Osteoarthritis: a problem of growth not decay?

The traditional view of OA is that it is primarily a disease of articular cartilage that results, by altering the biomechanics of the joint, in secondary changes to the subchondral bone and, through secondary inflammation, other joint tissues. This focus on cartilage tends to ignore other musculoskeletal changes reported, especially those remote from affected joints. It has been proposed instead that generalized OA is a systemic musculoskeletal disorder with a metabolic component. Evidence for this position will be presented by summarizing changes identified in all the major musculoskeletal ti

Histone Deacetylase 4: The Epigenetic Hub in Degenerative Musculoskeletal Diseases.

Aging has rendered Degenerative Musculoskeletal Diseases (DMDs) an increasingly significant public health burden. Growing evidence suggests that epigenetic dysregulation plays a crucial mechanistic role in the pathogenesis of DMDs. Histone Deacetylase 4 (HDAC4), known for its regulatory roles in the nervous, cardiovascular, and cancer systems, has also emerged as a central coordinator of musculoskeletal physiology and pathology. It dynamically modulates cellular metabolism in cartilage, bone, and muscle tissues. This review aims to provide a comprehensive overview of current research on HDAC4

Syphilitic arthritis and osteitis.

Syphilis again has become a significant clinical problem and clinicians must reacquaint themselves with the classic as well as the changing clinical manifestations, new diagnostic methods, interaction with HIV infections, and outcomes of therapy. Musculoskeletal manifestations can be associated with congenital, secondary, and tertiary syphilis and can mimic a wide variety of rheumatic and systemic diseases of worse prognosis. Musculoskeletal manifestations due to congenital and secondary syphilis usually subside completely after accurate diagnosis and antibiotic therapy.

Lyme disease: the latest great imitator.

Lyme disease is caused by the spirochete B. burgdorferi. Like its counterpart syphilis, it causes multisystem disease particularly affecting the skin, nervous system, heart and musculoskeletal system. It is endemic in several areas of the United States as well as in Europe. The prompt recognition of this disease and its diverse manifestations should lead to early treatment and resolution. Prevention is aimed at avoidance of the tick vector.

Regional pain syndrome: clinical characteristics, mechanisms and management.

Regional soft-tissue complaints are commonplace, and they usually relate to a disease process, such as strain, inflammation or degeneration of a muscle, tendon or related muscle-tendon unit. The clinical features and investigations of the causative processes of these complaints are characteristic, and outcomes to treatments are usually predictable and satisfactory. Regional pain syndromes are different: these syndromes present with regional pain and tenderness, and other sensory symptoms unaccounted for by a simple musculoskeletal mechanistic explanation. Approved classification criteria for r

Could serum HMGB1 levels be a predictor associated with psoriatic arthritis?

Background/aim: Psoriasis is a chronic autoimmune disease that predominantly affects the skin and musculoskeletal system. We hypothesized that HMGB1, an inflammatory nuclear protein, may play a role in the musculoskeletal involvement of psoriasis. Methods: Forty patients with psoriasis and 45 with psoriatic arthritis were involved in the study; the results were compared with 22 healthy controls. Serum HMGB1 levels were evaluated from peripheral blood samples. Results: Serum HMGB1 levels were found to be significantly higher in patients with psoriasis regardless of joint involvement (p < 0.001)…

Joint-Predominant Rheumatic Complications of Immune Checkpoint Inhibitor Therapy in Patients with Thymic Epithelial Tumors.

Immune checkpoint inhibitors (ICIs) have revolutionized the treatment of advanced cancers. However, activation of the immune system can occasionally cause life-threatening toxicity involving critical organs. Induction of immune-mediated toxicity is a significant concern for patients with thymic epithelial tumors (TETs) due to defects in immune tolerance. An increased risk of skeletal and cardiac muscle inflammation following treatment with ICIs is well recognized in patients with advanced TETs. However, uncommon musculoskeletal and rheumatic complications can also occur. The cases presented in

[Scanning Technique in Wrist and Hand Ultrasonography].

This guide to ultrasound examination of the wrist and hand describes the basic scanning planes and contains corresponding high-resolution ultrasound images. The patient and probe positionings are explained in a simplified step-by-step manner using schematic drawings. This standard and simple examination technique seems to be an appropriate way to improve the learning curve of novice musculoskeletal sonographers. In wrist section, an emphasis is placed on the carpal tunnel and extensor tendons evaluation. In the hand region, the scanning of the flexor pulley system and the flexor pollicis longu

[Bilateral femoral cephalic osteonecrosis in a human immunodeficiency virus-infected child taking HAART].

Musculoskeletal complications are uncommon in HIV-infected patients. In adults, one manifestation is osteonecrosis which can be associated with risk factors such as alcohol consumption, smoking or use of corticosteroids. Such risk factors are rare in children. Over the last decade musculoskeletal complications have been observed in patients with advanced-stage disease or whose immune system has been reconstituted with highly active antiretroviral therapy (HAART). This complication has been reported in adults but to date not in children. We present the case of a 5-year-old child with AIDS (C st

[Cardiac Perspective on Amyloidosis: Appropriate Cardiac Diagnostic Approaches Following Histological Amyloid Detection in Tendinopathies].

Amyloidosis is a rare, typically systemic disease that may cause progressive heart failure when cardiac involvement occurs. The two most common subtypes leading to cardiomyopathy - AL and ATTR amyloidosis - differ substantially in terms of diagnosis, treatment, and prognosis. Retrospective studies have shown that musculoskeletal manifestations such as carpal tunnel syndrome, stenosing tenosynovitis, Dupuytren's contracture, or tendon ruptures often occur years before the diagnosis of cardiac amyloidosis and may serve as early clinical markers. A risk-adapted, targeted histopathological work-up

Standardization of body weight distribution using a Force-Sensitive Resistor (FSR) matrix and foot analytics.

Balanced body weight distribution across both feet is essential for maintaining musculoskeletal health and minimizing postural and ambulatory issues. Conventionally foot weight distribution is measured using specialized clinical equipment which are expensive and limits routine screening. Therefore, in this work, we present a standardized foot weight distribution model having a custom 32×32 force-sensing resistor (FSR) matrix based embedded system. A total of 4,000 volunteers participated in the Physical Fitness Assessment (PFA). Of these, 2,200 who passed the musculoskeletal fitness criteria w

Relationship between tooth macrowear and jaw morphofunctional traits in representative hypercarnivores.

The mammalian adult dentition is a non-renewable resource. Tooth attrition and disease must be accommodated by individuals using behavioral, physiological, and/or musculoskeletal shifts to minimize impact on masticatory performance. From a biomechanical perspective, the musculoskeletal system becomes less efficient at producing bite force for a given amount of muscle input force over an individual's life, because tooth-food contact area increases as cusps wear. In this study we ask the question: does mandibular biomechanical performance show evidence of compensation with increasing tooth wear?…

To do one and to get more: Part I. Diabetes and bone.

Type 2 diabetes mellitus (T2DM), is a chronic metabolic disease, characterized by the presence of hyperglycemia and insulin resistance. The key treatment strategies for T2DM include modification of lifestyle, medications, and continuous glucose monitoring. DM patients often have DM-associated morbidities and comorbidities; however, disorders of musculoskeletal system are often neglected, compared to other major systems in DM patients. Based on sharing similar pathophysiology of DM and osteoporosis, it is supposed that the use of antidiabetic agents (ADAs) may not only provide the lowering gluc…

Musculoskeletal biomarkers in health and disease: implications for the aging spine-a review for spinal surgeons by the SRS adult spinal deformity task force on senescence.

The incidence of spine-related pathologies is expected to increase in developed countries due to ongoing fundamental demographic shifts toward an older population. These changes present significant challenges to public health, as healthcare systems worldwide must confront the burden of musculoskeletal aging and its related consequences. Here, we synthesize current knowledge on the biologic mechanisms underlying musculoskeletal aging, focusing on the implications for the aging spine. The complexity of the aging process, characterized by a convoluted interplay between genetic, environmental, and

A comparative probabilistic analysis of human and chimpanzee rotator cuff functional capacity.

Computational musculoskeletal modeling represents a valuable approach to examining biological systems in physical anthropology. Probabilistic modeling builds on computational musculoskeletal models by associating mathematical distributions of specific musculoskeletal features within known ranges of biological variability with functional outcomes. The purpose of this study was to determine if overlap in rotator cuff muscle force predictions would occur between species during the performance of an evolutionarily relevant horizontal bimanual arm suspension task. This necessitated creating novel p

Patterns of Co-Occurring Birth Defects in Chinese Infants With Congenital Diaphragmatic Hernia: A National Hospital-Based Surveillance Study.

BACKGROUND: The landscape of co-occurring birth defects among infants with congenital diaphragmatic hernia (CDH) remains underexplored. AIMS: This study aims to elucidate the complex patterns of co-occurring defects in Chinese population. MATERIALS AND METHODS: We analyzed cases from the Chinese Birth Defects Monitoring Network (2007-2019) with CDH that presented along with at least one additional defect but without a syndromic diagnosis. Adjusted observed-to-expected prevalence (O/E) ratios were calculated for all two- to five-way combinations to discern patterns of co-occurring defects wit

Balancing benefits and risks of aerobic exercise for aging and musculoskeletal health.

BACKGROUND: Long-term exercise is recognized as one of the most effective means of maintaining health after aging, but the relationship between moderate exercise and health in the older population is often overlooked. OBJECTIVE: The present study was conducted to investigate the effects of excessive endurance exercise on the old mice musculoskeletal system. RESULTS: The 8 weeks of normal endurance exercise significantly improved skeletal muscle mitochondrial biogenesis and increased femoral osteogenesis in young and old mice. However, the continued accumulation of total exercise volume as th…
