Hair and nails are skin appendages that share with other ectodermal tissues a common developmental pathway. Inherited disorders affecting these two structures therefore very often involve other epithelial components and present with multiple anomalies, generating both physical and psychological distress among patients and their families. The present review briefly describes major recent advances in our understanding of hair and nail genodermatoses.

We present the cases of two patients diagnosed with non-microcytic lung carcinoma who were treated with epothilone, a new chemotherapeutic agent which disrupts microtubule dynamics. The first one developed nail symptoms consisting of painful nails with a hemorrhagic appearance, which developed into significant structural disorders and onycholysis of all of the fingernails. In case of suspicion of a deep mass beneath the nail plate, radiographs remain necessary to assess the distal phalanx and the distal interphalangeal joint. Ultrasonography may be helpful to depict a cystic or a vascular component of a tumor. Computed tomography is only indicated to accurately assess tiny abnormalities of the cortex of the distal phalanx. MR imaging offers a whole imaging of the soft parts of the nail unit and the underlying bone.

Onychomatricoma is a rare fibroepithelial lesion of the nail matrix with peculiar clinical and histological features. Clinically, it is characterized by a longitudinal band of yellow thickening of the nail plate with transverse overcurvature and splinter hemorrhages. We present four cases and a review of the literature regarding nail-gun related limb injury. Nail guns have significant potential to cause limb and other major injuries. In most cases improper use of the nail-gun is the single most important factor in causing such types of injuries. Treatment of these injuries requires careful assessment of the limb and the type of nail involved in order to enable safe extraction. We recommend the introduction of training in the workforce to encourage awareness of the danger of such devices. We also emphasize the continuing requirements for improved workplace

Although many consider the fingertip injury a very mundane and simple problem, the mismanagement of this injury can often lead to a chronic esthetic and possibly painful deformity. As noted in this article, the appropriate understanding of the involved structures of a fingertip injury as well as the meticulous management of these injuries can generally lead to a very esthetic and functional fingertip. As previously noted by Elvin Zook, there is probably nowhere that Meskimen's Law, "There is never time to do it right but there is always time to do it over," is more true.

There are many causes for nail deformities, the most common being the result of trauma or pressure caused by infections, tumors, congenital anomalies, and irregularity of the underlying bone or unknown causes such as the pincer nail. The corrective treatment of the more common of these will be discussed in this and other articles. Examples of nail reconstructions have been described in the 1990 Hand Clinics and as a rule will not be repeated here.

Nail involvement in pemphigus vulgaris is rare. We describe 5 patients with pemphigus vulgaris presenting nail involvement. In this disease, nail manifestations present, by order of frequency, as chronic paronychia, onychomadesis, onycholysis, Beau's lines and trachyonychia. All our 5 cases presented with paronychia, and 1 of them also had Beau's lines. Treatment with prednisone and/or cyclophosphamide controlled mucocutaneous and nail manifestations in all cases.

Therapeutic ablation of nail is necessary in a variety of settings. The challenge is to achieve destruction of nail matrix with minimal pain and to ensure that there is no re-growth of nail. There are many different methods of nail ablation, with excisional surgery and phenolic ablation of matrix representing the most common. The balance of published data favours phenolic ablation as a simple procedure with low morbidity and the least chance of recurrence.
