Comparison of effects of perflubron and surfactant lung lavage on pulmonary gas exchange in a piglet model of meconium aspiration.

In a piglet model of meconium aspiration we compared lavage with surfactant with that with perflubron (PFOB) and a control group. A human meconium suspension was instilled into piglets which were randomized in 3 (n = 6 each) groups. After lung injury, the control group was ventilated with high-frequency oscillatory ventilation (HFOV) without suctioning and lavage. A second group was lavaged with 10 ml/kg diluted surfactant, a third with 10 ml/kg pre-oxygenated PFOB. Thereafter, the animals of both groups were ventilated with HFOV. After lung injury by instillation of meconium, no further impro…

Genetic comparisons of patients with cystic fibrosis with or without meconium ileus. Clinical Centers of the French CF Registry.

Cystic fibrosis (CF) is an autosomal disorder caused by mutations in the cystic fibrosis transmembrane conductance regulator gene (CFTR). Neonatal meconium ileus (MI) occurs in 10-20 percent of newborns with CF. The purpose of this study was to determine the allelic frequencies of the CF mutation in French patients with and without MI and the incidence of MI in 7 homozygotes or compound heterozygotes for mutation of the CFTR gene. Our study confirms the positive association between delta F508, the most frequent CF mutation, G542X mutation and MI and a negative association with G551D.

Inflammation of the lungs, umbilical cord and placenta associated with meconium passage in utero. Review of 123 autopsied cases.

It is uncertain how often the passage of meconium in utero is a response to some event causing fetal distress as opposed to being simply the physiologic functioning of a maturing intestinal tract. The extent to which meconium may produce injury or inflammation in pulmonary and placental tissues after intrauterine exposure is also unclear. This study was a retrospective review of 123 cases, 79 stillborn and 44 liveborn less than one month of age, autopsied at The Johns Hopkins Hospital, and showing histologic evidence of intrauterine meconium exposure by aspirated meconium or meconium macrophag…

[Value of toxicological research in newborn infants of addicted mothers by the study of several samples (urine, meconium, hair)].

BACKGROUND: Urinary detection of prenatal drug exposure in the neonate may give false-negative results. We report our experience on meconium and hair testing, in addition to urine testing in order to improve diagnosis of fetal drug exposure. POPULATION AND METHODS: Thirty-one infants (aged 1-45 days) whose mothers were confirmed (n = 12) or suspected (n = 19) to be drug-addicted were included in the study. One or more specimens of urine, meconium or hair were collected in the 31 infants, two of the specimens in 17 and three in six. Drugs and their metabolites were detected by immunoenzymologi…

Failure of acute perinatal asphyxia or meconium aspiration to produce persistent pulmonary hypertension in a neonatal baboon model.

OBJECTIVE: Our purpose was to determine whether perinatal asphyxia or meconium aspiration, or both, can produce the physiologic and histologic pulmonary vascular changes associated with the meconium aspiration syndrome. STUDY DESIGN: Twenty neonatal baboons were studied in four groups: 1, control; 2, meconium aspiration; 3, asphyxia (intermittent cord compression); and 4, asphyxia with meconium aspiration. Animals were ventilated for 24 hours under ketamine, diazepam, and pancuronium. Data were analyzed by means of mixed model analysis of measures. RESULTS: Meconium significantly impaired ox…

The age-related incidence of meconium ileus equivalent in a cystic fibrosis population: the impact of high-energy intake.

During the decade from 1976 to 1986, the age-related incidence of meconium ileus equivalent (MIE) was calculated on the basis of 240 Danish cystic fibrosis (CF) patients. In the first 5 years, the patients were given enteric-coated granules of pancreatic enzymes (Pancreatin, Rosco, Denmark), and a low fat diet was recommended. In the last 5 years, the Center recommended supplementation with acid-resistant, enteric-coated, encapsulated microspheres (Pancrease, Cilag, Birkerød, Denmark), and high-energy intake with a 40% fat content and no dietary restrictions. There was no difference in the inc…

Echogenic fetal bowel in the third trimester associated with meconium ileus secondary to cystic fibrosis. A case report.

Echogenicity of the fetal bowel seen sonographically can vary at different gestational ages. Increased echogenicity of small bowel loops is seen commonly in the second trimester. We encountered a case of increased bowel echogenicity with shadowing in the third trimester in a fetus who had meconium ileus associated with cystic fibrosis. Other reports also indicate that a persistent echogenic appearance of fetal small bowel in the third trimester, particularly with shadowing, is worrisome and can be associated with meconium ileus.

[Prenatal diagnosis of fetal meconium peritonitis, therapy and clinical course. With a contribution to the differential diagnosis of fetal ascites].

We report on the prenatal diagnosis of a case of foetal meconium peritonitis pursuant to colon perforation in the 34th week of gestation. A sonographic examination of the foetus showed ascites, dense zones around the peritoneum and intestine, hepatosplenomegaly and hydroceles, as well as a slight hydramnios. The foetal abdomen was punctured under ultrasonographic control, and ascitic fluid was withdrawn. It was sterile but included granulocytes, leucocytes, epidermal epithelial cells and lanugo hairs, thereby confirming the diagnosis. The bilirubin concentration was very high, and the protein …

Predictors of adverse neonatal outcomes in low-risk nulliparous women at term with meconium-stained amniotic fluid.

OBJECTIVES: Meconium-stained amniotic fluid (MSAF) has been linked to adverse neonatal outcomes, but most evidence comes from heterogeneous cohorts that included multiparous and high-risk women. Nulliparous women without comorbidities are generally considered low risk, yet their outcomes in the presence of MSAF remain less well defined. This study aimed to identify predictors of adverse neonatal outcomes in low-risk nulliparous women at term with MSAF. DESIGN: A retrospective cohort study. PATIENTS: Seven hundred sixty low-risk nulliparous women with documented MSAF at term delivery between …

Diagnostic Pitfalls in Meconium-Related Ileus: Red Flags for Surgical Pathologies in Extremely Low Birth Weight Infants.

AIM: Meconium-related ileus (MRI) often mimics other surgical pathologies in extremely low birth weight infants (ELBWIs), leading to diagnostic difficulties. This study aimed to clarify the clinical presentation and management outcomes of enema therapy in ELBWIs with a specific focus on the clinical indicators of underlying non-MRI surgical pathologies. METHODS: A retrospective review was conducted of 31 ELBWIs (birth weight < 1000 g) who received enema therapy for suspected MRI at a single tertiary center between 2008 and 2024. Infants were categorized into Success (n = 20) and Failure (n = …

Biomonitoring and exposure predictors of 29 dioxin, furan, and dl-PCB congeners in newborn meconium from Spain.

Prenatal exposure to persistent organic pollutants (POPs) is a matter of great concern due to the vulnerability of the foetus. Seventeen polychlorinated dibenzo-p-dioxins and dibenzofurans (PCCD/Fs) and 12 dioxin-like polychlorinated biphenyls (dl-PCBs) were measured in meconium samples from 50 Spanish newborns using gas chromatography coupled to high-resolution mass spectrometry (GC-HRMS). Median concentrations (wet weight) of PCDD/Fs and dl-PCBs were 0.017 pg toxic equivalents (TEQ)/g and 0.0017 pg TEQ/g, respectively. Highly significant correlations were observed between some congeners of d…

Detection of various microplastics in placentas, meconium, infant feces, breastmilk and infant formula: A pilot prospective study.

Pregnancy and infancy are sensitive windows for environmental exposures. However, no study has investigated the presence of microplastics (MPs) in mother-infant pairs, and the exposure sources. Therefore, we aim to assess MPs exposure in placenta, meconium, infant feces, breast milk and infant formula samples, and assess the potential sources of pregnancy and lactational exposure to MPs. A total of 18 mother-infant pairs were recruited, and placentas and meconium samples were collected. Infant feces, breast milk and infant formula samples were collected at 6 months of age. We also collected da…
