Nerve conduction studies and its importance in diagnosis of acute poliomyelitis.

Motor nerve conduction velocities was performed on 50 subjects in the pediatric age group. Thirty two patients with acute poliomyelitis and 18 controls. The MNCV was studied in the median nerve in the upper limb and the posterior tibial in the lower limb. The motor nerve conduction velocity in polio patients matched well with the controls, as well as within the accepted standards for normal. The MNCV of the median nerve ranged from 41.8 +/- 2.76 m/sec in under 1 year to 44 +/- 2.1 m/sec in 3-8 years, in polio patients, while the range in controls varied from 37 to 53 m/sec. Similarly, for the

Peripheral neuropathy in patients with beta-thalassaemia.

As some patients with beta-thalassaemia manifested neurological signs, clinical and electrophysiological investigations were carried out on 53 thalassaemic patients and 29 healthy control subjects. Twenty per cent of the patients showed clinical and electrophysiological findings of a mild peripheral sensorimotor neuropathy, mainly of the lower limbs. The clinical symptoms were numbness, pins and needles sensations, muscular cramps, myalgia and muscle weakness. The electrophysiological abnormalities were manifested by decreased motor conduction velocity (MCV) and prolonged F-wave latencies of t

AAEM case report #23: acute paralytic poliomyelitis.

A 56-year-old man with acute paralytic poliomyelitis is described. The illness started with fever and diarrhea after an overseas trip, and an enterovirus other than poliovirus was isolated from the patient's stool. The onset of weakness was rapid and asymmetric, with primary involvement of the lower extremities. Nerve conduction studies revealed low amplitude motor responses after the first week, with normal results for sensory studies. Serial electromyographic studies were performed, documenting acute denervation followed later by reinnervation in the distribution of multiple segments. The cl

Importance of compression onset rate for the degree of impairment of impulse propagation in experimental compression injury of the porcine cauda equina.

The effects of a rapid (0.05-0.1 seconds) and a slow (20 seconds) onset rate of cauda equina compression on impulse propagation, in terms of tail muscle electromyogram (EMG) amplitude, were analyzed in an experimental model of the pig cauda equina. Sham compression and compression at 50 mm Hg at either rapid or slow onset induced no or only minor functional impairment. Compression at 100 or 200 mm Hg induced impairment of the impulse propagation that was proportional to the applied pressure. The effects were more pronounced (P less than 0.01) for the rapid than for the slow onset of compressio

Nerve-roots of the cauda equina. The effect of hypotension and acute graded compression on function.

The influence of hypotension on the function of nerve-roots of the cauda equina under acute graded compression was studied with use of an established porcine model. In twenty adult miniature pigs, the nerve-roots of the cauda equina were compressed at pressures of zero (sham), fifty, 100, or 200 millimeters of mercury, and hypotension was induced with sodium nitroprusside. Compression was maintained for two hours, and a ninety-minute period of recovery followed. The nerve-roots were monitored electrophysiologically throughout the experiment. To assess the effect of compression and hypotension

Nerve conduction studies and their clinical applications.

Nerve conduction studies provide the most sensitive and accurate account of peripheral neuropathophysiology. Though nerve conduction studies should never be performed in lieu of a clinical examination, the electrodiagnostic evaluation provides an optimal adjuvant to the clinical examination. Indications for nerve conduction studies are many, including evaluation of the nature of the pathophysiology, quantification of the severity of involvement, detection of the level of a neurologic deficit, and determining prognosis. Among other indications, limitations of the study should also be acknowledg

[Chronic demyelinating polyradiculoneuropathy associated with malignant histiocytosis].

A case of malignant histiocytosis (MH) presenting with peripheral nerve involvement is described. A 67-year-old man initially noted left facial weakness on July 20, 1986. The symptom was improved within a week, however, two weeks later he noticed numbness in his fingers and a burning pain in his legs, which was followed by double vision and progressive weakness in all four limbs. On examination, he was found to have total ophthalmoparesis of the right eye and a trace of the left facial palsy. There was severe weakness of the leg muscles, as well as mild weakness of the arms. This was accompani

Upper extremity tourniquet effects in carpal tunnel release.

We studied the presence and extent of upper extremity denervation in limb muscles after surgical release of the carpal tunnel. Sixty consecutive patients were randomly assigned to have the procedure performed with (31 patients) or without (29 patients) tourniquet application. Cervical paraspinal and upper extremity muscles were examined electromyographically 3 weeks before and 3 weeks after carpal tunnel release. Three weeks after the operation 77% of the patients in the tourniquet group had denervation in other than thenar muscles. Only one patient in the control (no tourniquet) group had sim

Neurologic symptoms posing as dapsone-induced polyneuropathy in two patients with dermatitis herpetiformis.

The clinical picture of neuropathy caused by dapsone is usually progressive muscle weakness and wasting, most often involving distal muscles of the extremities. Two patients with neurologic symptoms following the use of dapsone for dermatitis herpetiformis are reported. Both patients had normal motor conduction velocities, and negative results on physical examination of muscle wasting. We conclude that their clinical presentation was not due to dapsone-induced neuropathy. A discussion and a review of cases of this unusual effect of dapsone are included.

[Post-radiation brachial plexopathy. Persistent conduction block. Myokymic discharges and cramps].

The distinction between radiation and tumor brachial plexopathy may be difficult. The electrophysiological recording of myokymic discharges, frequently present in the former but rare in the latter type of plexopathy, can be helpful for the diagnosis. However, the pathophysiology and the site of origin of these discharges remain unclear. We describe a patient presenting with radiation brachial plexopathy, clinical myokymia, cramps and pain. In this patient, the myokymia--due to abundant myokymic discharges--and the cramps, were related to the existence of persistent conduction block of several

Carpal tunnel syndrome. A scientific basis for clinical care.

The series of experimental studies reviewed in this article supports a classification of median nerve compression at the wrist into early, intermediate, advanced, and acute nerve compression. The findings correlate well with experimental studies on the pathophysiology of nerve compression. A patient-specific management approach is recommended based upon the clinical and electrophysiologic findings which can be correlated with previously demonstrated intraneural pathologic changes.

Penile electrodiagnosis: penile peripheral innervation.

Many electrophysiologic tests have been performed and have proved useful for diagnosing sexual function. However, there have been limitations to the interpretation of the tests because of paucity of details concerning penile innervation. Therefore, electrophysiologic studies were performed in 3 patients with psychogenic impotence and in 15 patients with organic impotence to assess accurately the peripheral somatic innervation of the penis. The glans penis was innervated by the dorsal nerve of the penis. In some patients dual innervation was present at the ventral aspect of the glans penis by t

Electrophysiological study on limb myokymia in three women.

Physiological studies on three women with limb myokymia were carried out. The patients had diabetic neuropathy, neuromyotonia, and autonomic polyneuropathy, respectively. The EMG discharge pattern, coincident with myokymia, in a patient with myokymia and neuromyotonia differed from those with myokymia without neuromyotonia. In only the first patient did the electrical stimulation of nerves evoke "late repetitive response" (LRR), which resembled the wave forms of the myokymic discharge. Epidural and peripheral nerve blocks abolished myokymia in the first and second cases, but peripheral nerve b

Electrodiagnostic localization of traumatic upper trunk brachial plexopathy.

Eighteen patients having traumatic upper trunk brachial plexopathy ("the stinger"), a common football injury, were investigated electrodiagnostically. Proximal nerve conduction was determined by stimulating the supraclavicular fossa and at the C5 root and recording from muscles supplied by the long thoracic, suprascapular, musculocutaneous, axillary, lateral pectoral and thoracodorsal nerves. The accessory nerve was stimulated in the lateral posterior triangle and the evoked potential recorded from the upper trapezius muscle. Median and ulnar nerves were also tested, sensory and motor fibers b

Acute Bell's palsy: prognostic value of evoked electromyography, maximal stimulation, and other electrical tests.

Thirty-seven patients with acute Bell's palsy who had complete unilateral facial paralysis were selected for this study. Evoked electromyography, conduction latency, electromyography, and nerve excitability testing was done by one author while the maximal stimulation test was done by another on each patient. The patients were examined within ten days of onset of facial paralysis and evaluated six months after onset to determine the degree of recovery of facial motor function. The results of the tests were correlated with the degree of recovery of facial motor function in each patient. Evoked e

Somatosensory evoked potentials from stimulation of cutaneous femoris lateralis nerve and their application in meralgia paresthetica.

A New technique for recording somatosensory evoked potentials after stimulation of the cutaneous femoris lateralis nerve is described. It has proven useful in two patients where the diagnosis of meralgia paresthetica was raised. The use of this relatively easy technique is suggested in the diagnosis of meralgia paresthetica.

Median nerve compression in the carpal tunnel--functional response to experimentally induced controlled pressure.

Controlled external compression was applied to the medium nerve of 16 volunteer subjects. Tissue fluid pressure in the carpal canal was monitored with a wick catheter and pressures of 30, 60 and 90 mm Hg were induced for periods varying from 30 to 90 minutes.l Sensory and motor conduction and two-point discrimination were continuously monitored. Tissue compression at 30 mm Hg caused mild neurophysiological changes and symptoms of hand paresthesias. Compression at both 60 and 90 mm Hg induced a rapid, complete sensory conduction block which consistently preceded a motor block by 10 to 30 minute

Pernicious anemia seen initially as orthostatic hypotension.

A 69-year-old man complaining of syncope was found to have pernicious anemia and orthostatic hypotension due to autonomic neuropathy. Following vitamin B12 replacement with cyanocobalamin, the blood pressure became normal. To our knowledge, this is the first reported case of neurogenic orthostatic hypotension as the initial feature of vitamin B12 deficiency.
