We report a case of severe, irreversible transverse myelitis following CD19-directed CAR-T cell therapy (axicabtagene ciloleucel) for refractory post-transplant lymphoproliferative disorder (PTLD) in a young female lung-transplant recipient. This case illustrates an emerging neurotoxic complication of CAR-T cell therapy and highlights convergent risk factors including demographics, tumor type and CAR construct. We discuss current evidence of CAR-T-associated myelopathy, its proposed inflammatory and immune-trafficking mechanisms, and the potential contribution of interleukins and chemokines su

A woman in her 50s was diagnosed with post-transplant lymphoproliferative disorder, diffuse large B-cell lymphoma, and germinal center B-cell-like type based on a lower gastrointestinal tract biopsy. The treatment course comprised R-CHOP chemotherapy. She had undergone a living-donor kidney transplantation 22 years ago. During the fifth cycle of R-CHOP, a pharmacist observed exertional dyspnea and edema of the left lower extremity. Transthoracic echocardiography revealed a decrease in global longitudinal strain; however, elevated D-dimer levels were noted. Additionally, the edema was unilatera

BACKGROUND: Post-transplant lymphoproliferative disorder (PTLD) is a serious complication following cardiac transplantation, frequently driven by Epstein-Barr virus (EBV) in the setting of chronic immunosuppression. Despite its clinical significance, data specific to adult cardiac transplant recipients remain fragmented, with variability in reported incidence, risk factors, and outcomes.
METHODS: A scoping review was conducted using the Arksey and O'Malley framework and Joanna Briggs Institute guidelines. Five databases were searched for studies published between 2000 and 2025 focusing on EBV

PURPOSE OF REVIEW: Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a rare, benign ulcerative lesion of the oral mucosa that exists in both adult and infantile (Riga-Fede) forms. This review examines TUGSE by exploring its clinical presentation, pathogenesis, histopathological features, and treatment approaches. It briefly discusses oral CD30+ T-cell lymphoproliferative disorders (TLPDs) and their potential relation with TUGSE lesions.
RECENT FINDINGS: While traditionally considered reactive in nature, some recent evidence suggests TUGSE may share features with CD30+ T-cell…

Post-transplant lymphoproliferative disorder (PTLD) is a lymphoid or plasmacytic proliferation that occurs in association with immunosuppression and is recognized as a relatively rare but serious complication following solid organ transplantation; however, PTLD following islet transplantation is extremely rare, and its optimal management remains unclear. We report the case of a 55-year-old woman with a history of type 1 diabetes mellitus who underwent living-donor kidney transplantation followed by deceased-donor pancreas transplantation and subsequently received three islet transplantations d

INTRODUCTION: Post-transplant lymphoproliferative disorders (PTLD) are severe complications of transplantation associated with poor outcomes. Compared to early-onset PTLDs, late-onset PTLDs are much less associated with allograft localization. Herein, we report a case of Epstein-Barr virus (EBV)-negative lymphoma involving a graft 7 years after liver transplantation.
CASE PRESENTATION: A 45-year-old man presented with hepatomegaly 7 years after liver transplantation. Liver puncture biopsy revealed B-cell lymphoma with a high proliferation index and a germinal center B-cell-like subtype. Tissu…

BackgroundMycophenolate mofetil (MMF) is widely used to treat systemic lupus erythematosus (SLE), particularly in cases with severe and organ-threatening complications such as lupus nephritis. However, multiple reports have documented lymphoproliferative disorder (LPD) in patients with SLE undergoing MMF therapy, predominantly developing in the central nervous system and requiring aggressive treatment, including chemotherapy, radiation, and surgery.Case ReportWe herein report the case of a 58-year-old female patient with SLE who developed cervical, hepatic hilar, and para-aortic lymphadenopath

Patients with rheumatoid arthritis (RA) are at high risk of developing cardiovascular disease, infections and malignancies during their lifetime, with lymphoma being the most common malignancy. These patients may clinically present with a wide spectrum of lymphoid proliferations or lymphomas, which are called lymphoproliferative disorders (LPD). The prediction sites of LPD can be divided into nodal and extranodal lesions, with the majority being extranodal lesions. The sites of extranodal lesions especially in RA patients with LPD receiving methotrexate (MTX) are the pharynx, liver, spleen, lu…

Methotrexate-related lymphoproliferative disorder is a rare but serious complication of methotrexate treatment. Recurrent methotrexate-related lymphoproliferative disorder in the central nervous system is uncommon. An 86-year-old woman, who had been on methotrexate for 3 years to treat rheumatoid arthritis, was admitted with left hemiplegia and sensory disturbance. Brain magnetic resonance imaging revealed a right basal ganglia lesion that was hyperintense on diffusion-weighted imaging and showed patchy enhancement. Methotrexate-related lymphoproliferative disorder was suspected, and methotrex

Indolent natural killer cell lymphoproliferative disorder of the gastrointestinal tract (iNKLPD-GI) is an uncommon, recently recognized lymphoid proliferation of mature NK cells primarily manifesting in the GI tract. Unlike NK/T lymphoma, iNKLPD-GI exhibits a rather indolent clinical course, underscoring the need for cautious management to prevent unnecessary interventions. However, clinical and molecular features of this entity have not been thoroughly understood. This study aimed to add more information to the current knowledge of this disease. Seven patients with iNKLPD-GI were included in

Primary myelofibrosis (PMF) is a rare myeloproliferative neoplasm characterized by elevated platelet counts and fibrous tissues in the bone marrow. The JAK1/2 inhibitor (JAKi), ruxolitinib, has demonstrated efficacy in reducing splenic size, alleviating myelofibrosis-related symptoms, and improving overall survival. While an increased risk of lymphoproliferative disease (LPD) is suggested in patients with PMF, particularly those treated with JAKi, the involvement of Epstein-Barr virus (EBV) in such cases remains poorly documented. Here, we present the case of a 69-year-old woman with PMF who d

Lymphoproliferative disorders (LPDs) are serious complications that arise in patients with rheumatoid arthritis (RA) receiving immunosuppressive drugs (ISDs). Here, we reported a 73-year-old woman diagnosed with RA at 60 years of age and treated with methotrexate, bucillamine, prednisolone, and infliximab. She was referred to our hospital, Osaka Metropolitan University Hospital, with general malaise, pancytopenia, a right adrenal mass, and enlarged periaortic lymph nodes. Epstein-Barr virus was detected in serum. We suspected LPD development and performed a bone marrow biopsy, on which no mali…

BACKGROUND: Solid organ transplantation and bone marrow/hematologic stem cell transplantation recipients face a heightened risk of developing malignancies or cancer as a result of immunosuppression. Posttransplant lymphoproliferative disorders (PTLD) are a range of disorders from benign lymphoid growth to lymphoma found post-transplant. Risk factors for PTLD include high immunosuppressive use and oncogenic effects of Epstein-Barr virus (EBV). There is a lack of comprehensive clinical and pathological documentation of PTLD cases among Saudi patients, and the available data are limited to a few …

BACKGROUND: Primary central nervous system post-transplant lymphoproliferative disorder (PCNS-PTLD) is a rare condition, posing diagnostic and treatment challenges, with histological biopsy essential for diagnosis. Standardized treatment protocols are lacking. This disease requires urgent attention due to the increasing number of organ transplant surgeries and the use of immunosuppressive agents.
METHODS: From 2020 to 2023, our center diagnosed five patients with PCNS-PTLD. We reviewed their clinical records and conducted a comprehensive analysis of 22 literatures on PCNS-PTLD cases following

Primary cutaneous CD4+ small or medium T-cell lymphoproliferative disorder (PCSM-LPD) is a clonal T-cell proliferation disease confined to the skin. PCSM-LPD shares expression of T follicular helper (Tfh) cell markers with various mature T-cell lymphomas. However, the benign presentation of PCSM-LPD contrasts the clinical behavior of other Tfh-lymphomas. The aim of our study was to delineate the molecular similarities and differences between PCSM-LPD and other Tfh-derived lymphomas to explain the clinical behavior and unravel possible pathological mechanisms. We performed targeted next-generat

The incidence of lymphoproliferative disorders associated with methotrexate is rising in patients with rheumatoid arthritis. These disorders typically exhibit spontaneous tumor regression upon discontinuation of methotrexate therapy. Spinal lesions associated with these diseases are extremely rare. We present a case of systemic lupus erythematosus in which the patient developed lumbar spine lymphoproliferative disorders secondary to methotrexate therapy, which failed to regress despite discontinuation of the drug, ultimately leading to pathological fracture necessitating posterior spinal fixat…

Posttransplant lymphoproliferative disorder (PTLD) is a complication of solid organ transplantation and is associated with Epstein-Barr virus (EBV). Recently, EBV-related PTLD was defined as probable PTLD or proven PTLD. Probable PTLD involves significant lymphadenopathy, hepatosplenomegaly, or other end-organ manifestations, without a histological diagnosis, together with significant EBV DNAemia. Proven PTLD is the detection of EBV-encoded proteins in a tissue specimen, together with symptoms and/or signs originating from the affected organ. Probable PTLD after pediatric liver transplantation

A 41-year-old woman received stem cell transplant due to acute myeloid leukemia. Six months after the stem cell transplant, the patient experienced monomorphic PTLD (posttransplant lymphoproliferative disorder) and underwent chemotherapy. Postchemotherapy PET/CT did not show any abnormal 18 F-FDG accumulation in the scanned body, including brain. Four months later, she presented with nausea and vomiting. Brain MRI showed a lesion in the right temporal lobe. In addition to the abnormal 18 F-FDG accumulation in the brain, no other abnormal foci were noted in the torso PET/CT. Biopsy pathology co

BACKGROUND: PTLD is a clinical condition with high mortality. Monitoring EBV replication can be a useful tool to avoid the development of PTLD.
MATERIALS AND METHODS: This was a retrospective analysis of 428 pediatric patients who underwent liver transplantation between 1989 and 2016. The patients were divided into 2 groups (transplanted before 2006, when PCR-EBV was not monitored, and after 2006, when PCR-EBV monitoring was started). Patients with continuous PCR measurements for EBV were evaluated for the impact of a reduction in immunosuppression or a change in immunosuppressants on the num
