Chronic lead poisoning: a "forgotten" cause of renal disease.

Chronic lead nephropathy occurs as a result of years of lead exposure. Nowadays, with the induction of high standards for industrial hygiene, symptomatic lead intoxication has become extremely rare. We report a case of chronic lead nephropathy in a 59-year-old man who worked in a battery-recycling unit and was diagnosed with plumbism during a regular health screening few years ago. The diagnosis was suggested by the following findings: serum creatinine 160 microg/L, creatinine clearance 46 ml/min, daily urine protein excretion 0.1 g, uric acid 9.7 mg/dl, blood lead 9.2 microg/dl, and a urinary…

Lead-related nephrotoxicity: a review of the epidemiologic evidence.

Chronic kidney disease (CKD) represents a major global public health concern. Efforts to prevent and/or slow progression of CKD are essential. Lead nephropathy, characterized by chronic tubulointerstitial nephritis, is a well-known risk of chronic, high-level lead exposure. However, in recent years, lead exposure has declined sharply, particularly in developed countries. We reviewed epidemiologic research in general, occupational, and patient populations to assess whether lead, at current exposure levels, still contributes to nephrotoxicity. Other pertinent topics, such as risk in children, ge…

Cadmium, lead, and thallium in mainstream tobacco smoke particulate.

The deliveries of cadmium, thallium, and lead in mainstream smoke particulate from cigarettes with different smoke delivery designs were determined by inductively coupled plasma-mass spectrometry in order to investigate their impact on the delivery of these known toxic compounds. Analyses showed that the levels of all three metals in smoke particulate were associated with their tar delivery category. After normalizing the metal concentrations to tar, there were no longer any statistically significant delivery differences between full-flavor, light or ultra-light cigarettes. When the concentrat…

Lead poisoning caused by Indian ethnic remedies in Italy.

BACKGROUND: Complementary or alternative medicine has become widespread in Western Countries and since the remedies are "natural" they are believed to be free of toxic effects and health risks. Ethnic remedies may contain lead, other metals and toxic substances. OBJECTIVES: To show how lead poisoning as a result of using ethnic remedies may be severe enough to cause serious damage to health, and to increase awareness among family doctors and occupational physicians of the risks associated with ethnic remedies. METHODS AND RESULTS: Description of ethnic remedy-related lead poisoning in 2 nati…

Benchmark dose of lead inducing anemia at the workplace.

To estimate the critical dose of lead inducing anemia in humans, the effects of lead on hemoglobin (Hb) and hematocrit (Hct) levels and red blood cell (RBC) count were examined in 388 male lead-exposed workers with blood lead (BPb) levels of 0.05-5.5 (mean 1.3) micromol/L by using the benchmark dose (BMD) approach. The BPb level was significantly related to Hb (regression coefficient beta=-0.276), RBC (beta=-11.35), and Hct (beta=-0.563) among the workers (p < 0.001) when controlling for age and working status. The average BPb levels were significantly higher in the workers with anemia (1.85 m…

Complex genomic rearrangements lead to novel primate gene function.

Orthologous genes that maintain a single-copy status in a broad range of species may indicate a selection against gene duplication. If this … affecting approximately 10% of human Chromosome 2, enabled a detailed reconstruction of the events leading to the appearance of a novel gene family. The eight members of this family originated from the highly conserved nucleoporin RanBP2 by several genetic rearrangements such as segmental duplications, inversions, translocations, exon loss, and domain …

Left ventricular malposition of pacemaker lead in Chagas' disease.

A 52-year-old Argentinian woman presented with third-degree AV block due to seropositive chronic stage of Chagas' disease. Subsequently, a DDD pacemaker was implanted. Interestingly, a postoperative chest X ray suggested left ventricular lead misplacement, an ECG showed a paced RBBB. Echocardiography confirmed suspected lead malposition in the left ventricle with perforation of a large aneurysm of the interatrial septum that might be related to Chagas' disease. The ventricular lead was successfully repositioned in the right ventricle. Therefore, to avoid lead malposition in Chagas' disease str…

Parasympathetic cardiac nerve stimulation with implanted coronary sinus lead.

A patient with drug-refractory paroxysmal atrial fibrillation associated with rapid ventricular rate underwent biatrial pacemaker implantation. During elective replacement of the pacemaker, a significant voltage- and frequency-dependent decrease in ventricular rate was achieved by high-frequency electrical stimulation (17 Hz) of parasympathetic cardiac nerves innervating the AV node with the implanted bipolar coronary sinus electrode. The negative dromotropic effect of parasympathetic stimulation was eliminated by intravenous administration of 1-mg atropine.

Cystic kidney diseases: all roads lead to the cilium.

Cystic kidney disorders are one of the leading causes of end-stage renal disease. Numerous experimental animal models have been used to understand the disease pathogenesis. Recent advancements in this field have provided a surprising finding: that many of the proteins associated with cystic kidney disease localize to a nearly forgotten organelle, the primary cilium.

Impact of home carpets on childhood lead intervention study.

Blood lead data for a childhood lead exposure study have been reanalyzed to examine the impact of carpets on the effectiveness of a cleaning intervention in 39 New Jersey urban houses. All eligible houses in the study were classified as carpeted and uncarpeted depending on the number of rooms that were carpeted. The cleaning protocol was associated with a significant reduction in the blood lead concentrations for the uncarpeted homes (P = 0.004), whereas no significant change was found for the carpeted homes (P = 0.566). We also completed correlation analyses between the number of cleaning vis…

Assessment of bone lead during pregnancy: a pilot study.

More than 85% of American children raised in the 1970s had blood lead (BPb) levels >/=10 microg/dL, the level that currently defines childhood Pb poisoning. With exposure and absorption Pb accumulates in bone. Bone Pb release back to blood also occurs, particularly when kinetic rates of bone turnover are elevated. We examined a group of childbearing age, urban African American and Hispanic women to determine whether they had measurable bone Pb and whether bone Pb levels changed during pregnancy. Tibial bone Pb content was assessed sequentially 3 times over 4 months by L-line X-ray fluorescence…

Lead poisoning and chelation in a mother-neonate pair.

We report the case of a pregnant woman with chronic lead toxicity and a blood lead of 57 microg/dL (2.7 micromol/L) who gave birth to a healthy-appearing neonate with a cord blood lead of 126 microg/dL (6.08 micromol/L). The mother was prescribed a single course of oral succimer late in the third trimester of pregnancy, without any appreciable change in her blood lead. The neonate was initially treated with intramuscular dimercaprol and intravenous edetate calcium disodium. After 3 days, the neonate was then switched to oral 2,3-dimercaptosuccinic acid because the blood lead had declined. The …

Prominent R wave in lead V1: electrocardiographic differential diagnosis.

Tall lead V1 (tall RV1), defined as an R/S ratio equal to or greater than 1, is not an infrequent occurrence in emergency department patients. This electrocardiographic finding exists as a normal variant in only 1% of patients. Physicians should therefore be familiar with the differential diagnosis for this important QRS configuration. The electrocardiographic entities which can present with this finding include right bundle branch block, left ventricular ectopy, right ventricular hypertrophy, acute right ventricular dilation (acute right heart strain), type a Wolff-Parkinson-White syndrome, p…
