[Procedure in case of suspected mycobacterial infection of lymph nodes of the neck].

Procedure in Case of Suspected Mycobacterial Infection of Lymph Nodes of the Neck. Cervical lymphadenitis of unknown origin is frequently observed in ENT. Besides metastasis, lymphomas and non specific cervical lymphadenitis, tuberculosis or infection with atypical mycobacteria has to be considered. This article tries to present the state of the art procedure in diagnostics to differentiate tuberculosis and atypical mycobacterial infections from other pathologies of cervical lymph nodes.

Cat scratch disease and glomerulonephritis.

Cat scratch disease is a generally benign, self-limited disease characterized by fever and regional lymphadenitis. Systemic complications are rare and most frequently affect the central nervous system, bone, liver or spleen. We report the first case of glomerulonephritis associated with cat scratch disease which resolved spontaneously concurrent with the resolution of lymphadenitis and constitutional symptoms. Immunopathologic and ultrastructural studies suggest a form of immune-mediated postinfectious glomerulonephritis.

Leishmaniasis and Tularemia Coinfection: A Case Report of Neglected Causes of Cervical Lymphadenopathy.

Leishmania and tularemia are infectious diseases that both can present with lymphadenopathy. Leishmania typically causes visceral or cutaneous forms, while tularemia can result in glandular tularemia characterized by lymphadenitis. We report a case of a patient presenting with localized cervical lymphadenopathy diagnosed with both leishmaniasis and tularemia. This case underscores the importance of considering both pathogenic agents in the differential diagnosis of localized lymphadenitis. Early treatment is crucial to prevent the dissemination of these infections.

Mediastinal lymphadenopathy in malignancy: metastatic or granulomatous?

Mediastinal lymphadenopathy in patients with malignancy may not be always metastatic disease. We present three patients with proven thoracic or extra thoracic malignancies with mediastinal lymphadenopathy which were subsequently proven as granulomatous lymphadenitis by endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA). The objective of the current report is to emphasise that granulomatous lymphadenitis should be considered as an important differential diagnosis in such patients especially in tuberculosis endemic countries like India.

Q fever and lymphadenopathy: report of four new cases and review.

Coxiella burnetii, the causative agent of Q fever, is responsible for various clinical syndromes, but lymphadenitis has been described during Q fever in only three recent case reports. Four new cases of acute Q fever associated with lymphadenopathy are reported here, and these cases are discussed along with the three previously reported cases. Coxiella burnetii was isolated for the first time from a lymph node. Q fever should be considered an etiologic agent of lymphadenitis.

Determination of Corynebacterium pseudotuberculosis prevalence and antimicrobial susceptibility pattern of isolates from lymph nodes of sheep and goats at an organic export abattoir, Modjo, Ethiopia.

UNLABELLED: Corynebacterium pseudotuberculosis is the causative agent of caseous lymphadenitis, a suppurative abscessation in the superficial and internal lymph nodes and internal organs of small ruminants. This study was conducted on the superficial lymph nodes and carcasses of 768 small ruminants slaughtered at a slaughterhouse during the study period; 82 had abscesses or caseous lymphadenitis. The most frequent sites of abscesses in goats were the prescapular (34, 5·54%) and prefemoral lymph nodes (24, 3·91%) respectively. Similar patterns were observed in sheep. The prevalence of caseous l…

Salivary duct carcinoma of the submandibular gland presenting a diagnostic challenge: A case report.

BACKGROUND: Salivary duct carcinoma (SDC) is a rare, extremely aggressive malignancy that arises in the submandibular gland. It can metastasize locally early and therefore is an important differential diagnosis of metastatic disease in cervical lymph nodes or specific lymphadenitis such as tuberculous cervical lymphadenitis. CASE SUMMARY: We report a case of SDC in the submandibular gland that presented diagnostic difficulty. The lesion was coincidentally discovered through examination of the radiolucent area of the maxilla. Imaging failed to confirm the possibility of specific inflammation, …

Atypical large plasma cells in lymph node granulomas in cat-scratch disease.

A histological variant of plasma cells found in the granulomas of cat-scratch disease (CSD) lymphadenitis is reported. Though the lesion shows the typical features of suppurative granulomatous lymphadenitis, many atypical giant cells which have abundant basophilic cytoplasm and bizarre nuclei with occasional multinucleated forms are noted among epithelioid histiocytes. The diagnosis of CSD lymphadenitis was confirmed by comparing clinical; histopathological, and histochemical (Warthin-Starry silver impregnation stain) studies on lymph node sections from five cases with features typical of the …

Kikuchi-Fujimoto Disease: Diagnosis in a Relapsing Case.

The differential diagnosis of cervical lymphadenopathy is varied. Different age groups require different approaches. Kikuchi-Fujimoto disease or histiocytic necrotizing lymphadenitis is a rare but important diagnosis to consider after excluding more common aetiologies. We present the case of a 21-year-old female with painful left cervical swelling, lasting over a week. Physical examination revealed multiple cervical lymphadenopathies, elastic, non-adherent to deep tissues which were tender to touch. Blood tests showed elevated acute phase proteins. Cytomegalovirus, Epstein-Barr, toxoplasmosis,…

Successful treatment of cat-scratch disease with ciprofloxacin.

Cat-scratch disease is usually a benign, self-limited disease. Infection may be asymptomatic but is commonly associated with painful regional lymphadenitis. Occasionally, disease may result in systemic symptoms and dissemination. Five adult patients, aged 24 to 57 years, were diagnosed as having cat-scratch disease, based on a positive history of cat scratches followed by typical symptoms including painful regional lymphadenitis, malaise, and positive cat-scratch skin tests. Diagnostic evaluations revealed no other cause for the lymphadenitis. Three patients had not received prior treatment wi…

[Lymphadenopathy: demarcation to malignant lymphomas].

Recognition of the differential diagnosis between lymphadenitis and malignant lymphoma requires good knowledge of the basic forms of the disease as well in depth knowledge of the structure of the individual compartments. There are defined forms of lymphadenitis where the differential diagnosis to certain lymphoma entities is known. Other reactive structural alterations show indistinct limits so that a decision is only possible after using additional techniques, such as immunohistochemistry and molecular analyses. Finally, there are marginal areas which can only be clarified by including clinic…

Childhood cervical lymphadenopathy.

Cervical lymphadenopathy is a common problem in children. The condition most commonly represents a transient response to a benign local or generalized infection, but occasionally it might herald the presence of a more serious disorder. Acute bilateral cervical lymphadenopathy usually is caused by a viral upper respiratory tract infection or streptococcal pharyngitis. Acute unilateral cervical lymphadenitis is caused by streptococcal or staphylococcal infection in 40% to 80% of cases. The most common causes of subacute or chronic lymphadenitis are cat scratch disease, mycobacterial infection, a…
