Two cases of well-known syndrome due to midbrain-brain stem hemorrhage without having a history of hypertension were reported. Case 1 is a 62-year-old female, presenting Weber's syndrome due to midbrain hemorrhage and diagnosed as Moyamoya disease angiographically. Case 2 is a 24-year-old-male, presenting One and a half syndrome due to brain stem hemorrhage from a brain stem cavernous angioma, which was diagnosed angiographically and by magnetic resonance imaging (MRI). Left hemiparesis seen in case 1 had improved by conservative therapy but no improvement could be obtained in the oculomotor n

OBJECTIVE: Mesenrhombencephalitis is a serious form of brainstem inflammation predominantly involving the deep and vital portions of the brain, that is, the mesencephalon (midbrain) and rhombencephalon (pons, medulla). Mesenrhombencephalitis is difficult to diagnose on the basis of clinical and laboratory findings alone, and access to this portion of the brain for surgical biopsy carries high morbidity. We describe the MR appearance of mesenrhombencephalitis and correlate the imaging findings with clinical information.
MATERIALS AND METHODS: Unenhanced and contrast-enhanced MR images of nine …

A 68-year-old man was admitted because of sudden onset gait disturbance and bradykinesia. He experienced left putaminal bleeding following mild right hemiparesis and emotional incontinence without any difficulties in his daily life since three years before admission. On neurological examination the patient was alert and oriented. He showed forced crying. Myerson's sign was positive. There were no abnormal findings in ocular movements, pupillary reflexes and other cranial nerves. Muscle tone was increased with cog-wheel phenomenon in bilateral upper extremities. Coordination was preserved. He s

We report a patient with meningovascular syphilis who had a dorsal midbrain syndrome, cognitive dysfunction, and a left peripheral seventh nerve palsy. Magnetic resonance imaging (MRI) disclosed a large lesion of the midbrain and thalamus with intense enhancement of the interpeduncular cistern, both of which resolved after treatment with intravenous penicillin. The clinical features, radiographic appearance, and response to therapy suggest that this lesion was a focal syphilitic inflammatory process, or gumma. We conclude that MRI with intravenous contrast may reveal the full spectrum of patho

We reported a patient with a brainstem ptosis (midbrain ptosis) associated with mesencephalic hemorrhage. A 57-year-old woman had the sudden onset of bilateral blepharoptosis and diplopia. On admission, computed tomography of the brain and magnetic resonance imaging disclosed a small hematoma in the left tegmentum of midbrain. She had no past history of hypertension of head trauma. Cerebral angiography gave no additional informations. The lesion involving the central caudal subnucleus of IIIrd nerve nucleus may be responsible for the bilateral ptosis, since this finding is consistent with curr

Internuclear ophthalmoplegia (INO) is a gaze disorder characteristic of lesions involving the medial longitudinal fasciculus (MLF). Two patients aged 62 and 74 years presented with acute neurologic deficits characteristic of INO. Computed axial tomography (CT) did not detect abnormalities in the region of the MLF. Magnetic resonance imaging (MRI) however, demonstrated brainstem hypersignals with long T2 characteristics in the region of the MLF which correlated with the clinical symptoms. These cases illustrate the value of the MRI in evaluation of patients with INO and suggest that MRI is supe

The study of clinico-neurological, electroencephalographical and pathological data in 37 patients with post-dislocation lesions of midbrain and pons Varolii yielded identification of the syndroms of isolated functioning of brainstem-cerebral and brainstem-spinal systems: mesencephalo-pontine, supramesencephalic, unilateral. Clinico-physiological features characteristic of these syndromes are discussed as different from those of acute brain dislocations. Their value in diagnosis and prediction of the death of brain as an organ.

A 61-year-old woman is presented with a bilateral total ophthalmoplegia as an initial ocular symptom, caused by a midbrain hematoma. She complained of acute headache, nausea, vomiting and bilateral closure of her eyelids. Examination on admission showed meningeal irritation; mild consciousness disturbance; bilateral total ophthalmoplegia; left hemiparesis; ataxia in all extremities, more marked to the left. Computed tomography demonstrated a small hematoma in the midbrain tegmentum. Angiography demonstrated midbrain arteriovenous malformation, and she was treated conservatively. Abduction of b

Efferent connections to midbrain and thalamus from portions of the cerebellar fastigial nucleus were investigated using autoradiographic tec … "secondary" injection zones were defined according to specific criteria. Labeled axons reached the mesencephalon via the contralateral uncinate fasiculus. Ascending fibers assembled in a diffuse contingent at the prerubral level adjacent to the ventrolateral periaqueductal gray. The heaviest projections were contralateral to the injection site, but ipsilateral …

The interpeduncular nucleus (IPN) is a midbrain structure that receives its major afferents from the medial habenulae via the fasciculi retroflexi. Among the axons projecting to the IPN is a population of substance P (SP)-containing axons. The IPN has been subdivided into the central, dorsal, intermediate, rostral, and lateral subnuclei using cytoarchitectonic criteria. The distribution of SP among these subnuclei was determined by using Sternberger's ('79) peroxidase-antiperoxidase technique. In the normal IPN the rostral subnucleus can be subdivided into two sectors on the basis of SP conten

Chronic experiments on conscious rabbits demonstrated that benzonal suppresses the spontaneous bioelectrical activity of the motor division of the cortex of large hemispheres while failing to affect the spontaneous biorhythms of the nonspecific nuclei of the thalamus, caudate nucleus, dorsal hippocamp and the basolateral nuclei of the tonsil. The drug decreased the electroexcitability of the motor cortex and caudate nucleus, exerted a mild depressant action on the reticular formation of the midbrain, tonsil, and hippocamp and had no effect on the excitability of nonspecific nuclei of the thala

To determine whether calcitonin (CT) is present in avian central nervous system (CNS), as it has been recently found in the CNS of other species, we studied extracts of pigeon (Columbia livia) brain by immunochemical methods. A sensitive and specific radioimmunoassay for salmon CT was used together with gel filtration and high performance liquid chromatography (HPLC). Extracts of hypothalamus contained 31.5 +/- 9.65 ng/g wet weight as salmon CT-like immunoreactivity while extracts of midbrain and brain stem contained 17.0 +/- 5.00 and 2.85 +/- 2.1 ng/g wet weight respectively. Extracts of whol

In 2 patients with retrocollis, the positive effects obtained by stereotactic high frequency coagulation of the prestitial nucleus on the tonic activity of the neck muscle were studied electromyographically and clinically. Before the stereotactic therapy, electromyographic recordings at rest were recorded from the musculi splenii. During stimulation of the prestitial nucleus at frequencies of 8, 25 and 50 Hz, the EMG activity at rest diminished in frequency and amplitude and ultimately disappeared. Passage from the EMG recordings at rest to electrical silence took place, in one case, though a
