Computed tomography of celiac disease.

Celiac disease is an autoimmune disorder that causes inflammation and destruction in the small intestine in response to the ingestion of gluten and related proteins. Although long considered a pediatric condition, it is now recognized as a condition that affects all ages, with significant complications if it goes untreated. Adults are most likely to present for imaging without a diagnosis for unspecified abdominal symptoms. Imaging the small intestine is complicated. Although the advent of multidetector computed tomography (CT) helps, the relative newness of its use, combined with the relative…

Left sinus of valsalva aneurysm ruptured into left ventricle: A case report of 320-multidetector CT findings.

RATIONALE: Ruptured aneurysm originating from the left coronary sinus of Valsalva into the left ventricle (LV) is extremely rare. Imaging features of sinus aneurysm has been commonly reported using echocardiography or angiography. Here, we report multidetector computed tomography (MDCT) findings of left sinus of Valsalva aneurysm extending into the LV and caused severe aortic regurgitation (AR) in a 44-year-old male with latent infective endocarditis. The role of MDCT in preoperative surgical planning was also emphasized. PATIENT CONCERNS: The patient visited our hospital due to worsening exe…

Association between hypertension and coronary artery disease as assessed by coronary computed tomography.

Multidetector row computed tomography (MDCT) enables the accurate noninvasive assessment of coronary artery stenosis and plaque imaging. The characteristics of patients who have coronary artery disease (CAD) as assessed by MDCT coronary computed tomography (CT) are not well known. Participants consisted of 513 consecutive patients with suspected CAD who underwent coronary CT. The authors quantified patient characteristics, including the prevalence of hypertension (HTN), hyperlipidemia and diabetes mellitus (DM), visceral fat area (VFA) and subcutaneous fat area using CT, and plasma levels of m

Myocardial calcification and subendocardial fatty replacement of the left ventricle following myocardial infarction.

We report a 70 years old man with old myocardial infarction of the anteroseptal wall. He had an acute anterior myocardial infarction ten years ago and coronary angiography showed a total occlusion of the left middle anterior descending coronary artery. He underwent a multidetector cardiac computed tomography for rule out another coronary stenosis before the surgery of a postrenal aortic aneurysm. The unenhanced computed tomography image realized for the quantification of the coronary calcium reveals a low-attenuation curvilinear stripe consistent with subendocardial fat at the anterior ventric

Double S-Curve Versus Cusp-Overlap Technique: Defining the Optimal Fluoroscopic Projection for TAVR With a Self-Expanding Device.

OBJECTIVES: The purpose of this study was to assess the concordance between transcatheter aortic valve implantation angles generated by the "double S-curve" and "cusp-overlap" techniques. BACKGROUND: The "double S-curve" and "cusp-overlap" methods aim to define optimal fluoroscopic projections for transcatheter aortic valve replacement (TAVR) with a self-expandable device. METHODS: The study included 100 consecutive patients undergoing TAVR with self-expanding device planned by multidetector computed tomography. TAVR was performed using the double S-curve model, as a view in which both the a…

Age, Sex, and Valve Phenotype Differences in Fibro-Calcific Remodeling of Calcified Aortic Valve.

Background In calcific aortic valve disease on tricuspid aortic valves (TAVs), men have higher aortic valve calcification and less fibrosis than women. However, little is known in bicuspid aortic valves (BAV). We thus aimed to investigate the impact of age, sex, and valve phenotype (TAVs versus BAVs) on fibro-calcific remodeling in calcific aortic valve disease. Methods and Results We included 2 cohorts: 411 patients who underwent multidetector computed tomography (37% women) for aortic valve calcification density assessment and 138 explanted aortic valves (histological cohort; 50% women). The…

Cervical spine clearance in the adult obtunded blunt trauma patient: A systematic review.

BACKGROUND: A missed cervical spinal injury could have devastating consequences. Patients with a suspected cervical spinal injury are kept in rigid collars for cervical immobilisation. Prolonged collar use has important clinical implications. A well-defined guideline related to the removal of cervical collars from adult obtunded blunt trauma patients has not been developed. AIM: We sought to determine if Magnetic Resonance Imaging offered a definitive benefit over Computer Tomography with respect to patient management. METHOD: We searched Ovid Online, EBSCO, NICE Evidence Journals, Medline,

A Rare Case of Partial Anomalous Pulmonary Venous Connection.

Partial anomalous pulmonary venous connection (PAPVC), in which the right and left lower pulmonary veins drain into the coronary sinus (CS), is very rare, and only one case has been reported previously. The diagnosis of PAPVC is difficult, as the symptoms may be not specific. Multidetector computed tomography (MDCT) angiography and MRI help in the diagnosis of congenital cardiac anomalies. Our case, who underwent closure of atrial septal defect (ASD) 31 years prior, presented with chest tightness and palpitation. MDCT angiography revealed PAPVC with both lower pulmonary veins draining into the…

Successful preoperative identification of fish bone causing appendicitis using 3-dimensional multidetector-CT.

A 79 years male with fishbone-induced acute appendicitis was treated surgically with laparoscopic procedure. In preoperative diagnostic study, 3-dimensional multidetector-computed tomography (3D-MDCT) demonstrated the foreign body in the appendix and 3D-reconstructed images seemed fishbone with sharp-pointed dorsal fin directing to the tip of the appendix. With these findings, surgical indication of appendectomy was confirmed and laparoscopic appendectomy was performed. He made a full recovery and was discharged in a satisfactory condition following 7 days of post-operative treatment. From thi

Emergency surgery for acute cholecystitis associated with cholecystocolonic fistula caused by gallbladder cancer: a case report.

Preoperative diagnosis of cholecystocolonic fistula (CCF) is difficult and the contribution of gallbladder cancer or colon cancer is unclear when there is associated malignancy. We present a case that was diagnosed with acute cholecystitis associated with CCF by multidetector computed tomography (MDCT) preoperatively and malignant neoplasm during emergency surgery. She was finally diagnosed with gallbladder cancer after the operation and underwent a two-stage surgery for regional lymph node dissection. Gallbladder cancer can be a primary malignant cancer causing CCF, whereas MDCT is useful for…

Catheter ablation of persistent atrial fibrillation in a patient with cor triatriatum sinister demonstrating a total common trunk of the pulmonary vein.

A 57-year-old male with persistent atrial fibrillation (AF) was referred for catheter ablation. Multidetector computed tomography (MDCT) revealed that a membrane divided the left atrium into two chambers, thus indicating the presence of cor triatriatum sinister. A 3D image reconstructed by MDCT showed that the accessory atrium received the left common and the right side PVs, as if it were a total common trunk, and this then flowed into the main atrium. After isolation of the pulmonary vein and posterior wall from the left atrium, AF could not be induced by any programmed pacing. The patient ha…

An unusual cause of recurrent pancreatitis: A gastric duplication cyst with an accessory pancreatic lobe.

Congenital anomalies of pancreas and its ductal drainage are uncommon but in general surgically correctable causes of recurrent pancreatitis. A gastric duplication cyst communicated with an accessory pancreatic lobe is an extremely rare cause of recurrent pancreatitis, but an early and accurate diagnosis of this anomaly is important because suitable surgical treatment may lead to a satisfactory outcome. Herein, we presented multidetector computed tomography and magnetic resonance imaging findings of a gastric duplication cyst communicating with an accessory pancreatic lobe via an aberrant duct…
