A comparison of the diagnostic accuracy of MARS MRI and ultrasound of the painful metal-on-metal hip arthroplasty.

BACKGROUND AND PURPOSE: Metal artifact reduction sequence (MARS) MRI and ultrasound scanning (USS) can both be used to detect pseudotumors, abductor muscle atrophy, and tendinous pathology in patients with painful metal-on-metal (MOM) hip arthroplasty. We wanted to determine the diagnostic test characteristics of USS using MARS MRI as a reference for detection of pseudotumors and muscle atrophy. PatienTS AND METHODS: We performed a prospective cohort study to compare MARS MRI and USS findings in 19 consecutive patients with unilateral MOM hips. Protocolized USS was performed by consultant musc

Abductor dysfunction and related sciatic nerve palsy, a new complication of metal-on-metal arthroplasty.

The optimal bearing for use in young patients with hip arthritis remains elusive. Current options include metal-on-cross-linked polyethylene, ceramic-on-cross-linked polyethylene, ceramic on ceramic, and metal on metal. Each of these bearing couples has advantages and disadvantages. Metal-on-metal designs allow the use of large heads that decrease impingement and improve stability. This fact has made this bearing an attractive option for surgeons and patients alike. This case report will illustrate a severe adverse reaction to metal debris with necrosis of soft tissues and subsequent damage to

Metal-on-metal total hip arthroplasty: do symptoms correlate with MR imaging findings?

PURPOSE: To determine the prevalence of magnetic resonance (MR) imaging abnormalities after metal-on-metal total hip arthroplasty and to determine whether presence of symptoms correlates with findings at MR imaging. MATERIALS AND METHODS: This HIPAA-compliant study was approved by the institutional review board, and informed consent was waived. MR imaging was performed with conventional sequences and a 1.5-T clinical imager in 192 hips (174 patients) evaluated during a 15-month period. Two observers retrospectively reviewed the images for the presence and size of pseudotumor, communication wi

Reorganizing metals: the use of chelating compounds as potential therapies for metal-related neurodegenerative disease.

Metal ions, particularly copper, zinc and iron, are implicated in several amyloidogenic neurodegenerative disorders. In the brain, as elsewh … deficiency may be a plausible way to limit neurodegeneration. In this review we discuss the role of metals in amyloidogenic, neurodegenerative disorders and highlight some mechanisms and compounds used in various therapeutic approaches.

Prevalence and clinical relevance of radiographic signs of impingement in metal-on-metal hybrid hip resurfacing.

BACKGROUND: Implant-related impingement has been reported following metal-on-metal hip resurfacing, and reactive osseous patterns associated with implant-bone impingement have been identified. The purpose of this study was to determine the prevalence and clinical implications of radiographic signs of femoral neck-acetabular cup impingement following metal-on-metal hip resurfacing. METHODS: Serial anteroposterior and lateral radiographs made five to 12.9 years postoperatively were available for ninety-one of the first 100 metal-on-metal hip resurfacing procedures (in eighty-nine patients) perf

Analysis of wear of retrieved metal-on-metal hip resurfacing implants revised due to pseudotumours.

The presence of pseudotumours, which are soft-tissue masses relating to the hip, after metal-on-metal hip resurfacing arthroplasty has been associated with elevated levels of metal ions in serum, suggesting that pseudotumours occur when there is increased wear. We aimed to quantify the wear in vivo of implants revised for pseudotumours (eight) and of a control group of implants (22) revised for other reasons of failure. We found that the implant group with pseudotumours had a significantly higher rate of median linear wear of the femoral component at 8.1 microm/year (2.75 to 25.4) than the 1.7

Eleven-year follow-up of second-generation metal-on-metal total hip arthroplasty.

PURPOSE: To investigate the cause of failure in matte-surface cemented stems in second-generation metal-on- metal total hip arthroplasty (THA). METHODS: Records of 26 men and 11 women (39 hips) aged 29 to 72 years who underwent primary cementless THAs by a single surgeon using second-generation metal-on-metal prostheses and were followed up for a mean of 122 (range, 120-141) months were reviewed. Two types of femoral stems were used: a cementless Ti-alloy stem (n=21) and a matte-surface, iron-based alloy, cemented stem (n=18). Clinical outcomes were measured using the Harris hip score. Large diameter head metal-on-metal bearings total hip arthroplasty: preliminary results.

INTRODUCTION: Although the use of the metal-on-metal bearings has been validated over the long term in total hip arthroplasty (THA) for standard 28 and 32 mm diameters, and over the medium term in resurfacing procedures, the use of larger metal head size in conventional THA has not yet been extensively reported. HYPOTHESIS: The large-diameter metal-on-metal head is beneficial in terms of implant stability without altering the result in terms of function and bone fixation compared to the standard 28 and 32 mm diameters. OBJECTIVE: The objective was to test this hypothesis by assessing the sho

Metal-on-metal total hip resurfacing arthroplasty in the presence of extra-articular deformities or implants.

BACKGROUND: Hip resurfacing has been proposed as an alternative to total hip replacement in patients who have proximal femoral deformities or retained hardware in the proximal aspect of the femur. In these situations, placement of a conventional stemmed hip prosthesis would be difficult or impossible, possibly necessitating a complex osteotomy or a custom prosthesis. The purpose of this study was to evaluate a series of patients who had extra-articular deformities of the proximal aspect of the femur and/or retained hardware and who were managed with a resurfacing hip prosthesis. METHODS: Fift

Comparison of single-level cervical fusion and a metal-on-metal cervical disc replacement device.

Cervical fusion is the common treatment for cervical disc disease but can cause secondary disorders. The Prestige ST cervical disc prosthesis (Medtronic Sofamor Danek, Memphis, TN) was designed to preserve spinal motion to potentially limit the secondary disorders. In this article, we report 2-year results from a single-center study comparing use of this device with use of anterior cervical discectomy and fusion (ACDF). Nineteen patients were prospectively randomized to receive the device or to undergo ACDF. Twenty-four months after surgery, patients who received the device demonstrated improv

Biological effects of clinically relevant wear particles from metal-on-metal hip prostheses.

The problems of osteolysis and late aseptic loosening associated with ultra-high molecular weight polyethylene (UHMWPE) particles has lead to a renewed interest in metal-on-metal prostheses. Wear particles generated by modern Co-Cr-on-Co-Cr prostheses are nanometre in size (range: 10-120 nm; mean: about 40 nm), an order of magnitude smaller than the size of UHMWPE known to be critical for activation of osteolytic cytokines by macrophages. Co-Cr wear particles will induce osteolytic cytokine production by human macrophages, but only at high volumetric concentrations. Unlike UHMWPE, Co-Cr partic

Metal-on-metal bearings and hypersensitivity in patients with artificial hip joints. A clinical and histomorphological study.

BACKGROUND: Some patients who have a total hip replacement with a second-generation metal-on-metal articulation have persistent or early recurrence of preoperative symptoms. Characteristic histological changes in the periprosthetic tissues suggested the development of an immunological response. Therefore, in order to determine the relevance of these symptoms, we performed a study of the clinical data and periprosthetic tissues associated with endoprostheses with a metal-on metal articulation that had been retrieved at revision. METHODS: Periprosthetic tissues as well as the clinical data on t

Serum levels of cobalt and chromium in patients with Sikomet metal-metal total hip replacements.

Metal/metal total hip replacements (M/M THRs) made of CoCr-based alloys were reintroduced in hip arthroplasty in order to avoid the problem of polyethylene wear and consequently to diminish the wear debris-related osteolysis. Emerging concerns accompanying the reintroduction of M/M THRs are related to the possibility increase of Co and Cr serum levels and, therefore, a careful surveillance of M/M patients is recommended. In the present work three groups of patients with Sikomet SM21 low-carbon alloy were investigated: retrospective group, prospective group and revision group. The investigation

Metal-on-metal resurfacing versus total hip replacement-the value of a randomized clinical trial.

This article describes a randomized clinical trial in young patients, comparing metal-on-metal cemented resurfacing hip replacement with cemented total hip replacement. The trial was stopped early, mainly because of a high incidence of failure of the cemented resurfacing acetabular component. The results reinforce the importance of clinical trials for evaluating the safety and efficacy of prosthesis designs before being used in a large cohort of patients. Although there may be advantages of resurfacing hip replacement, trials are also required to demonstrate it has a midterm success that reaso

Comparative study of maxillary complete dentures constructed of metal base and metal structure framework.

A removable denture designed using a three-dimensional cast metal framework (hereafter referred to as the 'structurally designed' denture) could extend denture longevity because it is unbreakable and easy to adjust. The aim of the present clinical study was to compare two types of maxillary removable dentures: conventional dentures and structurally designed denture. One edentulous and five partially dentate patients were fitted with two maxillary dentures made from the same impression and same occlusal relationship. About 20 days after delivery of the denture, masticatory analysis was conducte

Succimer-coated wooden spoons for in situ removal of heavy metals in food.

Heavy metals in food present significant health risks; however, consumer-level solutions for reducing exposure remain limited. This study presents a novel consumer-focused solution in which succimer (an FDA-approved chelation therapy) is stably grafted onto the surface of a wooden spoon. Using a standard coupling reaction, succimer is covalently attached to lignin in the wood matrix through ester bond formation. Succimer-functionalized spoons elicit improved metal retention compared to the pristine wooden spoon. To demonstrate the efficacy of this food contact device, we tested its use during …

Friend and Foe?: Metals as Facilitators and Disruptors of Steroid Nuclear Receptor Signaling.

Steroid hormone signaling, transduced by nuclear receptors (NRs), is essential for mammalian physiology. Like other transcription factors, N … mains as crucial mechanisms of metal-by-NR interactions. Subsequent molecular studies indicate that metals differentially affect NRs, largely driven by a metals' valence state (divalent, trivalent, etc). Some toxic metals can directly activate NRs.
