined nerve root: a case report.

We report a rare case of congenital absence of the L5-S1 facet joint, which was associated with a conjoined nerve root. Combination of these two anomalies has been quite rarely reported in the literature. A 39-year-old man presented with acute low back pain and right leg radiating pain. Muscle weakness and sensory disturbance of the right leg were also apparent in the region innervated by L5 and S1 nerve roots. Preoperative multidetector three-dimensional computed tomography (3D-CT) showed complete absence of the right S1 superior articular process. Magnetic resonance (MR) images showed lumbar

Balancing hemostasis and function: the impact of hemostatic materials on voice and swallowing outcomes in nerve-preserved thyroidectomy.

OBJECTIVE: Post-thyroidectomy dysphonia may occur even when recurrent laryngeal nerve (RLN) and external branch of the superior laryngeal nerve (EBSLN) integrity are preserved. Hemostatic materials (HMs) are widely used to enhance intraoperative hemostasis and reduce postoperative drainage; however, their use may introduce a functional trade-off between bleeding control and postoperative voice and swallowing outcomes. This study aimed to evaluate how different HMs balance hemostatic efficacy and functional preservation in nerve-preserved total thyroidectomy. METHODS: This retrospective study

Uterine Leiomyosarcoma Incidentally Diagnosed After Sigmoid Colon Perforation: A Case Report and Review of the Literature Highlighting Individualized Surgical and Oncologic Decision-Making.

Introduction: Uterine leiomyosarcoma (uLMS) is a rare, highly aggressive malignancy arising from the smooth muscle tissue of the uterine wall. It typically presents with abnormal vaginal bleeding, pelvic pain, or a pelvic mass. In rare instances, symptoms may result from local invasion or metastasis. We report a case of uLMS initially diagnosed following colonic perforation due to direct tumor invasion, accompanied by a comprehensive narrative review of the literature on the incidental identification of uterine sarcomas during emergency general surgery to further emphasize the diagnostic chall

Acute airway obstruction requiring nasotracheal intubation following hypoglossal neuromonitoring: a case report.

BACKGROUND: Intraoperative neurophysiological monitoring (IONM) is utilized for both the localization of critical structures and for real time detection and prevention of intraoperative neurological injury. Use of IONM to monitor the hypoglossal nerve is performed during neurosurgical, otolaryngological, and vascular procedures to improve surgical outcomes. There is a paucity of literature describing potential complications of IONM of the hypoglossal nerve, especially with respect to airway compromise. Here we present our findings regarding a case of acute airway obstruction following hypoglos

The NHS carbon footprint of paediatric tonsillectomies.

BACKGROUND: Single-use waste is prevalent in many surgical procedures; it is also a contributor to carbon emissions released during surgery. With high-frequency procedures such as coblation tonsillectomy and adenotonsillectomy it is important that we promote and implement sustainable practices going forward and explore ways of reducing single-use waste. We aimed to investigate the carbon footprint produced postoperatively by the waste and management of the waste from single-use, nonbiosafe, nonrecyclable items used in paediatric coblation tonsillectomy and adenotonsillectomy, hoping to raise a

Inadvertent parathyroidectomy in thyroid surgery with and without near-infrared autofluorescence: a two-centre retrospective observational study.

PURPOSE: Inadvertent parathyroidectomy is a recognised complication of thyroid surgery and contributes to post-operative hypoparathyroidism. Adjuncts such as near-infrared autofluorescence imaging may aid parathyroid identification, but multi-centre evidence remains limited. This study evaluated rates of inadvertent parathyroidectomy in thyroid surgery performed with and without autofluorescence, and examined the impact of central neck dissection, participation in a fluorescence-guided surgery trial, and temporal changes in practice. METHODS: This two-centre retrospective cohort study included
