Assessing the Relationship Between Volumetric Changes and Functional Connectivity in Patients with Mild Cognitive Impairment.

Background: Amnestic mild cognitive impairment (aMCI) is considered a transitional state between normal aging and dementia, often without visible abnormalities on standard brain magnetic resonance (MR) images. The aim of the study was to analyze both microstructural and functional brain abnormalities using advanced MR techniques. Methods: The study included 27 patients with aMCI and an age-matched control group (CG) of 25 healthy subjects. All MR studies were performed on a 3T MR scanner (Philips, Ingenia) with a 32-channel head and neck coil using volumetric 3D T1 sequences, followed by a res

Thalamocortical connectivity and nucleus-specific thalamic dysfunction in temporal lobe epilepsy: an integrative review.

Temporal lobe epilepsy (TLE) is increasingly recognized as a distributed network disorder extending beyond mesial temporal structures. Among the brain regions implicated in epileptic network organization, the thalamus has emerged as a key hub due to its extensive structural and functional connections with cortical, limbic, and subcortical systems. This integrative review aimed to synthesize current evidence regarding nucleus-specific thalamic abnormalities and thalamocortical connectivity alterations in TLE.A literature search was conducted following PRISMA guidelines using PubMed and Google Sc

Advancing the Paradigm of Temporal Lobe Epilepsy as a Network Disease: The Promise of Biomarkers and Targeted Disease Modification.

Temporal lobe epilepsy (TLE) is increasingly conceptualized not as an isolated hippocampal lesion, but as a complex, multiscale limbic network connectomic disorder. Despite advances in pharmacological management, over 30% of patients experience drug-resistant epilepsy, underscoring the urgent need to shift from symptomatic seizure control to mechanism-directed disease modification. This review comprehensively synthesizes the pathophysiological architecture of epileptogenesis in TLE, spanning mitochondrial bioenergetic alterations, chronic neuroinflammation, synaptic reorganization, and ionic p

Neuromodulation for Epilepsy.

In patients with an unresectable epileptic focus, such as an undefined focus or epileptic focus within functional areas, various neuromodulation therapies, including vagus nerve stimulation, deep brain stimulation, and responsive neurostimulation, have been adopted as alternative treatment modalities. Vagus nerve stimulation, the earliest approved neuromodulation therapy in Japan, may be indicated irrespective of the epilepsy type and offers several advantages, including craniotomy not being required; however, its efficacy remains limited. Among deep brain stimulation targets, the anterior nuc

Cognitive decline and neuroimaging correlates in comorbid type 2 diabetes and depression: A systematic review.

Background: The comorbid state of diabetes and depression increases the risk of developing Alzheimer's disease compared to a single disease, but the relationship between cognitive decline and brain imaging changes in this state remains unclear. Objective: To systematically review the association between brain imaging changes and mild cognitive impairment or cognitive decline in this comorbid state. Methods: We searched four databases until April 15, 2025, combining keyword searches with MeSH terms and free words, and supplemented relevant studies with reference back. Observational studies were

Enrichment of patients with concomitant limbic-predominant age-related TDP-43 encephalopathy (LATE) on the Alzheimer's disease continuum using hippocampal volume.

INTRODUCTION: Clinical overlap between Alzheimer's disease (AD) and limbic-predominant age-related transactive response DNA binding protein 43 (TDP-43) encephalopathy (LATE), combined with the absence of validated in vivo biomarkers, complicates identification of mixed AD/LATE pathology. We labeled individuals along the AD continuum with suspected concomitant LATE using the lower quartile hippocampal volume (HV) cut-off and examined associated atrophy and cognitive profiles. METHODS: We studied cognitively impaired (CI) Alzheimer's Disease Neuroimaging Initiative (ADNI) participants with T1-m

Brain signatures of catastrophic events: Emotion, salience, and cognitive control.

Anticipatory brain activity makes it possible to predict the occurrence of expected situations. However, events such as traffic accidents are statistically unpredictable and can generate catastrophic consequences. This study investigates the brain activity and effective connectivity associated with anticipating and processing such unexpected, unavoidable accidents. We asked 161 participants to ride a motorcycle simulator while recording their electroencephalographic activity. Of these, 90 participants experienced at least one accident while driving. We conducted both within-subjects and betwee

Principal neurons in the olfactory cortex mediate bidirectional modulation of seizures.

Although the piriform cortex (PC) has been previously implicated as a critical node for seizure generation and propagation, the underlying neural mechanism has remained unclear. Here, we found increased excitability in PC neurons during amygdala kindling acquisition. Optogenetic or chemogenetic activation of PC pyramidal neurons promoted kindling progression, whereas inhibition of these neurons retarded seizure activities induced by electrical kindling in the amygdala. Furthermore, chemogenetic inhibition of PC pyramidal neurons alleviated the severity of kainic acid-induced acute seizures. Th

Longitudinal alterations of cortical structural-functional coupling in temporal lobe epilepsy.

BACKGROUND AND PURPOSE: To investigate the longitudinal alterations of cortical structural-functional coupling (SF coupling) in patients with temporal lobe epilepsy (TLE) over a 2-year follow-up, thereby exploring the neuropathophysiological mechanisms of TLE. METHODS: Twenty-eight TLE patients and 42 age- and gender-matched healthy controls (HCs) were recruited. We used resting-state functional MRI and diffusion-weighted imaging to estimate and compare SF coupling at the multiscale network level (whole-brain, modular, and regional levels). Then, we analyzed the relationships between the spat

Systematic Review of Different Neuroimaging Correlates in Mild Cognitive Impairment and Alzheimer's Disease.

Alzheimer's disease (AD) is a heterogeneous progressive neurocognitive disorder. Although different neuroimaging modalities have been used for the identification of early diagnostic and prognostic factors of AD, there is no consolidated view of the findings from the literature. Here, we aim to provide a comprehensive account of different neural correlates of cognitive dysfunction via magnetic resonance imaging (MRI), diffusion tensor imaging (DTI), functional MRI (fMRI) (resting-state and task-related), positron emission tomography (PET) and magnetic resonance spectroscopy (MRS) modalities acr

Reduced gray matter volume and cortical thickness associated with traffic-related air pollution in a longitudinally studied pediatric cohort.

Early life exposure to air pollution poses a significant risk to brain development from direct exposure to toxicants or via indirect mechanisms involving the circulatory, pulmonary or gastrointestinal systems. In children, exposure to traffic related air pollution has been associated with adverse effects on cognitive, behavioral and psychomotor development. We aimed to determine whether childhood exposure to traffic related air pollution is associated with regional differences in brain volume and cortical thickness among children enrolled in a longitudinal cohort study of traffic related air p

Developmental MRI markers cosegregate juvenile patients with myoclonic epilepsy and their healthy siblings.

OBJECTIVE: MRI studies of genetic generalized epilepsies have mainly described group-level changes between patients and healthy controls. To determine the endophenotypic potential of structural MRI in juvenile myoclonic epilepsy (JME), we examined MRI-based cortical morphologic markers in patients and their healthy siblings. METHODS: In this prospective, cross-sectional study, we obtained 3T MRI in patients with JME, siblings, and controls. We mapped sulco-gyral complexity and surface area, morphologic markers of brain development, and cortical thickness. Furthermore, we calculated mean geode

The role of rosemary extract in degeneration of hippocampal neurons induced by kainic acid in the rat: A behavioral and histochemical approach.

Systemic Kainic Acid (KA) administration has been used to induce experimental temporal lobe epilepsy in rats. The aim of this study was to evaluate the neuroprotective effect of rosemary extract (RE, 40% Carnosic acid) against KA-induced neurotoxicity in hippocampus and impaired learning and memory. Animals received a single dose of KA (9.5 mg/kg) intraperitoneally (i.p.) (KA group) and were observed for 2 h and were scored from 0 (for normal, no convulsion) to 5 (for continuous generalized limbic seizures). RE (100 mg/kg, orally) was administered daily for 23 days, starting a week before KA i

The association between lesion location and functional outcome after ischemic stroke.

BACKGROUND: Infarct location has a critical effect on patient outcome after ischemic stroke, but the study of its role independent of overall lesion volume is challenging. We performed a retrospective, hypothesis-generating study of the effect of infarct location on three-month functional outcome in a pooled analysis of the EPITHET and DEFUSE studies. METHODS: Posttreatment MRI diffusion lesions were manually segmented and transformed into standard-space. A novel composite brain atlas derived from three standard brain atlases and encompassing 132 cortical and sub-cortical structures was used

Comprehensive morphometry of subcortical grey matter structures in early-stage Parkinson's disease.

Previous imaging studies that investigated morphometric group differences of subcortical regions outside the substantia nigra between non-demented Parkinson's patients and controls either did not find any significant differences, or reported contradictory results. Here, we performed a comprehensive morphometric analysis of 20 cognitively normal, early-stage PD patients and 19 matched control subjects. In addition to relatively standard analyses of whole-brain grey matter volume and overall regional volumes, we examined subtle localized surface shape differences in striatal and limbic grey matt

Abnormal resting-state functional connectivity patterns of the putamen in medication-naïve children with attention deficit hyperactivity disorder.

Structural and functional alterations of the putamen have been reported in patients with attention deficit hyperactivity disorder (ADHD), but the functional relationships between this area and other brain regions are seldom explored. In the present study, seed-based correlation analyses were performed in the resting-state functional magnetic resonance imaging (fMRI) data to examine the differences in functional connectivity of the putamen between medication-naïve children with ADHD and normal children. Positive functional connectivity with the putamen-ROIs was seen in bilateral sensorimotor ar
