Altered Neurocritical Care Management of Patients with Severe Traumatic Brain Injury Following Changed Positions of the Zero-Reference Points for Intracranial and Arterial Pressure Measurement.

BACKGROUND: Many traumatic brain injury (TBI) treatment protocols, including the Lund concept, advocate the highest point of the subarachnoid space (typically the vertex) as the zero-reference point for intracranial pressure (ICP) and the level of the right atrium as the zero-reference point for mean arterial blood pressure (MAP). In 2017, at the Department of Neurosurgery in Lund, Sweden, the zero-reference points for ICP and MAP were both changed to the external auditory meatus (EAM), thus altering the calculated cerebral perfusion pressure (CPP) levels. We hypothesized that the ICP and MAP

The unexpected paradox of geriatric traumatic brain injury outcomes: Uncovering racial and ethnic disparities.

BACKGROUND: Healthcare disparities have always challenged surgical care in the US. We aimed to assess the influence of disparities on cerebral monitor placement and outcomes of geriatric TBI patients. METHODS: Analysis of 2017-2019 ACS-TQIP. Included severe TBI patients ≥65 years. Patients who died within 24 h were excluded. Outcomes included mortality, cerebral monitors use, complications, and discharge disposition. RESULTS: We included 208,495 patients (White = 175,941; Black = 12,194) (Hispanic = 195,769; Non-Hispanic = 12,258). On multivariable regression, White race was associated with

Tetanic stimulation of the pudendal nerve prior to transcranial electrical stimulation augments the amplitude of motor evoked potentials during pediatric neurosurgery.

OBJECTIVE: Reportedly, tetanic stimulation prior to transcranial electrical stimulation (TES) facilitates elicitation of motor evoked potentials (MEPs) by a mechanism involving increased corticomotoneuronal excitability in response to somatosensory input. However, the posttetanic MEP following stimulation of a pure sensory nerve has never been reported. Furthermore, no previous reports have described posttetanic MEPs in pediatric patients. The aim of this study was to investigate the efficacy of posttetanic MEPs in pediatric neurosurgery patients and to compare the effects on posttetanic MEP a

Low-Dose of Rocuronium During Thyroid Surgery: Effects on Intraoperative Nerve-Monitoring and Intubation.

BACKGROUND: Intraoperative Neurophysiological Monitoring (IONM) reduces the incidence of Recurrent Laryngeal Nerve (RLN) injuries during thyroid surgery. To preserve nerve function, long acting neuromuscular blocking agents (NMBA) should be avoided. However, NMBA are necessary for laryngoscopy and endotracheal intubation. We designed this double-blinded, randomized, placebo-controlled trial to assess if a low-dose of rocuronium given at intubation would affect the IONM data recorded before the thyroid dissection. METHODS: Hundred patients undergoing elective thyroid surgery were randomized to…

Continuous neuromonitoring during radiofrequency ablation of benign thyroid nodules provides objective evidence of laryngeal nerve safety.

INTRODUCTION: The recurrent laryngeal nerves(RLN) run immediately posterior to the thyroid capsule and could be injured during thyroid radiofrequency ablation(RFA). This study assesses whether RLN functional integrity is altered during RFA using continuous intraoperative neuromonitoring(CIONM). METHODS: Prospective case series of twenty nodules treated with RFA under general anesthesia utilizing the laryngeal adductor reflex(LAR) for CIONM. RESULTS: Thirteen nodules abutted the posterior thyroid capsule and 'danger triangle' for RLN injury. The ablative field did not breach the posterior cap

Effect of neck extension on ultrasonographic optic nerve sheath diameter as a surrogate for intracranial pressure in patients undergoing palatoplasty: A prospective observational study.

Palatoplasty is performed with neck extension in patients with a cleft palate. The neck extension required for a better surgical view during palatoplasty can affect intracranial pressure. We evaluated the effect of neck extension on intracranial pressure by measuring the optic nerve sheath diameter using ultrasonography during palatoplasty. The optic nerve sheath diameter was measured in 30 patients at 10 min after anesthetic induction in the supine position (T1), at 10 min after neck extension before preparing for a sterile field (T2), at the end of surgery with neck extension (T3), and at 10

Intraoperative Nerve Monitoring for Tarsal Tunnel Decompression: A Surgical Technique to Improve Outcomes.

The aim of the current study was to evaluate the effectiveness of intraoperative neuromonitoring (INM) as an adjunct in performing tarsal tunnel decompression surgery. We reviewed 38 patients who met inclusion criteria. INM was used to measure the voltage of the abductor hallucis and digiti quinti muscles both before and after decompression. Observed changes intraoperatively were acute and within minutes of the decompression performed by the surgeon. Patient outcomes were ascertained from clinical findings and classified as excellent, fair, or poor. Patient outcomes and the voltage change were

Preserving executive functions in nondominant frontal lobe glioma surgery: an intraoperative tool.

OBJECTIVE: The goal of surgery for gliomas is maximal tumor removal while preserving the patient's full functional integrity. At present during frontal tumor removal, this goal is mostly achieved, although the risk of impairing the executive functions (EFs), and thus the quality of life, remains significant. The authors investigated the accuracy of an intraoperative version of the Stroop task (iST), adapted for intraoperative mapping, to detect EF-related brain sites by evaluating the impact of the iST brain mapping on preserving functional integrity following a maximal tumor resection. The role of intraoperative nerve monitoring in tracheal surgery: 20-year experience with 110 cases.

PURPOSE: To determine the utility of intraoperative nerve monitoring (IONM) during tracheal resection or slide tracheoplasty to prevent recurrent laryngeal nerve injury. METHODS: 110 patients underwent tracheal resection or tracheoplasty between 1997 and 2016. During the first 10-year period, IONM was not used while during the second 10-year period, IONM was used. 49 patients had surgery without IONM while 61 had surgery with IONM. During the post-operative period, patients with nerve injury were compared to determine if significant difference existed between the two modalities. RESULTS: In

Targeted Epidural Blood Patch Under O-Arm-Guided Stereotactic Navigation in Patients with Intracranial Hypotension Associated with a Spinal Cerebrospinal Fluid Leak and Ventral Dural Defect.

OBJECTIVE: Targeted epidural blood patch (EBP) at the site of a presumed cerebrospinal fluid leak reportedly has better outcomes than non-targeted EBP; however, it is associated with a higher risk of wrong-site injection, such as iatrogenic subarachnoid or intramuscular injections, which lead to reintervention because of the insufficient coverage of injected blood. METHODS: Eight patients with intracranial hypotension owing to a CSF leak diagnosed with myelographic computed tomography (CT) and thin-cut magnetic resonance imaging (MRI) received an epidural blood patch under O-arm-guided stereo

Feasibility of eliciting the H reflex in the masseter muscle in patients under general anesthesia.

OBJECTIVE: To explore the feasibility of eliciting the brainstem H reflex in the masseter muscle in patients under general anesthesia. METHODS: We electrically stimulated the masseteric nerve, a branch of the trigeminal nerve, and recorded ipsilateral masseteric and temporalis muscle responses. We tested eight patients who presented with trigeminal neuralgia; one patient had a temporal bone tumor and one patient had a brainstem arteriovenous malformation. All responses were elicited when patients were under general anesthesia and before the initiation of surgery. RESULTS: The H reflex in the

Combined use of intraoperative indocyanine green and dynamic angiography in rotational vertebral artery occlusion.

Rotational vertebral artery occlusion, also known as bow hunter's syndrome, is a well-documented surgically amenable cause of vertebrobasilar insufficiency. Traditionally, patients have been imaged using dynamic rotational angiography. The authors sought to determine whether intraoperative indocyanine green (ICG) angiography could reliably assess the adequacy of surgical decompression of the vertebral artery (VA). The authors report two patients who presented with multiple transient episodes of syncope provoked by turning their head to the right. Rotational dynamic angiography revealed a domin

Propofol as a substitute for amobarbital in Wada testing.

We describe a patient with equivocal findings on functional MRI (fMRI), who underwent a propofol Wada test, review the literature on this topic and suggest a protocol for the use of propofol for a Wada test. Although fMRI techniques can usually accurately lateralize language, the Wada test remains the gold standard for preoperative lateralization and is occasionally still required if there are non-diagnostic findings on fMRI. Amobarbital, the agent of choice for the Wada test, has become increasingly difficult to obtain and requires regulatory approval, which may delay definitive management an

Safety of staged epilepsy surgery in children.

BACKGROUND: Surgical resection of epileptic foci relies on accurate localization of the epileptogenic zone, often achieved by subdural and depth electrodes. Our epilepsy center has treated selected children with poorly localized medically refractory epilepsy with a staged surgical protocol, with at least 1 phase of invasive monitoring for localization and resection of epileptic foci. OBJECTIVE: To evaluate the safety of staged surgical treatments for refractory epilepsy among children. METHODS: Data were retrospectively collected, including surgical details and complications of all patients

Preliminary Evidence of Inhibitory and Error-Related Neural Alterations Associated with Early Relapse in Cocaine Use Disorder: A Retrospective Go/No-Go Event-Related Potential Study.

INTRODUCTION: Cocaine use disorder (CUD) is associated with impairments in inhibitory control and error-monitoring, two neurocognitive mechanisms strongly linked to relapse. Characterizing how these processes differ at treatment entry between patients who later remain abstinent versus those who relapse may help clarify the neurophysiological factors associated with early recovery. METHODS: Thirty-one CUD patients undergoing a 15-day inpatient detoxification program were included and classified as abstainers (n = 16) or relapsers (n = 15) based on drug use reports 1 month after discharge. Aberrant error monitoring in traumatic brain injuries: A meta-analysis of event-related potential studies.

OBJECTIVE: Although individuals with traumatic brain injuries (TBI) often manifest altered error monitoring, evidence using event-related potentials (ERPs) to index these cortical processes is inconsistent. Therefore, this meta-analysis study aimed to comprehensively compare the error-related negativity (ERN) and error positivity (Pe) between individuals with TBI and healthy controls (HC) from the existing literature. METHODS: Literature search was performed using PubMed/MEDLINE, Web of Science, and Cochrane Library. The effect sizes (Hedges' g) in the comparisons of ERN and Pe amplitudes bet

Value of Intraoperative Neuromonitoring in Spinal Tuberculosis Surgery: A Case Report of Neurological Compromise with Normal Imaging.

Spinal tuberculosis (TB), also known as Pott's spine, remains a significant cause of spinal deformity and neurological compromise, particularly in endemic regions. Surgical correction of chronic deformities is associated with significant risks, including spinal cord injury. Intraoperative neurophysiological monitoring (IONM) has become an essential tool to minimize neurological complications. We report a case of an adolescent boy with longstanding thoracolumbar kyphosis due to spinal TB, where IONM identified true positive motor pathway compromise during deformity correction, despite intraoper…
