Postpartum Linkage to Primary Care Among Patients With Diabetes and Hypertensive Disorders of Pregnancy Following Medicaid ACO Implementation.

BACKGROUND: Timely postpartum linkage to primary care is important for managing diabetes and hypertensive disorders of pregnancy (HDP), yet linkage remains suboptimal. Medicaid accountable care organizations (ACOs) aim to strengthen care coordination and improve postpartum linkage, although effects may vary by ACO design. OBJECTIVE: Evaluate whether Massachusetts Medicaid ACO implementation was associated with changes in postpartum primary care linkage among enrollees with diabetes or HDP and whether effects differed by ACO model. DESIGN: Retrospective cohort study using the 2016-2023 Massac…

Effect of Primary Care Value-Based Payment Implementation on Low-Value Care Among Medicaid Beneficiaries in North Carolina.

OBJECTIVE: To assess the impact of primary care value-based payment implementation on low-value service use among Medicaid beneficiaries. STUDY SETTING AND DESIGN: On July 1, 2021, North Carolina Medicaid launched the Advanced Medical Home (AMH) program to improve care quality and coordination under managed care. Health plans are now required to include financial incentives for care coordination and performance in contracts with the highest-tier primary care practices (Tier 3 AMHs). Leveraging the tiered structure of the program and using a difference-in-differences design, we compared change…

Impact of Medicaid waivers on medication for opioid use disorders in residential facilities: Evidence from twenty-five states.

BACKGROUND: Medicaid Section 1115 waivers allow states to test demonstration programs with the potential to shape access to care for millions of beneficiaries. The Institutions for Mental Diseases (IMD) waiver enables states to use federal funds to treat substance use disorders in residential facilities, bypassing Medicaid's long-standing IMD exclusion. We assessed the impact of IMD waiver adoption on the use of medication for opioid use disorder (MOUD) by estimating cohort-specific effects among four groups of states that adopted waivers between 2016 and 2019. METHODS: We used Treatment Epis…

Spillover Effects of Medicaid Expansion on Insurance Coverage, Diagnosis, and Survival Among Low-Income Older Patients With Cancer.

BACKGROUND: Medicaid expansion under the Affordable Care Act primarily benefits adults aged <65 years but may also indirectly benefit individuals aged ≥65 years. This study examines the associations of Medicaid expansion with insurance coverage and cancer outcomes among older patients residing in low-income areas. PATIENTS AND METHODS: Using the National Cancer Database, we identified patients aged ≥65 years, newly diagnosed with cancer 2010-2021, and residing in areas with median household income <200% of the federal poverty level. Using the quasi-experimental difference-in-differences desig…

Facilitation of Health Plan-Clinic Partnerships to Improve Colorectal Cancer Screening for Rural Medicaid Enrollees: A Qualitative Study.

PURPOSE: Mailed fecal immunochemical test (FIT) and patient navigation for follow-up colonoscopy have been shown to increase colorectal cancer (CRC) screening rates, but implementation barriers exist in rural settings. Partnerships between clinics and Medicaid health plans may address these barriers. In a large pragmatic randomized trial, we conducted a qualitative analysis to identify ways in which practice facilitation supported the implementation of a partnered health plan-clinic intervention of mailed FIT and patient navigation to improve CRC screening among rural Medicaid enrollees. METH…

Association between peer support services, treatment engagement and overdose risk among Kentucky Medicaid beneficiaries with opioid use disorder.

BACKGROUND: Peer-support specialists leverage experience to assist individuals with opioid use disorder (OUD) in their recovery, leading to better patient health outcomes. However, the longitudinal impact of peer support on treatment and health outcomes is understudied. METHODS: We identified the universe of patients with OUD beneficiaries in Kentucky's Medicaid managed care program from 2020 to 2021. Among this population, we utilized mixed-effect logistic regression models to study the association between peer-support engagement duration and buprenorphine receipt and opioid overdose risk. W…

Mailed Outreach and Patient Navigation for Colorectal Cancer Screening Among Rural Medicaid Enrollees: A Cluster Randomized Clinical Trial.

IMPORTANCE: Approximately 60 million adults live in rural regions of the US, which historically have low rates of colorectal cancer (CRC) screening and follow-up. Rural residents enrolled in Medicaid have particularly low CRC screening and follow-up rates. OBJECTIVE: To determine the effectiveness and implementation of a collaborative Medicaid health plan-clinic program of mailed fecal immunochemical test (FIT) outreach and patient navigation to colonoscopy following an abnormal FIT result when implemented in rural clinics as part of standard care. DESIGN, SETTING, AND PARTICIPANTS: This clu…

Most State Medicaid Programs Cover Routine Eye Exams For Adults, But Coverage Of Other Routine Vision Services Varies.

More than twelve million US adults ages forty and older are affected by vision impairment, and projections suggest that this number will double by 2050. Although most vision impairment can be eliminated with corrective lenses, many adults lack access to routine eye care. In this study, we analyzed detailed state-by-state Medicaid policies for 2022 and documented variability in coverage for adult vision services. Most fee-for-service Medicaid programs covered routine eye exams, although many did not cover glasses (twenty states) or low vision aids (thirty-five states), and about two-thirds of s…

Association of Tobacco Dependence Treatment Coverage Expansion With Smoking Behaviors Among Medicaid Beneficiaries Living With Substance Use Disorder.

INTRODUCTION: Many U.S. states expanded the generosity of Medicaid insurance coverage of tobacco dependence treatment over the last fifteen years, but little is known about how coverage impacts cigarette smoking, especially for individuals with substance use disorder. METHODS: Data are from the 2009 to 2018 National Survey on Drug Use and Health and include Medicaid beneficiaries 18-64 years old with past year substance use disorder who smoked at least 100 cigarettes in their lifetime. Outcomes were smoking cessation, nicotine dependence, and number of cigarettes smoked per month. Difference-…

Prenatal opioid use as a predictor of postpartum suicide attempts among reproductive-age women enrolled in Oregon Medicaid.

BACKGROUND: The rates of suicide and opioid use disorder (OUD) among pregnant and postpartum women continue to increase. This research characterized OUD and suicide attempts among Medicaid-enrolled perinatal women and examined prenatal OUD diagnosis as a marker for postpartum suicide attempts. METHODS: Data from Oregon birth certificates, Medicaid eligibility and claims files, and hospital discharge records were linked and analyzed. The sample included Oregon Medicaid women aged 15-44 who became pregnant and gave live births between January 2008 and January 2016 (N = 61,481). Key measures inc…

The Association Between Hospital-Based Food Pantry Use and Subsequent Emergency Department Utilization Among Medicaid Patients With Diabetes.

We explored the association between the use of a hospital-based food pantry and subsequent emergency department (ED) utilization among Medicaid patients with diabetes in a large safety-net health system. Leveraging 2015-2019 electronic health record data, we used a staggered difference-in-differences approach to measure changes in ED use before vs after food pantry use. Food pantry use was associated with a 7.3 percentage point decrease per patient per quarter (95% confidence interval, -13.8 to -0.8) in the probability of subsequent ED utilization ( P = .03). Addressing food insecurity through…

Assessing health care resource use, outcomes, and costs among Medicaid beneficiaries receiving factor IX prophylaxis for hemophilia B.

BACKGROUND: Hemophilia B is characterized by a deficiency of clotting factor IX (FIX), leading to excessive bleeding. Hemophilia B is commonly treated using replacement FIX therapy, which may be administered prophylactically or on-demand following a bleeding episode. Previous research has found high health care resource use (HCRU) and costs among Medicare and commercially insured people with hemophilia B (PwHB), with FIX therapy being a primary driver of health care costs. OBJECTIVE: To assess HCRU, outcomes, and costs among US Medicaid beneficiaries receiving FIX prophylaxis for hemophilia B…

Treatment setting among individuals with opioid use and criminal legal involvement, housing instability, or Medicaid insurance, 2015-2021.

BACKGROUND: Individuals with criminal legal involvement (CLI), housing instability, or Medicaid insurance may experience barriers accessing substance use treatment in certain settings. Previous research has found individuals in these groups are less likely to receive medications for opioid use disorder (MOUD), but the role treatment setting may play in low rates of MOUD is unclear. METHODS: We conducted a cross-sectional study using nationally representative survey data from 2015 to 2021. We estimated the proportion of individuals who had CLI, housing instability, or Medicaid insurance who re…
