Early rehabilitation of victims of traumatic ulnar injury using tendon transfer.

BACKGROUND: Poor awareness among the patients and lack of resources for proper management of nerve injuries leads to delayed presentation of most traumatic ulnar nerve injuries. When this injury is present in the proximal forearm it leads to poor outcomes as Ulnar nerve repair with grafts takes a prolonged time to restore function. Addition of a tendon transfer to this procedure can allow patients to return to work earlier. The purpose of this study is to examine whether the addition of a tendon transfer to nerve repair surgery will lead to improved hand function and rehabilitation earlier tha

Rare median and musculocutaneous nerve fusion with intraoperative electrical confirmation: illustrative case.

BACKGROUND: Nerve transfer is a surgical technique in which a redundant or expendable fascicle is transferred or coapted to an injured nerve distal to the site of injury for the purpose of reinnervation. Successful nerve transfer is dependent on correct intraoperative identification of donor and recipient nerves. OBSERVATIONS: An 8-year-old male was recommended for ulnar nerve fascicle to biceps branch of musculocutaneous nerve transfer to restore elbow flexion weakness after a demyelinating spinal cord injury. The biceps branch was identified approximately midway along the upper arm. Proxima…

Evidence of vagus nerve sprouting to innervate the urinary bladder and clitoris in a canine model of lower motoneuron lesioned bladder.

AIMS: Complete spinal cord injury does not block perceptual responses or inferior solitary nucleus activation after genital self-stimulation, even though the vagus is not thought to innervate pelvic structures. We tested if vagus nerve endings sprout after bladder decentralization to innervate genitourinary structures in canines with decentralized bladders. METHODS: Four reinnervation surgeries were performed in female hounds: bilateral genitofemoral nerve transfer to pelvic nerve with vesicostomy (GNF-V) or without (GFN-NV); and left femoral nerve transfer (FNT-V and FNT-NV). After 8 months,…

Current Procedure of Double Free Muscle Transfer for Traumatic Total Brachial Plexus Palsy.

INTRODUCTION: We describe the current procedure of not only double free muscle transfer but also supplemental techniques including nerve transfer for shoulder and elbow reconstruction and secondary surgery for the wrist and fingers to improve prehensile function following traumatic total brachial plexus palsy1-4. STEP 1 PREOPERATIVE PLANNING: Coronal and transverse MRIs and intraoperative electrical stimulation are useful for nerve-root evaluation. STEP 2 RECONSTRUCTION OF SHOULDER FUNCTION STAGE I: If the nerve gap is <10 cm, use the sural nerve as an interpositional graft; if the nerve gap…

A Prospective Randomized Comparison of Median and Ulnar Fascicle Transfers to the Biceps Motor Branch in Upper Brachial Plexus Injury.

BACKGROUND: Transfer of an ulnar nerve fascicle to the motor branch of the biceps has been widely adopted in patients with upper brachial plexus injuries, with consistently favorable outcomes. Transfer of a median nerve fascicle has also been reported to yield satisfactory results. This study aimed to compare the outcomes of median and ulnar nerve fascicle transfer to the biceps in patients with C5-C6 and C5-C7 brachial plexus injuries. METHODS: A total of 50 consecutive patients underwent surgery between June 2013 and December 2018. The donor nerve was randomized before surgery. After exclud

Assessment, management, and rehabilitation of traumatic peripheral nerve injuries for non-surgeons.

Electrodiagnostic evaluation is often requested for persons with peripheral nerve injuries and plays an important role in their diagnosis, prognosis, and management. Peripheral nerve injuries are common and can have devastating effects on patients' physical, psychological, and socioeconomic well-being; alongside surgeons, electrodiagnostic medicine specialists serve a central function in ensuring patients receive optimal treatment for these injuries. Surgical intervention-nerve grafting, nerve transfers, and tendon transfers-often plays a critical role in the management of these injuries and t…

Facial reanimation: evolving from static procedures to free tissue transfer in head and neck surgery.

PURPOSE OF REVIEW: The purpose of this article is to review and evaluate the surgical options for treating patients with facial paralysis, covering primary neurorrhaphy to facial reanimation, with microvascular free tissue transfer. RECENT FINDINGS: In recent years, free tissue transfer has been increasingly common for rehabilitating the paralyzed face, providing a more dynamic and aesthetic outcome, than has been possible prior to microvascular surgery in facial plastic and head and neck surgery. SUMMARY: Although primary facial nerve repair attains the best results, nerve grafting with the

Secondary surgery following brachial plexus injuries.

The favourable treatment of post-traumatic brachial plexus lesions based on our experience of 362 cases over a 12 year period is reported. Twenty-five percent of the patients needed secondary operations. The spectrum of the latter consisted of arthrodesis, tenodesis, and musculotendinous transfer, including free neurovascular tissue transfer partially innervated by nerve transposition. Functionally, secondary tendon transfer can help to improve the effect of nerve repair techniques. To restore shoulder function the trapezius transfer (n = 22) has been used mainly; elbow flexion has been regain

Five Operations That Give the Best Results after Brachial Plexus Injury.

Treatment of brachial plexus injuries has improved slowly over the past 45 years. Changes in strategy, techniques, microsurgical equipment, and technology have expanded the surgical options for reconstructing these life-altering, highly complex injuries. The surgical techniques available include neurolysis, nerve repair, nerve grafting, nerve transfers, tendon transfer, muscle transfer, and other soft- and bony-tissue procedures. In this article, the authors have selected five surgical procedures (i.e., Oberlin procedure, Leechavengvongs procedure, free functional muscle transfer, radial nerve…

Significance of shoulder function in the reconstruction of prehension with double free-muscle transfer after complete paralysis of the brachial plexus.

Reconstruction of shoulder stability and movement in cases with complete paralysis of the brachial plexus was performed to improve the outcomes for universal function of prehension after double free-muscle transfer (Doi's procedure). In cases in which the C5 or C6 nerve root was available as a donor, neurotization of the supra-scapular nerve was performed with a nerve graft. If the C5 or C6 nerve root was not available, then the contralateral C7 nerve root was chosen as the donor motor nerve and was transferred to the suprascapular nerve by using a vascularized ulnar nerve graft. Seven cases w

Long-term functional results of contralateral C7 transfer.

From 1986 to 1994, 82 patients with brachial plexus root avulsion were operated on using a contralateral C7 nerve-root transfer. Forty-four patients underwent a one-stage procedure in which the distal end of the ulnar nerve was anastomosed to the contralateral C7 root, and the other 38 underwent a two-stage procedure (first phase: C7 root --> ulnar nerve; second phase: ulnar nerve --> recipient nerve). Twenty postoperative cases were followed-up for 2 years. Of them, the ulnar nerve was transferred to the musculocutaneous nerve in six cases, with recovery of the biceps up to M3 in four and S3

Peripheral pathway rerouting shapes whole-brain functional network.

Pathway rerouting is critical for severe peripheral nerve injury (PNI), which induces widespread neuroplastic alterations within the central nervous system, yet whether different rerouting approaches differentially affect whole-brain functional networks remains unknown. The present study investigated alterations in brain network organization following different peripheral pathway rerouting after PNI. Adult Sprague-Dawley rats underwent complete left brachial plexus transection and were assigned to nerve repair, end-to-end nerve transfer, end-to-side nerve transfer, or untreated PNI groups. Res…

Reconstructive surgery in upper limbs in leprosy.

Of all the deformities in leprosy, 80% require minor surgery and the rest major surgery. Upper and lower limbs and face are mainly affected by deformities. Either the median or ulnar nerve and rarely the radial nerve are involved in isolation or in combination. Tendon transfer is the only option available in cases of deformities like simian hand, ulnar claw hand, wrist drop and failure to the metacarpals. The only motor function to be restored in median nerve palsy is opponensplasty. For radial nerve palsy standard FCU transfer, FDS transfer and FCR transfer are the reconstructive methods. For

Entinostat, a histone deacetylase inhibitor, increases the population of IL-10+ regulatory B cells to suppress contact hypersensitivity.

Facial palsy is a life-changing disability. This can have an added impact on patients undergoing cancer surgery, both physically and mentally. Following their cancer treatment, most patients want to get back to normality and have a meaningful life filled with laughter, even if it is in their last remaining days. Masseteric nerve transfer has a vital role to bring back those smiles in their lives with minimal morbidity. Age, radiation and duration of palsy are often considered to have a negative impact on nerve regeneration. Facial nerve resection is sometimes required for cancer clearance, par…

Anatomic evaluation of the subcoracoid pectoralis major transfer in human cadavers.

Subcoracoid transfer of the pectoralis major has recently been described as a reconstruction for subscapularis insufficiency. The purpose of this study was to examine the surgically relevant anatomy of this transfer. The importance of understanding this anatomy was recently highlighted to us following our encounter with musculocutaneous neuropraxia in 2 patients after transfer of the entire pectoralis major, one deep to the musculocutaneous nerve. Dissections were performed on 20 fresh, whole human cadavers in which the entire pectoralis major muscle, medial and lateral pectoral nerves, and mu
