[Significance and results of subcapital osteotomy in severe slipped capital femoral epiphysis].

Today treatment with in situ fixation for mild slips in chronic SCFE is undisputed. However, it is far more difficult to give a definite recommendation in cases where a severe dorsal slip of >70 degrees has already occurred. With this position of the femoral head, a severe functional deficit combined with an early onset of osteoarthritis has to be expected. With these severe slips we prefer the subcapital osteotomy, a technique which results in an almost normal anatomical alignment of femoral head and acetabulum. In the literature, this procedure is subject to controversy because of the relati

The effect of lengthening of the femur on the extensors of the knee. An electromyographic study.

Surface electromyography of the quadriceps femoris muscle was performed in seven patients in order to determine the effect of extensive lengthening of the femur on the muscles that extend the knee. Electromyograms were made during sustained (thirty-second) isometric extension of the knee, at an angle of flexion of the knee of 30 degrees, while a load equal to 15 per cent of body weight was applied to the leg just proximal to the ankle. The average circumference of the thigh was smaller, the motor-unit recruitment of the muscles was slower, and the fatigability was greater on the involved side

A graphic method for timing the correction of leg-length discrepancy.

We have developed a clinical method for the graphic recording, analysis and planning of treatment of leg-length discrepancy during growth. Initially, the clinically determined discrepancy is plotted against the chronological age yearly, and then in late childhood at six-monthly intervals. CT and measurements of skeletal age are made in middle and late childhood to confirm the clinical findings. In a prospective study in 20 children, we observed that only eight had a linear increase in discrepancy. The observed pattern of increase was therefore used to estimate the mature discrepancy. Postoperative sciatic and femoral nerve palsy with reference to leg lengthening and medialization/lateralization of the hip joint following total hip arthroplasty.

The radiographs and prospective records of 1284 (1152 primary and 135 revisions) Charnley low friction arthroplasties performed by one surgeon were studied in reference to postoperative elongation of the limb and lateralization or medialization of the center of rotation of the hip joint and their effect on postoperative nerve palsy. Displacement of the center of the hip joint in relation to fixed points on the pelvis was measured. In primary low friction arthroplasties, leg lengthening ranged from 0.4 to 4 cm; in the revision group, they ranged from 0.04 to 5.8 cm. Sixty-six hips were lengthen

Premature greater trochanteric epiphysiodesis secondary to intramedullary femoral rodding.

Five skeletally immature patients developed premature closure of the greater trochanteric physis consequent to placement of an intramedullary rod for primary treatment of a femoral diaphyseal fracture. Each patient developed increased femoral neck valgus as compared with the contralateral hip. To date, however, none of these patients has had any functional disability, although one has developed mild radiographic subluxation. In addition, anatomic specimens demonstrated the likelihood of traversing a portion of the greater trochanteric physis. Other methods of fracture treatment, either operati

Measurement and restoration of equality in length of the lower limbs in total hip replacement.

Alteration of the length of the lower limb was measured in 39 patients undergoing total hip replacement. Radiographs were taken in the erect posture prior to operation and 6 months postoperatively. The inequality of limb length was measured by the difference between the height of the vertices of the femoral heads and by the difference between the lowest points of the ischial tuberosities. The change of the length was measured intraoperatively as the difference observed between two reference points: one above the acetabulum and the other on the proximal femur. Good correlation was obtained betw

[Results of the follow-up of varus osteotomy in Perthes disease with reference to the hip value].

A total of 89 patients with 105 hip joints affected by Perthes' disease at Catterall's stages II to IV, which had been treated by varus osteotomy, were re-examined clinically and radiologically between two and ten years after surgery. The clinical and subjective results of treatment were generally good. The best clinical and radiologic results were found in patients who contracted the disease before the age of seven and in whom necrosis was not yet at an advanced stage. The results in children who did not undergo treatment until after the age of eight and those with the disease at Catterall's

Fractures of the adult femur excluding the femoral head and neck: a review and evaluation of current therapy.

A review and appraisal of current methods of treatment of fractures of the femur including operative and nonoperative methods as well as complications or limitations of various methods, reveals that a specific appliance or procedure is of value only when used with proper indication and proper technique. When internal fixation is performed, load-sharing between appliance and bone is mandatory, because of the large mechanical stresses involved. Intramedullary fixation permits load-sharing more often than extramedullary fixation and should be used on femoral fractures whenever possible. Conservat

Comparative follow-up study of conservative and surgical treatment of femoral shaft fractures in children.

The guiding principles of treatment of femoral shaft fractures in children are discussed. Up to the age of six years a Bryant or Weber bandage was used. In older children surgical treatment was preferred. The indications are outlined. Between 1959 and 1974 100 children with femoral shaft fractures were treated in the Maasziekenhuis, 21 percent received surgical treatment. In 92 cases follow-up was possible. For detection of changes X-ray examination is preferred. Significant deformities were not found. An increase in length by more than one centimeter of the fractured femur was seen in 22 perc

Accuracy of two different imageless navigation systems for leg length and global offset change in total hip arthroplasty: A comparison using two-dimensional radiographic and three-dimensional CT-based evaluation.

PURPOSE: This study aimed to evaluate the accuracy of two imageless navigation systems for restoring leg length change (LLC) and global offset change (GOC) in total hip arthroplasty (THA) using two-dimensional (2D) radiographic and three-dimensional computed tomography (3D CT)-based assessment methods. METHODS: Patients undergoing primary cementless THA were divided into two groups based on the imageless navigation system used: a large-console group (n = 120) and a portable handheld group (n = 83). Intraoperative navigation measurements of the LLC and GOC were compared with values derived fro

Are artificial intelligence generated lower extremity radiographic measurements accurate in a cohort with implants?

OBJECTIVE: Leg length discrepancy (LLD) and malalignment of the lower extremity can lead to pain and increased risk of osteoarthritis. Radiographic measurements on anteroposterior (AP) full-length radiographs can be used to assess LLD and lower extremity alignment. The primary aim of this study was to evaluate the accuracy of AI software in performing lower extremity radiographic measurements in patients with implants. The secondary aim was to compare its efficiency to that of radiologists. MATERIALS AND METHODS: This study used the following eight angles and five lengths: hip-knee-angle (HKA

Modular endoprosthetic implant for maximizing residual limb length: A case report.

BACKGROUND:: For transfemoral amputations, the residual femoral length is critically important to prosthetic function. The aim of this report is to describe a novel method of extending femoral length and to assess its stability over time. CASE DESCRIPTION AND METHODS:: A 57-year-old woman with recurrent parosteal osteosarcoma was treated with above-knee amputation. A portion of the distal femoral endoprosthesis, which included uncemented fixation with a Compress® stem, was retained in an effort to extend the short femoral remnant and maximize stump length. FINDINGS:: At 3 years follow-up, th

Soft tissue tension is four times lower in the unstable primary total hip arthroplasty.

PURPOSE: The aim was to compare patients who suffered recurrent dislocation following total hip arthroplasty (THA) with those who did not to clarify the degree of soft tissue tension in dislocation patients. METHODS: The subjects were 18 hips with recurrent dislocation (unstable THA group) and 37 hips without dislocation (stable THA group). To evaluate soft tissue tension, radiographs were taken while applying distal traction at traction forces of 40, 30, and 20% of the body weight (BW) and femoral head displacement was measured. Acetabular offset, femoral offset, limb offset, and leg length

Acute Shortening and re-lengthening in the management of open tibia fractures with severe bone of 14 CMS or more and extensive soft tissue loss.

INTRODUCTION: A traumatic event to the tibia with more than 14 cm of bone and soft tissue loss represents a challenge to most orthopedic surgeons and is considered a limb-threatening condition. Few solutions are available in such clinical situations and among them is the acute shortening and re-lengthening of bone and soft tissue. MATERIALS AND METHODS: Our study presents the management of 7 patients with grade III B open fractures (according to the Gustillo-Anderson classification) of the tibia who underwent resection of all the devitalized tissues, acute limb shortening to close the defect,
